Before deployment, run a documented lifecycle review: define the business purpose, map affected people and data, examine the model and supplier chain, set task-specific acceptance thresholds, test representative and adversarial cases, assign a named human owner, and monitor the system after launch.
This approach gives decision-makers evidence for a proportionate go, limited pilot, redesign or no-go decision. It also creates a record of why the organization accepted residual risk and how it will respond when conditions change.

What a workplace AI risk assessment must establish
An AI tool can influence hiring, scheduling, fraud screening, customer treatment, credit decisions, productivity reviews, health and safety or access to services. The assessment must therefore cover more than whether the software produces plausible text or predictions. It should establish:
- What task the system performs and which decisions its output can influence.
- Who operates it, who is evaluated or affected, and how an affected person can challenge an output.
- What data enters the system, where that data goes, how long it is retained and who can access it.
- Which model, hosting provider, plugins, support teams and subprocessors form the supply chain.
- What could go wrong across privacy, fairness, accuracy, security, robustness, explainability, safety, intellectual property and worker impact.
- What controls, human reviews and stop conditions apply before and after launch.
NIST describes these qualities as trustworthy AI: valid and reliable; safe; secure and resilient; accountable and transparent; explainable and interpretable; privacy-enhanced; and fair, with harmful bias managed. Its AI Risk Management Framework (AI RMF) is voluntary guidance, not a legal safe harbor, and NIST says version 1.0 is being revised. The Generative AI Profile, NIST-AI-600-1, was released on July 26, 2024.

Use the NIST lifecycle: Govern, Map, Measure and Manage
NIST’s Playbook groups suggested actions under four functions. NIST expressly says the Playbook is voluntary and is neither a checklist nor a sequence every organization must follow. Use the functions as a disciplined way to assign work and preserve evidence.
Govern: make accountability explicit
Set the organization’s risk appetite, approval route and ownership before anyone buys a license or connects an application programming interface. Name a business owner who is accountable for the use case, a technical owner for operation, and an independent review function such as privacy, security, legal, compliance or worker representation where appropriate.
Record the applicable jurisdictions and rules, the training required for users, the escalation route for complaints and incidents, and the authority that can pause or terminate the system. Governance should also cover the supplier’s future model changes, subcontractors, data-use practices and incident communications.
Map: describe the system and its context
Write a plain-language purpose statement and define what is out of scope. Identify intended users, foreseeable secondary users and people who may be affected without using the tool themselves. List every decision influenced by an output, including decisions a human is expected to make after reading a recommendation.
Draw the data flow from collection to deletion. Include prompts, uploaded files, telemetry, logs, model training or fine-tuning, backups, support access and cross-border transfers. Identify confidential, personal, special-category or otherwise sensitive information before it reaches a provider.
Document the model version, retrieval sources, connected tools, plugins, hosting region, subprocessors and support chain. A polished interface can hide several distinct suppliers, each with different retention, security and change-control terms.

Measure: test against pre-agreed requirements
Choose measures that reflect the actual task and harm. Test accuracy and reliability, error costs, group performance, privacy, security, robustness, explainability, latency, availability and human-review workload. Include normal, edge, ambiguous and adversarial cases, not only examples selected by the vendor.
Recommended Free Tools
Have qualified staff verify outputs and record the method, data set, model version, reviewer qualifications and unresolved limitations. Compare false positives and false negatives where both matter, and examine results for relevant demographic or worker groups. A single overall accuracy number can conceal unacceptable performance for a smaller group or a high-cost error.
Manage: control residual risk throughout use
Decide whether evidence supports deployment, a constrained pilot, redesign or rejection. Specify mitigations, the person responsible for each one, the deadline and the residual risk accepted by the approval body. Define rollback and stop conditions before launch, not after an incident.
After deployment, monitor drift, accuracy, fairness, privacy complaints, security events, overrides, usage, cost and supplier changes. Reassess after a model update, a new data source, a new user group, a material incident or a change in law.
A ten-part pre-deployment worksheet
Use the following record for every proposed tool. Keep supporting evidence with the approval decision so another reviewer can reproduce the reasoning.

