Assess an AI tool with a documented, lifecycle-based go/no-go process—not a vendor questionnaire alone. Define the intended use, users, affected people, data flows, model or provider, foreseeable misuse and accountable owner. Then map legal duties, identify harms, test realistic and adverse cases, select controls, record residual risk and approval conditions, and set monitoring and rollback triggers. Reopen the assessment when the model, data, purpose, users, environment or incidents change. No universal score proves that an AI system is safe to deploy; the acceptable threshold depends on the use case, affected groups, jurisdiction and evidence.
What should an AI risk assessment establish before deployment?
A useful assessment answers four questions: what the system is supposed to do, what could go wrong, how well controls reduce those risks, and who is authorized to accept what remains. Treat the document as a decision record that follows the system from procurement through retirement.

NIST AI RMF 1.0, published January 26, 2023, organizes this work into Govern, Map, Measure and Manage. It is voluntary, use-case agnostic and adaptable, not a legal safe harbor or a universal checklist. Its companion Playbook supplies prompts and suggested actions, but NIST explicitly says it is not a one-size-fits-all resource or an ordered implementation sequence.
“The AI RMF is intended to be voluntary, rights-preserving, non-sector specific, and use-case agnostic,” NIST says.

How do I define the use case and accountability?
Start with a one-page system description that another team could use to understand the deployment without speaking to the project founder. Ambiguous purpose creates ambiguous testing and makes later incidents difficult to assign.
Record the system’s identity and operating context
- System name, supplier or model provider, model name and version, release date and planned change process.
- Intended purpose and measurable outcome; prohibited or out-of-scope uses.
- Who operates it, who receives its outputs and who can act on them.
- People and communities affected directly or indirectly, including customers, employees, applicants, borrowers, patients or members of the public.
- Deployment geography, language, industry and relevant regulatory regimes.
- Inputs, outputs, data retention, external services, APIs, plug-ins and downstream systems.
- Whether the output recommends, ranks, predicts, generates content, takes an action or makes a decision.
Assign decision rights
Name one accountable owner, a technical owner and the people responsible for legal, privacy, security and operational review. Write who may approve launch, set operating limits, override a recommendation, suspend access and authorize retirement. Delegated authority should be documented rather than inferred from job titles.
Do these 3 things before closing this tab:
1Repair Windows errors before they cause bigger problems2Fix the driver behind crashes, sound loss and screen glitches3Clear out junk files and repair common Windows errorsNIST governance guidance also points to recording legal requirements, impact assessments, data provenance, testing methods, metrics, outcomes and auditability. For a finance team, this means identifying who owns customer-loss exposure, regulatory reporting, model-risk oversight and remediation costs—not merely who manages the software contract.
How do I map harms, misuse and legal obligations?
List harms before choosing controls. Include ordinary failure, foreseeable misuse and interactions with the surrounding workflow, not only the model’s headline accuracy.
Use a harm taxonomy
- Safety and reliability: wrong instructions, unsafe recommendations, outages, automation loops or failures under unusual inputs.
- Rights and fairness: disparate error rates, exclusion, unlawful discrimination, loss of due process or inability to contest an outcome.
- Privacy: collection without a valid basis, re-identification, sensitive-attribute inference, retention beyond purpose or disclosure through prompts and outputs.
- Security: prompt injection, data exfiltration, account takeover, malicious files, model theft or unsafe tool calls.
- Financial and economic: incorrect payments, credit or pricing decisions, fraud losses, contractual penalties, customer compensation and remediation expense.
- Information integrity: fabricated citations, misleading summaries, impersonation, misinformation or disinformation amplification.
- Accessibility and labor: barriers for disabled users, language gaps, deskilling, unsafe workload changes or opaque performance monitoring.
- Environmental and social: resource use, supply-chain effects and community impacts where material.
Determine which rules apply
Classify the system separately for every country, sector and role involved. A provider, deployer, importer and distributor can have different obligations. Do not describe a voluntary framework as proof of compliance.
Under the EU AI Act, obligations depend on the system’s use and the actor’s role. For covered high-risk systems, Article 9 requires a documented and maintained risk-management system covering known and reasonably foreseeable risks, intended use and foreseeable misuse, post-market information and targeted mitigation. Public authorities and public-service deployers may need a fundamental-rights impact assessment before first use; deployers also have duties involving monitoring, human oversight and relevant, sufficiently representative input data.

