The safest approach is controlled resilience, not an abrupt exit from China. Drug manufacturers first map dependencies below the finished-product supplier, identify single-source ingredients and quality chokepoints, then qualify alternate suppliers, sites or flexible capacity before moving routine production. Inventory buffers, continuous-manufacturing options, stronger surveillance and a tested response plan provide additional time if a disruption occurs.
Every change must preserve validated processes and applicable good manufacturing practice (GMP) and regulatory requirements. A second supplier that has not completed the necessary quality, comparability, validation and regulatory work is only a project, not interchangeable production capacity.

What “China supply-chain risk” actually means
China exposure can exist several tiers upstream of the company that manufactures and releases a finished medicine. An approved finished-dose supplier may depend on an active pharmaceutical ingredient (API) plant, which may depend on key starting materials (KSMs), intermediates, critical excipients, solvents, packaging components or specialized logistics. A country label on the finished-product vendor therefore does not reveal the whole risk.

The practical question is not simply whether a supplier is Chinese. It is whether a particular material or site is concentrated, difficult to replace, slow to qualify, hard to inspect or essential to a product that patients cannot readily substitute.

The available FDA evidence does not establish a current percentage for China’s share of all U.S. medicines, APIs or KSMs. Companies should therefore avoid broad claims about national dependence and make product-level decisions using documented evidence.
1. Map dependencies below the finished-drug level
Start with a product-by-product bill of materials and extend it through the material tiers. The map should identify the legal supplier, the actual manufacturing site and, where possible, the site supplying the upstream input. Include indirect suppliers and logistics routes when they can affect release-ready inventory.
Build the material map
- List each API, KSM, intermediate, critical excipient and component used in the product.
- Record every approved manufacturing and testing site, including contract manufacturers and laboratories.
- Document country, port, transport mode, lead time, minimum order quantity and normal replenishment path.
- Mark sole-source nodes, geographically concentrated clusters, long transfer routes and inputs with limited substitutability.
- Capture quality history, inspection findings, import alerts, deviation trends and changes in ownership or process.
FDA’s supply-chain discussions specifically call attention to API KSMs and other component chokepoints. Stopping the analysis at the finished-dose contract manufacturer can leave the most consequential dependency invisible.

China location is one data point, not a complete risk score. A Chinese site with multiple qualified alternatives, short replenishment time and strong quality performance may pose less immediate risk than a sole-source site elsewhere that requires a year-long technical transfer. Rate each node by its role in the product, available alternatives, quality history, transportation vulnerability and the time required to recover usable supply.
2. Rank risks by patient and production impact
Use a documented quality-risk-management process to prioritize work. FDA’s May 2022 draft guidance, Risk Management Plans to Mitigate the Potential for Drug Shortages, presents a proactive framework aligned with ICH Q9 principles. It is explicitly draft and non-binding, so it is a planning model rather than a universal regulatory template.
Score the events that matter
For each dependency, assess likelihood, patient and business impact, detectability, time to recovery and the availability of qualified alternatives. A failure that stops a critical injectable in two weeks deserves earlier action than a noncritical input with six months of finished inventory and several approved sources.