- People and rights. List users, data subjects, workers, customers and bystanders. Explain notice, access, correction, appeal and human review routes. Consider whether a person could suffer financial, employment, safety, reputational or liberty-related harm.
- Data map. Inventory inputs, sources, formats, retention periods, access permissions, backups and deletion methods. Mark special-category, confidential, regulated and proprietary data. Confirm whether prompts or files can be used to train a provider’s model.
- Vendor and supply chain. Identify the model developer, application provider, host, training-data sources, plugins, subprocessors and support locations. Record jurisdictions, transfer mechanisms, dependency on external services and the process for replacing a component.
- Risk register. Describe plausible harms under privacy, fairness, accuracy, security, robustness, explainability, safety, intellectual property and worker impact. For each harm, record likelihood, severity, affected groups, existing controls and the remaining exposure.
- Controls and thresholds. Set minimum requirements for task performance, group performance, latency, uptime, security, retention, access control and human review. Set them before signing so a vendor cannot define success after seeing the results.
- Evidence request. Ask for model and system documentation, test results, known limitations, incident history, security controls, data-use terms, audit rights, subprocessor lists, change-notification commitments and support arrangements. Treat unsupported marketing claims as unverified.
- Testing plan. Use representative, edge and adversarial cases. Compare relevant groups, measure false positives and negatives, test prompt-injection or abuse scenarios where relevant, and have qualified staff review outputs. Preserve test data, scripts, results and exceptions.
- Decision and ownership. Name the accountable owner and approval body. Record the decision, residual-risk acceptance, user training, notice text, escalation path, rollback trigger and the person authorized to stop processing.
- Monitoring schedule. Define metrics, alert thresholds, review frequency, complaint handling, security response, supplier-change review and reassessment triggers. Assign someone to examine the data rather than assuming the dashboard will be read.
When a DPIA or equivalent impact assessment is needed
A data protection impact assessment (DPIA) is a structured way to identify and control privacy risks. The UK Information Commissioner’s Office (ICO) says high-risk processing generally triggers a DPIA; even when an organization decides the risk is not high, it should document that decision and its reasoning.

Consider a DPIA or equivalent assessment when the tool uses personal data at scale, evaluates people systematically, makes or supports decisions with significant effects, combines data sets, monitors workers, uses sensitive data or creates a novel risk. The assessment should cover necessity and proportionality, lawful basis, data minimization, accuracy, retention, access, security, rights handling, supplier responsibilities and residual risk. It does not replace employment, equality, sector-specific or cybersecurity reviews.
UK guidance on AI in recruitment tells organizations to consult ICO guidance or independent legal advice, complete a DPIA for development and deployment involving personal data, and examine supplier assessments and limitations before procurement. The ICO also advises reviewing outsourced services throughout deployment rather than treating procurement as the end of oversight.
Regulatory checkpoints by jurisdiction
European Union
The European Commission’s overview of the EU AI Act describes high-risk obligations covering risk assessment and mitigation, data quality, logging, documentation, human oversight, robustness, cybersecurity and accuracy. Transparency rules are scheduled to apply in August 2026. Check the current implementation timetable, national guidance and whether your use case is classified as high risk before launch; obligations differ by role, system type and use.
Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Clear out junk files and repair common Windows errors3Scan for outdated or missing drivers - takes under a minuteUnited States employment decisions
The U.S. Equal Employment Opportunity Commission’s September 20, 2024 compliance plan calls for an AI use-case inventory, evaluation templates covering performance, reliability, bias, fairness, accountability, transparency, security and privacy, and an AI questionnaire in information-technology and acquisition reviews. These controls help identify discrimination risk, but they do not replace applicable federal, state or local employment laws.
Cybersecurity and civil-rights considerations
The Cybersecurity and Infrastructure Security Agency’s 2023–2024 roadmap calls for AI governance that includes ethical and safety oversight as well as legal, procurement, privacy, civil-rights and civil-liberties considerations. Include those functions in governance for systems used in critical operations or affecting the public.
Set acceptance thresholds before procurement
No authoritative source establishes a universal workplace-AI accuracy threshold, bias percentage, incident rate or return-on-investment figure. A useful threshold depends on the task, population, baseline process and harm tolerance. Define the requirement in writing, then require evidence that the system meets it under conditions resembling production.

What to demand from a supplier and put in the contract
Ask the supplier to identify the model and version, training or retrieval sources where legally disclosable, intended use, prohibited use, geographic processing locations, subprocessors, retention, security controls, test methods, known limitations and incident history. Request evidence from user testing, not only benchmark scores. Ask how the supplier detects drift, handles harmful outputs, investigates complaints and restores service.
Convert critical requirements into measurable contractual terms. The ICO recommends agreeing accuracy KPIs or service-level agreements before procurement and reviewing outsourced services throughout deployment. Include:
- Performance measures tied to your task and test population.
- Availability, support response and incident-notification deadlines.
- Data-use, retention, deletion and confidentiality restrictions.
- Audit or assurance rights, including access to relevant logs and test evidence.
- Advance notice of model, training-data, hosting, subprocessor or policy changes.
- Cooperation with data-subject requests, investigations and regulatory inquiries.
- Exit assistance, data export and secure deletion if the service is terminated.
A supplier’s questionnaire is evidence about the supplier, not proof that your deployment is safe. Validate claims against your own workflow and risk tolerance.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. How to run a meaningful pilot
Build the test set
Use historical or newly collected examples that represent normal operating conditions, important minority groups, edge cases and foreseeable misuse. Remove or protect personal information as required. Keep a holdout set that users and the supplier do not tune against.
Measure outcomes and error costs
Compare the AI-assisted process with the existing process or a qualified human baseline. Record correct results, false positives, false negatives, abstentions, time saved, review time and downstream corrections. Weight errors by consequence: an incorrect low-risk classification is not equivalent to an incorrect employment or safety decision.