What belongs in an AI risk register?
Use one row per material risk. A consistent scale is more valuable than false precision: define what “low,” “medium” and “high” likelihood or severity mean, and record the evidence behind each rating.
| Field | What to record |
|---|---|
| Risk ID and harm statement | A plain-language description such as “A generated payment instruction sends funds to the wrong account.” |
| Affected group | Customers, employees, applicants, suppliers, bystanders or a specific vulnerable population. |
| Hazard, failure mode and cause | What fails, why it could fail and which dependency or human action contributes. |
| Likelihood, severity and detectability | Defined scales, assumptions, exposure and evidence; include reversibility where relevant. |
| Existing controls | Data restrictions, authentication, business rules, human review, validation, rate limits and technical safeguards already operating. |
| Treatment and owner | The additional control, accountable person, due date and implementation status. |
| Evidence | Test result, audit log, contract term, incident, stakeholder input or other supporting record. |
| Residual risk and acceptance | Risk after controls, rationale for accepting or rejecting it, approval authority and conditions. |
Prioritize using impact, likelihood, exposure and the availability of effective mitigation. A low-probability event with irreversible financial or rights harm may outrank a frequent but easily corrected error. NIST’s Manage function emphasizes documenting controls, testing, feedback and risk-tolerance or acceptance decisions.
How should I test an AI tool before release?
Set pass/fail criteria before running tests. A result is meaningful only when the data, population, metric, threshold and decision rule are specified in advance.

Compare performance across relevant subgroups rather than relying on one aggregate score.
Test misuse and adversarial conditions
- Prompt injection, jailbreaks, malicious documents and attempts to override business rules.
- Privacy leakage, memorization, re-identification and disclosure through logs, error messages or support tools.
- Unsafe or prohibited outputs, fabricated facts, unsupported confidence and citation errors.
- Abuse of automated actions, excessive requests, credential compromise and tool-call escalation.
- Failures caused by stale data, upstream outages, latency, partial integrations or unexpected file formats.
Evaluate human factors and accessibility
Observe whether users over-trust fluent outputs, ignore warnings, misunderstand uncertainty or skip required review under time pressure. Test keyboard access, screen-reader compatibility, language coverage and accommodations for disabled users where the interface affects access to a service or decision.
Keep a complete test record
Retain the test plan, data description, metrics, thresholds, sample sizes, subgroup results, failure examples, limitations, incidents and remediation decisions. EU Article 9 requires high-risk systems to be tested during development and before market placement or service against predefined metrics and probabilistic thresholds appropriate to the intended purpose. The UK AI Risk Management Toolkit asks whether performance holds when systems are deployed and scaled, under less-than-ideal conditions and unexpected data, and how drift, misinformation and disinformation are tested.
Which controls reduce AI risk most effectively?
Prefer controls that remove or reduce a hazard before adding warnings around it. Layer technical, process and governance controls so one failure does not become an incident.

Design and data controls
- Remove unnecessary sensitive data, minimize retention and separate production data from testing.
- Use allowlists, schema validation, retrieval boundaries and business rules to constrain inputs and outputs.
- Require provenance checks for training, reference and customer data.
Access and security controls
- Use least-privilege accounts, strong authentication, environment separation and secrets management.
- Set rate limits, transaction caps, approval gates and isolation for tools that can send messages, move funds or change records.
- Log prompts, outputs, actions, model versions and administrator changes while protecting sensitive content.
Human and customer protections
- Define when a qualified person must review, override or escalate an output.
- Give affected people clear notice where required, a way to ask questions and a practical appeal or correction route.
- Provide user training on limitations, prohibited uses, escalation and incident reporting.
Fallback and operational controls
Specify what happens when the model is unavailable, uncertain, drifting or producing unsafe results. A fallback can be manual processing, a constrained rules engine or a complete pause. Record who can activate it and how service continuity, customers and records will be handled.
What should the go/no-go approval record say?
Approval should be conditional and scoped, not a blanket statement that the tool is safe. The decision record should include:
- Approved purpose, users, geography, data classes, integrations and prohibited uses.
- Evidence reviewed, test results, known limitations and unresolved assumptions.
- Required controls, control owners and implementation deadlines.
- Residual risks, affected groups, risk-tolerance rationale and the person accepting them.
- Human-oversight requirements, alert thresholds, audit access and complaint handling.
- Monitoring cadence, reassessment date and explicit stop, rollback or kill-switch triggers.
- Conditions for expansion, such as additional testing before new markets, data or decisions are added.
Possible outcomes are approve, approve with restrictions, defer pending evidence or reject. If a control is promised but not operating, treat it as an open condition, not as evidence that the risk has already been reduced.
What documents belong in the AI evidence file?
Keep a versioned, access-controlled file that lets an auditor, regulator, successor team or incident investigator reconstruct the decision.
- System description, purpose, architecture or vendor information and version history.
- Data inventory, provenance, licensing, retention and cross-border transfer analysis.
- Legal, policy, privacy, security and procurement assessments.
- Stakeholder and affected-group consultation, including accessibility input.
- Risk register, impact assessment, test plans, results, metrics and subgroup analysis.
- Human-oversight design, user instructions, training and appeal or complaint procedures.
- Control specifications, validation evidence, incident records and remediation decisions.
- Approval, residual-risk acceptance, monitoring plan, change log and decommissioning plan.
For EU high-risk systems, Article 11 requires technical documentation before the system is placed on the market or put into service, kept up to date and sufficiently clear and comprehensive for authorities and notified bodies. Annex IV specifies minimum elements. The requirement is not satisfied by a marketing brochure or a completed generic template.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. How do I monitor an AI system after launch?
Monitoring must cover real-world outcomes and impacts, not only model metrics. Define indicators, severity levels, alert thresholds, review frequency and the people authorized to suspend the system before launch.
Track leading and lagging indicators
- Accuracy, rejection, escalation, override and abstention rates.
- Subgroup performance, complaints, appeals, accessibility issues and unexplained outcome differences.
- Security alerts, privacy events, unsafe outputs, policy violations and near misses.
- Data drift, concept drift, latency, outages, cost anomalies and changes in user behavior.
- Financial losses, customer remediation, regulatory contacts and operational backlogs.
Define review triggers
Reopen the assessment after a material model or data change, a new purpose or user group, a new integration or geography, a significant incident, a change in threat conditions or evidence of drift. Record whether the response is to continue, constrain, retrain, replace or retire the system. EU Article 9 describes risk management for high-risk systems as continuous and iterative across the lifecycle; NIST’s Manage guidance likewise emphasizes feedback from end users and impacted groups and documented decisions to continue, scale or decommission.

