Before you start, gather accurate information about everyone applying, household income, state residence, Social Security numbers where applicable, and citizenship or immigration status. You may not need to upload a standard packet of papers: states often verify information electronically and request supporting documents only when a detail cannot be confirmed or a discrepancy needs explanation.
Your state Medicaid agency—not a national checklist—decides which pathway applies, what evidence is acceptable, and how you submit it. Use the agency’s current instructions for your state and applicant category.

Why there is no single Medicaid document checklist
Medicaid is a joint federal-state program. Each state operates its own application system, sets program rules within federal requirements, and determines eligibility for its residents. The application route, income standards, verification process, and documents requested can therefore differ by state and by the person applying.

Some states use a benefits portal, some use a state-linked Marketplace process, and others direct applicants through another state-designated channel. Follow the route identified for your state rather than assuming that a neighboring state’s process applies.

CMS’s state help directory is the starting point for finding the agency that handles applications, eligibility questions, and status checks. The state agency is also the authority for the final decision.
How to apply for Medicaid: the five-step process
1. Find your state’s Medicaid agency
Start with the current state contact and program information listed in the Centers for Medicare & Medicaid Services (CMS) state directory. Confirm that you are using the official state route before entering personal information. The agency can tell you whether to apply through its benefits portal, a Marketplace-linked application, by phone, in person, or through another channel the state designates.
2. Use the channel your state specifies
States commonly provide an online application, but the exact portal and workflow are state-specific. A Marketplace application may screen the household for Medicaid or CHIP and transfer the case to the state when appropriate. Do not submit duplicate applications simply because you see more than one public program website; use the path your state identifies and follow its instructions.
3. Complete the household and eligibility questions accurately
Enter the people applying, other household members requested by the form, current address and contact information, income sources, and any category-specific facts the application asks about. Depending on the applicant, those facts can include age, pregnancy, parent or caretaker relationship, disability-related information, or immigration and citizenship details.
Answer for the requested reporting period and explain unusual income when the form provides a place to do so. Medicaid eligibility for most children, pregnant people, parents, and adults uses Modified Adjusted Gross Income (MAGI) rules, but other eligibility pathways use different methods. The application may ask questions that determine which method applies.
4. Submit the application and monitor follow-up
After submission, the agency may check information against electronic data sources. If the data are complete and reasonably consistent, no paper proof may be needed. If information is missing, unavailable, or does not match, the agency can ask for an explanation or supporting documents.

5. Contact the state agency about status or unclear notices
Use the state Medicaid agency for application status, correction of an error, clarification of a notice, or instructions for sending evidence. A federal overview cannot tell you which file format, mailing address, upload screen, or deadline your state uses. If a request is unclear, ask the agency before the deadline shown on the notice.
What information to have ready
Think of this as a preparation list, not a promise that every item must be uploaded. The state may verify several details electronically and request only the items it cannot confirm.
| Information category | Examples of details to prepare | How it may be used |
|---|---|---|
| People in the household | Full names, dates of birth, relationships, and which people are applying | Identifies applicants and helps determine household composition under the applicable eligibility rules. |
| Income | Current earnings and other income sources for the people the application asks about | Supports financial eligibility calculations; most children, pregnant people, parents, and adults are evaluated under MAGI rules, while other groups may use a different method. |
| Residence and contact | State address, mailing address if different, phone number, and email address | Shows where the application belongs and provides channels for notices and follow-up. |
| Social Security information | Social Security number information where applicable | Allows the agency to identify an applicant and perform available electronic checks. |
| Citizenship or immigration information | Status information for applicants whose status is relevant to the program | Helps the agency evaluate the applicant’s eligibility category and verify status. |
| Category-specific facts | Age, pregnancy, household relationships, or other questions shown in your state application | Determines which Medicaid or CHIP pathway and verification rules apply. |
Use the exact names and numbers shown on official records. A typo in a name, Social Security number, birth date, or address can create a mismatch that leads to a follow-up request even when the applicant is otherwise eligible.
Documents you may be asked to provide
There is no nationally required paper packet for every Medicaid applicant. States use available electronic sources to verify much of the information reported on an application. A document request generally means the agency needs to resolve a particular issue, not that every applicant must submit every possible record.
If the agency asks for evidence, the notice should identify what it needs and explain how to submit it. The request might concern income, identity, residence, citizenship or immigration status, or another fact relevant to the applicant’s category. Read the notice line by line and send the requested material through the stated channel.