Verify and validate
Verification checks whether the system was built and configured as specified; validation checks whether it is fit for the intended use in its operating context. The ICO’s internal policy states: "No AI solution or functionality should be deployed and made available to users without it having passed a documented verification and validation phase." Keep the protocol, results, reviewer sign-off, exceptions and decision.

Test human factors
Observe whether users over-trust confident wording, ignore warnings, copy outputs without checking or face an unmanageable review burden. Test the explanation and appeal process with the people who will use it and, where feasible, people affected by decisions. Revise training, interface, permissions or workflow when human behavior creates additional risk.
Comparing two viable tools
When more than one system passes the minimum gate, compare them on the dimensions below. A cheaper or more accurate tool can still be the worse choice if it creates larger privacy, fairness or oversight obligations.
| Comparison axis | Decision questions |
|---|---|
| Task performance and error cost | Which tool performs better on representative and edge cases, and which errors are more harmful? |
| Fairness | How do results differ across affected groups, and can disparities be mitigated or appealed? |
| Privacy and retention | What data leaves the organization, where is it processed, how long is it kept and can it be deleted? |
| Security and resilience | Which option offers stronger access controls, logging, recovery and incident support? |
| Explainability and contestability | Can users and affected people understand, question and correct an output? |
| Human-review workload | How much qualified review is required, and can the organization sustain it? |
| Integration and logging | Can events, prompts, outputs, overrides and model versions be recorded for audit? |
| Vendor transparency and contract | Will the supplier disclose changes, support investigations and accept measurable obligations? |
| Total cost | What are licensing, integration, review, training, monitoring, incident and exit costs? |
| Regulatory fit | Does the design support obligations in every jurisdiction and sector where it will operate? |
Accountability when an output causes harm
Responsibility does not transfer automatically to the model provider. The deploying organization controls the purpose, data, workflow, user permissions and decision to rely on the output. Allocate accountability in writing among the business owner, technical operator, privacy and security functions, managers who make decisions and the supplier.

Protect those logs from unauthorized access and retain them only as long as justified.
When a harmful or suspect output appears, stop or constrain the affected workflow according to the pre-set trigger; preserve evidence; notify the incident owner, privacy and security teams and the supplier; assess affected people and legal notification duties; correct or reverse decisions where possible; and record the root cause and remediation. Resume only after verification that the control works.
Common approval mistakes and their fixes
- Starting with a product instead of a problem. Write the purpose, alternatives and out-of-scope decisions before evaluating features.
- Using one headline accuracy figure. Require task-specific, disaggregated testing and document the cost of each error type.
- Assuming a human in the loop removes risk. Define the reviewer’s authority, time, training and ability to reject the output; measure whether review actually occurs.
- Ignoring the supply chain. Map hosts, models, plugins, subprocessors, support access and cross-border transfers.
- Treating a DPIA as a certificate. A DPIA identifies and controls risk; it does not prove that deployment is safe.
- Approving a pilot without stop conditions. Set rollback triggers, notification routes and a named person who can pause use before the pilot begins.
- Failing to reassess changes. Re-run relevant tests after model, data, workflow, user-group, supplier or legal changes.
A practical approval record
At the approval meeting, require a concise record containing the purpose and scope; affected people and rights; data and supply-chain maps; risk register; thresholds; test protocol and results; supplier evidence and contract protections; DPIA decision; training and notice plan; accountable owner; residual-risk acceptance; monitoring schedule; and rollback conditions. The record should make clear what was decided, by whom, on what evidence and under which assumptions.
Revisit the record on a scheduled basis and whenever a material change or incident occurs. NIST notes that its AI RMF was developed with more than 240 contributing organizations; that participation describes how the framework was developed, not whether any particular tool is safe. Safety remains a use-case-specific judgment supported by evidence, controls and continuing oversight.
