Random freezes, missing sound and display glitches usually trace back to one bad driver. Find and replace yours safely.Free scan · under a minuteIf Social Security added an income-related monthly adjustment amount (IRMAA) to your Medicare Part B or Part D premium and a qualifying life-changing event has reduced your income, file Form SSA-44 to request a new initial determination. You can submit the form online, or send the completed PDF and supporting evidence by mail or fax to a Social Security office. You can also call Social Security or request an appointment.
The request is not a general appeal based only on having less income. You must identify an eligible event, give its date, and provide more recent or corrected income information. If you believe Social Security made an error in the IRMAA decision itself, use the separate reconsideration process, which generally has a 60-day request period after you receive the determination notice.

What Form SSA-44 does
IRMAA is an income-related surcharge added to the standard Medicare premiums for Part B, Part D, or both. Social Security makes the determination using Internal Revenue Service tax information, your tax-filing status, and the tax year identified in your notice.

Form SSA-44, “Medicare Income-Related Monthly Adjustment Amount—Life-Changing Event,” is the official way to tell Social Security that a major event changed your circumstances and that a newer income year should be used. The current form is identified as SSA-44 (12-2025). Its stated purpose is to request a reduction in IRMAA when a major life-changing event has occurred and income has gone down.

Approval is not automatic. Social Security decides whether the event qualifies, whether it caused the relevant reduction in modified adjusted gross income (MAGI) or changed filing status as required, and what premium adjustment follows from the evidence.
Which life changes can qualify
A lower paycheck or smaller tax return, by itself, does not establish eligibility. The reduction must be connected to an event in Social Security’s recognized categories. Public guidance gives broad examples, while the form and operating instructions identify the more specific categories below.
| Qualifying event category | How to frame the request |
|---|---|
| Death of a spouse | Explain the date of death and how the loss changed household income or filing status. |
| Marriage | Give the marriage date and explain the resulting change in income or filing status. |
| Divorce or annulment | Provide the date and explain the resulting reduction or filing-status change. |
| Work reduction | Describe the reduction in work and the resulting income change. |
| Work stoppage | Describe when work ended and how the stoppage reduced income. |
| Loss of income-producing property | Identify the loss and connect it to the lower income figure. |
| Loss of employer pension income | Explain the loss and the resulting reduction in income. |
| Receipt of an employer payment | Explain the employer payment and why the more recent income year is more representative. |
Social Security’s public examples also refer to loss of income and an employer settlement payment. Those descriptions do not mean every income decline qualifies; the event must fit the applicable category and have the required relationship to your MAGI or filing status.

Start with your IRMAA notice
Before completing the form, read the notice that imposed the surcharge. It tells you what Social Security decided and identifies the tax year used for that determination. Record the following:
- Whether the additional amount applies to Part B, Part D, or both.
- The Medicare premium year covered by the notice.
- The tax year and filing status Social Security used.
- The date you received the notice, especially if you may also need to request reconsideration.
This information prevents a common mistake: asking for an updated income review when the real problem is an incorrect tax record, filing status, or calculation. The two situations can overlap, but they are handled under different procedures.

How to complete SSA-44
1. Select the event that caused the reduction
The form asks you to identify the life-changing event and its date. Choose the category that actually occurred rather than describing only the financial result. For example, “income is lower” is incomplete; “work stoppage on [date]” identifies the event Social Security must evaluate.
The form’s instructions require the event date to be in the same year as, or an earlier year than, the tax year you are asking Social Security to use. Check the date against the income year you enter before submitting the form.
2. State when the event occurred
Use the date the event happened, not merely the date your income first appeared lower. The date helps Social Security determine whether the event falls within the period covered by the requested income year and whether an adjustment can be effective for that year.
3. Provide the newer income information
SSA-44 asks for more recent tax-year information that Social Security should consider. Enter the income and filing-status information requested on the current version of the form, and make sure it is consistent with the evidence you send.
Social Security operating instructions require evidence of the qualifying event and more recent or more accurate tax information. In situations where the most recent return has not yet been filed, procedures may allow an estimate of MAGI while evidence is pending. Follow the form’s current line-by-line directions and contact Social Security if you need to confirm what it will accept for your event or estimated income.

4. Attach evidence
Include documents that establish both parts of the request: the life-changing event and the newer income information. The exact evidence depends on the event and your circumstances. A submission that explains a lower income without documenting the event may not give Social Security enough information to make a new determination.
Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Repair Windows errors before they cause bigger problems3Scan for outdated or missing drivers - takes under a minuteKeep a complete copy of the signed form, every attachment, and the date and method of submission. Keeping these records is practical protection if Social Security asks for clarification or you need to follow up; it is not a special filing requirement stated in the form.
Ways to submit the request
Social Security offers several submission routes. Choose the one that lets you provide the required evidence and obtain help if your situation is unusual.
| Route | Best use | What to do |
|---|---|---|
| Online | You can sign in and upload or provide the requested information electronically. | Complete the SSA-44 online submission after signing in, following the current prompts and evidence instructions. |
| Mail or fax | You already have the PDF and supporting documents ready. | Complete the current SSA-44 PDF and send it with evidence to a Social Security office. Keep the transmission or mailing record. |
| Phone | You need help understanding the process or need to arrange an appointment. | Call 1-800-772-1213. TTY users can call 1-800-325-0778. |
| Office appointment | Your event, income estimate, or evidence needs personal assistance. | Request an appointment through Social Security before visiting and bring the completed form and evidence. |
Call-center support is available in most U.S. time zones on weekdays, but hours can change. Check Social Security’s live information before relying on a particular calling window. If you live outside the United States, contact a Federal Benefits Unit to arrange assistance.
New initial determination versus reconsideration
The correct procedure depends on what you want Social Security to review. A life-changing event with lower income is a request for a new initial determination. A disagreement with the determination Social Security already made is a reconsideration request.