- Likelihood: consider supplier quality signals, geopolitical exposure, transport reliability, weather and utility dependence.
- Impact: estimate consequences for patients, therapeutic substitution, market commitments and batch release.
- Detectability: determine whether a problem will appear through routine testing, supplier notification, customs data or only after a shortage develops.
- Recovery time: include investigation, material procurement, testing, manufacturing, release and regulatory review.
- Alternatives: distinguish an approved source from a candidate that still needs qualification or a filing.
Assign owners and triggers
A useful plan names an accountable owner for every high-priority dependency, defines early-warning indicators and sets escalation triggers. Indicators can include late shipments, unexplained allocation changes, repeated deviations, adverse inspection information, raw-material shortages or a supplier’s inability to provide required data. The plan should specify who can authorize inventory allocation, quality holds, supplier escalation and regulator contact. The FDA materials support this planning approach, but they do not prescribe one scoring scale or threshold.
3. Qualify alternate sources before a disruption
Diversification only reduces risk when the alternative can make conforming material legally and technically. FDA-hosted remarks on supply-chain vulnerabilities warn that component substitutions are complex and that companies cannot easily switch suppliers of starter materials or device components.
What qualification normally covers
- Supplier quality assessment: review the quality system, manufacturing controls, data integrity, audit history, deviations, complaints and change-control practices.
- Technical comparability: compare identity, purity, impurity profile, particle characteristics, microbiological attributes and other product-specific critical quality attributes.
- Process and analytical work: perform studies, method qualification or validation, engineering batches and stability work as applicable to the material and dosage form.
- Regulatory assessment: determine whether a supplement, amendment, prior approval, master-file reference or other filing is required in each market.
- Operational readiness: confirm capacity, testing laboratories, packaging, transportation, release procedures and change-control ownership.
Keep the evidence and approvals inside the applicable quality system. A contract, letter of intent or successful laboratory sample does not make a source production-ready.
Use more than one type of alternative
For a high-priority API, the best design may combine a second supplier, an alternate site at the same supplier and a prequalified manufacturing route. That combination can address different failure modes: a supplier-specific quality event, a country-wide logistics interruption or a capacity shortfall. It also reduces the chance that diversification merely moves the bottleneck to another sole-source KSM.
4. Balance inventory with capacity and qualification options
Inventory can bridge a temporary interruption while a qualified alternative is activated, but stock is not a substitute for resilience. Set buffer levels by product criticality, shelf life, demand variability, replenishment time, storage conditions and the time required to transfer or qualify supply.
Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Clear out junk files and repair common Windows errors3Scan for outdated or missing drivers - takes under a minuteThere is no universal FDA-endorsed number of months of inventory. A historical observation from 2020 should not be presented as a current benchmark. Excess stock can expire, consume working capital and hide an unresolved single-source problem; too little stock leaves no time for investigation or regulatory action.

Design the buffer around usable time
Calculate how long existing material supports production after accounting for testing, quarantine, release, safety stock, forecast error and campaign scheduling. Then compare that usable time with the realistic recovery time for each alternative. If qualification takes longer than the buffer, start the technical and regulatory work before an incident rather than relying on emergency procurement.
Reserve flexible capacity
Capacity reservations, campaign slots and agreements with qualified manufacturers can be more valuable than simply buying more inventory. The arrangement should specify quality responsibilities, data access, change notification, testing capacity and the conditions under which the reserved line can be activated.
5. Use process and network flexibility
Continuous manufacturing
FDA’s final March 2023 guidance, Q13 Continuous Manufacturing of Drug Substances and Drug Products, provides scientific and regulatory considerations for development, implementation, operation and lifecycle management of continuous manufacturing. Continuous processing can improve throughput, reduce some scale-up steps and make production more flexible, but it does not by itself remove dependence on a Chinese API, KSM or excipient supplier.

The process, product, facility, control strategy, digital systems and regulatory pathway must all support the change. For some products, a robust batch process with two qualified sources will reduce risk faster than converting the entire operation.
Regional and multi-site capacity
FDA’s 2021 supply-chain review pillars include boosting local production while fostering international cooperation, supporting innovative manufacturing, strengthening quality-management maturity and using data to improve resilience. These pillars support a diversified network, not an assumption that moving every step into one new country eliminates risk. New regional capacity may still rely on the same KSM, specialized equipment, utility or shipping lane.
6. Prepare notification and incident response
Resilience depends on information moving quickly enough for quality and supply teams to act. FDA’s February 2024 draft guidance on notifying the agency of a discontinuance or interruption discusses timely, informative notifications for certain manufacturing interruptions or discontinuances involving finished products, biologics and APIs. It is draft Level 2 guidance and not for implementation; companies must check the current statutory and regulatory duties that apply to their product and situation.