How to respond to an evidence request
- Identify the exact issue. Note the person concerned, the fact being verified, the documents listed, and the response deadline.
- Send only what the notice requests unless the agency tells you otherwise. Extra, unrelated records can make a case harder to review.
- Use the specified submission method. That may be an online upload, mail, fax, phone explanation, or an in-person delivery, depending on the state.
- Keep proof of delivery. Save upload confirmations, fax receipts, mailing tracking, and copies of every page sent.
- Ask for clarification promptly. Contact the state agency if the document is unavailable, the deadline is unclear, or the requested format is not practical.
When electronic data and the application are not reasonably compatible, CMS policy allows the agency to seek an explanation or paper documentation. A request for documents is therefore a verification step; it is not by itself a denial.
Which Medicaid eligibility rules apply?
MAGI-based groups
MAGI-based financial rules generally apply to most children, pregnant people, parents, and adults. The application asks for household and income information used under that method. The household definition and income treatment can differ from the way you calculate income for taxes or a personal budget, so answer the state’s questions rather than substituting your own calculation.
Other eligibility pathways
Some applicants are evaluated under pathways that do not use MAGI. The state may apply different financial, medical, or category rules for those groups. The application questions help route a person to the appropriate pathway, and the state agency makes the final determination.

State-specific standards
Income thresholds and program decisions vary by state and applicant category. A CMS eligibility-level table surfaced for comparison is explicitly based on state decisions as of December 1, 2023. It is a dated reference, not a current national income limit. Do not use a percentage from that table—or another state’s limit—as an estimate of your own eligibility without checking current state guidance.
| What can differ | How the difference affects your application |
|---|---|
| Application location | You may use a state benefits portal, a state-linked Marketplace application, or another channel specified by the state. |
| Eligibility category | MAGI rules cover many children, pregnant people, parents, and adults; other groups can follow different pathways. |
| Verification method | Electronic data may confirm information, or the agency may request an explanation or paper evidence when data are missing or inconsistent. |
| Income and program standards | Current limits and covered categories are state-specific and can change; a December 1, 2023 comparison table is not a live estimate. |
How electronic verification works
When you submit an application, the state can compare reported information with available electronic sources. This may verify identity, income, residence, or status without asking you to upload a document. The precise sources and matching procedures depend on the state.
A mismatch can result from a changed address, a recently started or ended job, income that varies from month to month, a different spelling of a name, or data that has not updated yet. Read the notice to see which fact needs attention. Provide the explanation or evidence requested and tell the agency if the underlying information has changed since you applied.

Do not ignore a request because you believe the agency can find the information electronically. The agency may still need a response to reconcile the records, and only the state can tell you whether an alternative document or explanation is acceptable.
Special situations to flag on the application
Pregnancy or a child applicant
Pregnancy and age can place an applicant in a specific Medicaid or CHIP category. Enter the information exactly where the state form asks for it so the application can be evaluated under the appropriate rules.
Multiple household members applying
List every person the application requests, even if only some household members are seeking coverage. The state uses household relationships and applicant status to determine which rules apply to each person.
Immigration or citizenship information
Provide status information for applicants when the form requests it. Do not guess. If you are unsure which document or status detail the state needs, ask the Medicaid agency for instructions rather than sending unrelated records.
Income that changes frequently
Report income as the application directs and explain fluctuations when there is a place to do so. Keep recent records available in case the state asks you to reconcile the amount you entered with electronic data.

Common application mistakes and fixes
- Using another state’s instructions: return to your own state’s Medicaid agency and use its current application route.
- Assuming everyone must upload documents: prepare the information first; wait for a specific evidence request unless the application tells you to attach a record.
- Leaving household members out: include the people and relationships the form asks about, even when they are not applying.
- Reporting an outdated address or income: enter current information and explain changes when prompted.
- Missing a notice: monitor the contact method on the application and respond by the deadline shown.
- Sending evidence without identifying the case: follow the agency’s labeling or upload instructions and retain delivery proof.
- Relying on a dated income table: use current state guidance; the CMS comparison table dated December 1, 2023 does not establish today’s limit.
What happens after you apply
The state reviews the application, performs available electronic checks, and contacts you if clarification or evidence is needed. Keep your contact information current while the application is pending. If you move, change phone numbers, or discover an error, tell the state agency using the method it provides.
For a status update, missing notice, correction, or explanation of the next step, contact the state Medicaid agency. CMS identifies state agencies as the source for application help and program-specific instructions because processing and verification are administered locally.
Practical preparation checklist
- Locate the official Medicaid agency route for your state.
- Write down the names, birth dates, relationships, and applicant status of household members requested by the form.
- Gather current income details and the sources of that income.
- Have your state residence and reliable contact information ready.
- Prepare Social Security number information where applicable.
- Prepare citizenship or immigration status information for applicants whose status is relevant.
- Note category-specific facts such as age or pregnancy.
- Submit through the state-designated portal or channel.
- Save the confirmation and monitor for follow-up.
- Respond to any evidence request exactly as instructed and keep proof of submission.
Bottom line for applicants
The reliable way to apply is to start with your state Medicaid agency, use the application channel it identifies, and provide complete household, income, residence, and status information. Do not spend time assembling a universal paper packet: electronic verification is common, and the state will tell you when supporting documents or an explanation are needed. Because eligibility categories, income standards, portals, and evidence rules vary, the current instructions from your own state agency control your application.