| Question | New initial determination | Reconsideration |
|---|---|---|
| What are you asking Social Security to review? | A qualifying event and newer income information that may reduce IRMAA. | The correctness of the existing IRMAA determination. |
| Main form or process | Form SSA-44, with event and income evidence. | Social Security’s reconsideration process for the determination notice. |
| Typical reason | Retirement, work reduction, death of a spouse, or another recognized event changed your income. | You believe the tax year, filing status, income figure, or calculation used in the notice is wrong. |
| Time limit highlighted by Social Security | Federal rules allow a request during the calendar year in which the significant MAGI reduction caused by a major life-changing event occurs. | Generally request reconsideration within 60 days after receiving the determination notice. |
Social Security’s operating instructions generally direct representatives to treat a claimed qualifying event that reduced MAGI as a new initial determination. The SSA-44 is designed for people reporting that event without going into an office.
If you disagree with the original decision and also experienced a qualifying event, both routes may be initiated in appropriate circumstances. State clearly which issue you are raising: a new event and updated income, an error in the existing decision, or both. Do not assume that filing SSA-44 automatically preserves the reconsideration deadline.
When to file and how retroactive changes work
Under the federal regulation governing this process, a request may be made during the calendar year in which the significant MAGI reduction caused by a major life-changing event occurs. The regulation also provides for necessary retroactive adjustments to the applicable effective year. If Part B enrollment began after January of that year, the effective date can instead be the first month of Part B enrollment during the year, subject to the governing rule.
That timing rule does not guarantee a particular refund, credit, or premium amount. The result depends on the event, the income information, the evidence, the effective year, and Social Security’s determination. Wait for the written decision before assuming what you will owe or recover.

Read the decision notice carefully
Social Security will issue a notice explaining the outcome. Compare it with the issue you submitted: the event category, event date, tax year, filing status, and income figures. If the notice does not address the event you reported or appears to use the wrong information, ask Social Security which review route applies.

Respond to evidence requests
If Social Security asks for additional proof, provide the requested material promptly and keep a copy. Make sure the response identifies your name, the relevant Medicare or Social Security information requested by the agency, and the original submission date so the documents can be matched to your case.
Track deadlines separately
A new initial determination and a reconsideration have different purposes. If your notice gives you a reconsideration right, calculate the generally applicable 60-day period from the date you received the notice and do not wait for an SSA-44 decision before protecting that deadline.
Common mistakes that delay or weaken a request
- Describing only a lower income. Identify the recognized life-changing event and its date.
- Using the wrong tax year. Match the income year to the year requested on the form and confirm that the event date is in the same year or earlier.
- Leaving out Part B or Part D details. Use the IRMAA notice to state which premium is affected.
- Submitting income figures without event evidence. Social Security needs support for both the event and the updated income information.
- Confusing an appeal with a new determination. An alleged calculation or record error belongs in reconsideration, not merely in an SSA-44 narrative.
- Waiting past the appeal period. Filing a life-event request does not automatically extend a generally applicable 60-day reconsideration deadline.
- Using an outdated form. Start with the current SSA-44 revision, identified in the current materials as 12-2025.
- Assuming approval or a refund. Social Security decides the effective date and amount from the facts and evidence; no standard savings figure or processing time is established.
Practical examples of when to use SSA-44
Retirement or work reduction
Suppose the income used on your IRMAA notice reflects a year when you worked more than you do now. If you actually stopped work or reduced work and that event caused the lower income, identify the work stoppage or work reduction, give its date, and provide the newer income information requested by SSA-44. A statement that retirement is financially difficult, without identifying the qualifying event and evidence, is not the same request.
Death of a spouse
If your spouse died and your household income and filing status changed, select the death-of-spouse category, provide the event date, and document the newer tax-year information. The fact that income is lower after the death does not by itself determine the result; Social Security must evaluate the event and the updated figures together.
Incorrect determination as well as a later income change
If the notice used the wrong filing status or tax information and you later experienced a qualifying work or family event, identify both issues. Protect any reconsideration deadline for the original notice while submitting the new-initial-determination information for the later event.
Checklist before you send SSA-44
- Locate the IRMAA notice and write down the affected Part B or Part D premium, premium year, tax year, filing status, and receipt date.
- Choose the specific qualifying event from the form’s categories.
- Confirm the event date is in the same year as, or earlier than, the tax year you are asking Social Security to use.
- Complete the current SSA-44, including the more recent income and filing-status information requested.
- Gather evidence of the event and the updated or estimated income information allowed under the current instructions.
- Select online, mail, fax, phone, or an appointment based on your access and need for assistance.
- Keep a full copy and proof of submission.
- Read Social Security’s decision notice and separately monitor any reconsideration deadline.
The bottom line
Use Form SSA-44 when a recognized life-changing event caused your income to fall and you want Social Security to make a new IRMAA determination using more recent information. The strongest request connects the event, its date, the changed MAGI or filing status, and supporting evidence. Use reconsideration instead when you are challenging the existing determination, and remember that the general reconsideration period is 60 days after receipt of the notice.