How do we decide whether an AI system is safe enough to deploy?
There is no authoritative universal numeric “safe to deploy” threshold for every AI tool. Set purpose-specific thresholds for accuracy, subgroup performance, unsafe-output rates, privacy leakage, security events, financial exposure and human-review effectiveness, then define what happens when each threshold is missed.
A defensible decision has four characteristics: material harms were considered; tests reflect the real operating context and foreseeable misuse; controls are implemented and owned; and residual risk is explicitly accepted by someone with authority. If evidence is missing for a high-severity or irreversible harm, the appropriate decision is to defer, narrow the use case or prohibit deployment—not to average the gap into a reassuring score.
What are the most common documentation failures?
- Copying a vendor’s claims: Treat supplier information as an input to verify. Request model versioning, data practices, test evidence, incident history, security controls and change-notification terms.
- Assessing the model but not the workflow: A reasonable output can cause harm when routed to the wrong employee, used without review or combined with another system.
- Using only average accuracy: Report subgroup, edge-case, abstention and false-positive or false-negative results that matter to the decision.
- Leaving ownership vague: A committee can advise, but one named role must be able to stop or constrain the deployment.
- Treating approval as permanent: Version, purpose, data and threat changes invalidate old assumptions.
- Confusing a template with proof: A completed form is not evidence that controls work or that residual risk is acceptable.
Can a small team run this process without slowing every project?
Yes. Use proportional depth while preserving the same decision logic. A low-impact drafting assistant may need a short system description, data review, misuse tests, access controls and a named owner; a tool that affects eligibility, payments, employment or essential services needs deeper stakeholder input, subgroup testing, legal review, human oversight and stronger monitoring.
Create reusable templates for the system description, risk register, test plan, approval record and monitoring dashboard. Require a lightweight intake for every tool, then route higher-impact uses to expanded review. Store evidence under version control, link each risk to a control and owner, and schedule reassessment rather than relying on memory.

Frequently Asked Questions
How often should an AI risk assessment be updated?
Set a calendar review appropriate to the system’s impact, then update it immediately after material changes or incidents. Changes to model version, data sources, purpose, user population, integrations, geography or threat conditions are common reassessment triggers.
What should a company ask an AI vendor during procurement?
Ask for model and release-version details, training and evaluation data practices, retention and deletion terms, security architecture, independent test evidence, known limitations, incident-notification commitments and change controls. Make the answers contractual where they are necessary for your risk treatment or audit rights.
How can affected customers challenge an AI-assisted decision?
Provide a clear notice when required, a channel that reaches a human or qualified reviewer, a way to submit correcting information and a defined response time. Log the appeal, outcome and any systemic pattern so complaints feed into monitoring and reassessment.
What is the difference between a model risk review and an AI impact assessment?
A model review concentrates on technical performance, validation and model behavior. An impact assessment is broader: it considers affected people, rights, privacy, accessibility, environmental or social effects, workflow design and whether the use should proceed at all.
