If Medicare charged you an income-related monthly adjustment amount (IRMAA) that you believe is wrong, the Social Security Administration (SSA)—not Medicare or your Part D drug plan—handles the challenge. The correct path depends on why you disagree: use Form SSA-44 to request a lower amount after a qualifying life-changing event that reduced your income, or request reconsideration when the initial determination contains an error or uses information you dispute. Your SSA notice controls the filing instructions and deadline.
Build your request around two things: the specific reason the determination should change and records that prove that reason. A life-event request generally must establish both the event and the resulting income reduction. A tax-data or factual-error challenge should identify exactly what SSA used and provide corrected or contradictory records.

What an IRMAA decision is—and who handles it
IRMAA is an additional amount added to Medicare Part B and Part D premiums when SSA determines that your income meets the applicable rules for the premium year. SSA makes the income-related determination and sends the notice explaining the premium year, tax information used, reason for the charge, and instructions for review.
SSA states: “If you’ve had a life-changing event that reduced your household income, you can ask to lower the additional amount you’ll pay for Medicare Part B and Part D.” That request is generally made with Form SSA-44 and supporting evidence.

This is different from a Medicare Part D plan appeal. A Part D appeal concerns a plan’s decision about drug coverage or payment. An IRMAA challenge concerns SSA’s calculation of an income-related premium, so sending a drug-plan appeal to resolve IRMAA will not address the underlying decision.
Start with the notice, not with a generic Medicare form. The notice identifies the premium year at issue and the information SSA relied on. Those details determine whether you are asking SSA to account for a later change in circumstances, correct an underlying record, or reconsider a factual conclusion. Keep the complete notice, including any pages that explain review rights.
Choose the right review route
Read the notice before completing paperwork. The reason for the dispute determines whether you are asking SSA for a new initial determination, requesting reconsideration of the existing initial determination, or potentially pursuing both when the facts overlap. These routes are related but are not interchangeable labels: one focuses on changed circumstances or newer information, while reconsideration challenges the initial determination itself.
| Route | When it fits | Primary form or request | Evidence focus | Review stage |
|---|---|---|---|---|
| New initial determination | A qualifying life-changing event lowered your household income, or newer information changes the income basis. | Form SSA-44 and the submission instructions from SSA. | Proof of the qualifying event and records supporting the resulting income change or estimate. | SSA makes a new initial determination using the updated circumstances. |
| Reconsideration | You believe the initial determination is wrong for another reason, such as incorrect tax information or a factual error. | A reconsideration request following the notice and SSA instructions. | Records identifying the exact error and showing what the correct information should be. | SSA reviews the initial determination at the reconsideration stage. |
| Both routes in an overlapping case | Your dispute includes a qualifying life event and a separate objection to the information used. | State each basis clearly and follow the notice’s filing directions. | Separate evidence for the life event, changed income, and alleged data error. | SSA policy allows a new initial determination and an appeal to be pursued together in some situations. |
Use the route that matches the actual reason for the request. A later drop in income is not, by itself, proof that an earlier tax record was wrong. Conversely, an amended tax record does not automatically establish a qualifying life event. If both facts exist, present them as two identifiable issues so SSA can evaluate each under the appropriate procedure.
When to use Form SSA-44
Use the life-changing-event route when an event reduced your household income and you want the additional Part B or Part D amount lowered. Qualifying examples listed by SSA include marriage, divorce, death of a spouse, loss of income, and an employer settlement payment. Complete SSA-44 and attach documentation of the event plus records supporting the income change or a reasonable income estimate requested by the form.
When to request reconsideration
Request reconsideration when you are challenging the initial determination itself—for example, because SSA used the wrong tax year information, relied on a tax record that was corrected, or made another factual mistake. Identify the determination date and the precise information you believe is wrong. Follow the form, delivery method, and deadline stated in your notice rather than relying on a generic timetable.
When both issues exist
A single situation can contain two distinct arguments. For example, your income may have fallen after a divorce, while SSA also used an incorrect tax return. Explain each basis separately, submit the SSA-44 evidence for the event and income change, and include the corrected tax records for the data dispute. SSA policy allows the routes to be pursued together in some circumstances, but your notice and SSA’s instructions govern how to file.

Step-by-step: prepare and submit the challenge
- Read the complete SSA notice. Mark the premium year, the tax information SSA used, the stated reason for IRMAA, the address or submission channel, and every deadline or next-step instruction. Keep all pages, including enclosures.
- Write a one-sentence basis. Use plain language such as “I am requesting a new initial determination because my spouse died and our household income fell,” or “I am requesting reconsideration because SSA used a corrected tax record for the wrong year.” This sentence keeps the evidence focused.
- Select the route and form. Complete SSA-44 for a qualifying life-changing event. For an alleged error in the initial determination, request reconsideration using the notice’s instructions. If both grounds apply, label them as separate sections rather than blending them.
- Make an issue-to-document list. For each statement in your explanation, name the attachment that proves it. For example, pair the date of a spouse’s death with the death certificate, then pair the resulting loss of income with employer, pension, or benefit records.
- Assemble records that prove the claim. Include the completed form, documentation of the event or error, and income or tax records that connect the facts to the premium year. Explain what each attachment proves.
- Check consistency. Confirm that names, dates, tax years, marital status, and income figures match across the form and attachments. If an amount is an estimate, identify the period covered and explain the basis for it.
- Submit through an SSA channel. SSA lists online submission, phone contact, fax or mail of the completed SSA-44 with evidence, and appointments. Use the channel and destination specified by your notice or SSA representative.
- Preserve proof of submission. Keep the signed form, every attachment, a copy of the notice, and evidence that the package was uploaded, faxed, mailed, or delivered. Record the date and method and retain any confirmation number or receipt.
- Respond to follow-up requests. If SSA asks for clarification or another document, send it using the instructions provided and keep a copy. Do not assume that sending an unrelated document will substitute for the requested proof.
Numbering attachments can make a complicated file easier to review. A short cover page can list the premium year, your requested action, and the attachment numbers. Keep the explanation factual and chronological: event or error, date, effect on income or tax information, and requested correction. Avoid sending duplicate or unrelated papers that obscure the document addressing the disputed fact.

Evidence for a life-changing-event request
Prove the qualifying event
Your first group of attachments should establish when and what happened. The appropriate record depends on the event. Examples include a divorce decree, a death certificate for a spouse, documentation of a marriage, employer records showing a loss of income, or records of an employer settlement payment. Use the document that directly identifies the event and its effective date.
The event document should connect to the household circumstances described on SSA-44. If a document uses a different name, explain the difference rather than leaving the reviewer to reconcile it. If the event occurred during the premium year, state the date and identify which income sources changed afterward.
Prove the resulting income change
The event alone is not enough. Show how it changed household income for the relevant premium year. Useful records can include pay statements, employer statements, pension or annuity records, benefit statements, settlement documentation, or other income records that support the figure on SSA-44. Explain the connection: identify the income source that ended, decreased, or changed and the period affected.

A final pay statement, an employer statement showing reduced hours, or a pension notice showing a changed payment can be more useful when accompanied by a sentence explaining what changed and when. Include only figures relevant to the requested determination, while retaining the originals in your personal file.
Support an estimate carefully
SSA-44 may require an estimate when the full year’s income is not yet known. State the period covered, identify the assumptions, and attach records supporting those assumptions. An estimate should be traceable to actual changes rather than a bare assertion that income is lower.
Explain whether the estimate reflects income that stopped, a reduced payment, or another documented change. If circumstances later change again, keep updated records and follow any instructions SSA provides for supplying revised information. Do not present a projection as a completed tax return.
Evidence when SSA used incorrect tax information
Start by identifying the tax year and information shown in the notice. Then obtain the record that demonstrates the correction. Depending on the situation, that may be a corrected or amended tax return, an IRS notice reflecting a change, or other official tax documentation. Explain which figure SSA used, which figure is correct, and why the corrected record applies to the premium year at issue.
Put the disputed figures side by side in your explanation. Identify the line, year, or filing information at issue and point to the attachment that resolves it. If an amended return is still being processed, state that clearly and provide the official documentation you have; follow SSA’s instructions about any additional record it requests.

Do not confuse a corrected tax record with income that became lower in a later year after a life-changing event. A corrected return addresses what the tax record should have shown; a life-event request addresses a subsequent change in circumstances. If both occurred, present both arguments and their separate supporting records.
Evidence of another factual error
For an error that is not a life event or tax correction, describe the mistake precisely. Examples might include an incorrect filing status, a wrong identity match, or a figure that does not belong to you. Attach records that directly address the disputed fact and explain the correct information. SSA evaluates the information in the record, so case-specific documentation is more useful than a large collection of unrelated papers.
Use a short chronology if the error involves several dates or notices. Quote or paraphrase the disputed entry, state why it is incorrect, and identify the document that establishes the correct fact. If you are unsure whether an issue is a factual error or belongs on SSA-44, describe the underlying circumstances plainly and ask SSA which route and form apply.

How to submit from inside or outside the United States
SSA identifies several submission and contact options: online submission, telephone contact, fax or mail of Form SSA-44 and evidence, and appointments. Your notice may direct you to a particular office or method, so use that destination even when another channel appears generally available.
People outside the United States, including residents of U.S. territories and commonwealths, are directed to contact a Federal Benefits Unit for an appointment. Ask which documents and transmission method that unit accepts, then retain confirmation of the appointment and submission.
Before sending, verify that every page is legible and that attachments are in the order described in your cover page. For fax or online transmission, retain the confirmation screen or report. For mail, keep the mailing receipt and a copy of the addressed package. These records do not replace the evidence itself, but they help establish what you submitted and when.
Deadlines, notice instructions, and further review
There is no single universal deadline to substitute for the instructions on your individual SSA notice. The notice controls when and how to request reconsideration and what to do if you disagree with the result. Submit as soon as you have the required records, and if a document is still pending, contact SSA promptly to ask how to preserve your review rights.

Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Common mistakes that weaken an IRMAA challenge
- Using the wrong agency. A Part D plan appeal addresses drug coverage or payment, not an SSA IRMAA calculation.
- Sending only a life-event document. SSA needs evidence of the event and the resulting income change or estimate.
- Calling a later income drop a tax correction. A later-year reduction generally belongs in the life-changing-event route; a corrected return addresses an error in the tax information used.
- Failing to name the disputed year. Identify the premium year and the tax year or record shown in the notice.
- Submitting unexplained documents. Label each attachment and state the fact it proves.
- Mixing separate arguments. Identify the life event, changed income, and alleged tax or factual error in separate sections.
- Using inconsistent numbers. Reconcile the figures on SSA-44, tax records, income statements, and your explanation before submitting.
- Missing the notice’s instructions. Generic advice cannot replace the address, deadline, or procedure printed on your determination.
- Keeping no submission record. Without a copy and proof of delivery, it is harder to show what SSA received and when.
- Assuming approval is automatic. SSA evaluates the evidence in your individual record; no generic checklist guarantees a lower amount.
Examples of matching the evidence to the issue
Income fell after a spouse’s death
Request a new initial determination with SSA-44. Include the death certificate, documentation showing the affected pension, wages, or other income, and records supporting the projected household income for the premium year. Explain the date the income changed.
A corrected return changes the figure SSA used
Request reconsideration and identify the tax year in the notice. Attach the corrected or amended tax documentation and explain the difference between SSA’s figure and the corrected figure. If a separate life event also reduced later income, include that as a distinct SSA-44 basis.
Recommended Free Tools
SSA’s notice contains a factual mismatch
Request reconsideration, describe the mismatch—such as an incorrect filing status or income attributed to you—and attach the record that establishes the correct fact. Organize the submission so the reviewer can locate the disputed line and the document that resolves it.
Income fell after an employer settlement or job loss
Use SSA-44 when the settlement payment or loss of income is the qualifying event. Include the employer or settlement record identifying the event, then provide pay, benefit, or other income records showing how household income changed for the premium year. Explain whether the event ended a source of income, reduced it, or altered its timing.
What to keep in your personal file
Retain the IRMAA notice, completed SSA-44 or reconsideration request, every supporting record, your written explanation, and proof of submission. Keep notes of telephone calls, including the date, office or representative contacted, and instructions received. This recordkeeping is practical protection if SSA asks for clarification or if you need to pursue the next review stage.
Store a simple index with the premium year, tax year, submission date, and status of each issue. If you send additional material later, add it to the index and note the reason. A complete chronological file helps you answer follow-up questions without changing the explanation you originally submitted.
Frequently asked questions
Frequently Asked Questions
Can I lower IRMAA if my income went down?
Possibly. If a qualifying life-changing event reduced your household income, ask SSA for a new initial determination using Form SSA-44 and evidence of both the event and the resulting income change. A lower income in a later year is not automatically a correction to the earlier tax information SSA used.

Who handles an IRMAA appeal?
SSA handles IRMAA determinations and requests for review. Medicare Part D plans handle separate appeals about drug coverage or payment. Send an IRMAA challenge according to the SSA notice, not through your drug plan’s coverage-appeal process.
What evidence should accompany Form SSA-44?
Include documentation identifying the qualifying event and records supporting the resulting income change or estimate. Depending on the event, SSA examples include marriage or divorce records, a spouse’s death certificate, employer records showing lost income, or settlement documentation, together with relevant wage, pension, benefit, or other income records.
When should I request reconsideration instead of using SSA-44?
Request reconsideration when you believe the initial determination is wrong because SSA used incorrect tax information or made another factual error. Identify the disputed tax year or fact and attach corrected or contradictory records. Use the form, delivery method, and deadline in your notice.

State both bases separately. Submit SSA-44 evidence for the event and changed income, and provide corrected tax records for the data dispute. SSA policy allows a new initial determination and an appeal to be pursued together in some situations, but the notice and SSA instructions control how you file.
Is there one IRMAA appeal deadline for everyone?
Do not rely on a universal deadline. The individual SSA notice controls the timing, filing method, and next steps for reconsideration or further review. Submit promptly and contact SSA if a needed document is pending or you need instructions to preserve review rights.
Can I submit an estimate of my income?
SSA-44 may require an estimate when the full year’s income is not known. State the period covered, explain the assumptions, and attach records showing why the estimate is reasonable. A bare statement that income is lower is not a documented estimate.
What should I do if SSA asks for more information?
Respond using the instructions in the request, provide the specific document or explanation requested, and keep a copy and proof of delivery. Continue to identify the premium year and issue so the additional material is connected to the pending determination.