Build a response plan
- Name decision owners for supply, quality, regulatory, medical, legal and communications functions.
- Maintain current supplier, site, logistics and regulator contact details.
- Define product-allocation rules before scarcity forces ad hoc decisions.
- Document quality-release constraints, testing priorities and any approved substitutions.
- Set internal escalation times for supplier notices, missed shipments and quality events.
- Run exercises using scenarios such as a port closure, an API contamination event and a sudden export restriction.
FDA’s APEC toolkit addresses the product lifecycle from raw materials to patients and includes quality practices, surveillance, track-and-trace, prevention, detection and incident response. It can help structure information flows and exercises across suppliers and internal teams.

How to compare resilience options
| Option | Primary risk addressed | Readiness work | Important limitation |
|---|---|---|---|
| Qualified second supplier | Single-source and supplier-specific interruption | Quality assessment, comparability, validation as applicable and regulatory approval | May share the same upstream KSM, geography or logistics route |
| Inventory buffer | Short-duration supply interruption or transport delay | Demand, shelf-life, storage, replenishment and release analysis | Can expire, tie up capital and postpone structural fixes |
| Alternate manufacturing site | Site outage, local disaster or capacity constraint | Technology transfer, process validation, analytical work and regulatory assessment | Transfer can be slow and may require new site or product approvals |
| Continuous manufacturing | Selected process, throughput and flexibility constraints | Q13-aligned development, control strategy, facility and lifecycle planning | Does not eliminate upstream material or supplier dependence |
| Regional or multi-site network | Geographic concentration and transport disruption | Capacity, quality systems, common specifications and coordinated oversight | Risk can reappear in shared inputs, utilities or specialist services |
Evaluate every option against risk reduction, quality and regulatory readiness, time to usable supply, scalable capacity, carrying and transfer costs, shelf-life constraints and residual concentration. The strongest option is the one that addresses the actual chokepoint and can deliver releasable material within the product’s recovery window.
What FDA statistics do—and do not—show about China
FDA’s Report on the State of Pharmaceutical Quality FY2024 covers fiscal years 2020 through 2024 and reports a narrowly defined subgroup: API manufacturing sites subject to specified FDA regulatory actions that exclusively supplied compounding pharmacies.
- 72%: the share of those regulatory-action sites that exclusively supplied compounding pharmacies. The same report says this supplier type represented 18% of API manufacturers in FDA’s Site Catalog.
- 51%: the share of violative sites in that compounding-API subgroup located in China; 30% were in India.
- 40%: the share of quality-related import alerts based on section 704(a)(4) record requests to sites not inspected by FDA.
These figures are surveillance and subgroup statistics. They are not estimates of China’s share of all U.S. APIs, finished medicines or pharmaceutical manufacturing. The report’s surrounding advice is directed to compounders: “This is why FDA urges compounders to know their suppliers of bulk drug substance or API, and excipients, and know whether the supplier is testing the component to ensure it is of appropriate quality to use in drugs.”

Stopping the map at the finished-product vendor
Failure: the company believes it is diversified because two contract manufacturers are approved, while both buy the same Chinese KSM.

Calling an unqualified candidate a second source
Failure: procurement signs a commercial agreement but quality and regulatory work remains incomplete.
Fix: label the candidate “under qualification” until required studies, approvals and release procedures are complete.
Buying inventory without a recovery plan
Failure: stock is increased, but no one knows which supplier, site or approval would be activated when it runs out.
Fix: connect each buffer to an owner, trigger, alternate capacity and documented allocation rule.
Treating relocation as a complete solution
Failure: production moves to a new country but retains the same API, KSM, utility or shipping bottleneck.
Fix: measure residual concentration after every network change and diversify the actual constrained input.
Assuming draft guidance is binding
Failure: a draft FDA recommendation is presented as a legal requirement or universal template.
Fix: identify the status of each document. The 2022 risk-management-plan guidance and February 2024 notification guidance are drafts; Q13 from March 2023 is final Level 1 guidance.
A practical implementation sequence
- Inventory dependencies: create the material, site and logistics map for every critical product.
- Prioritize: rank nodes by patient impact, recovery time, detectability, quality history and alternative availability.
- Choose interventions: combine qualified sources, alternate sites, inventory, capacity reservations and process changes according to the chokepoint.
- Complete qualification: perform supplier, technical, validation and regulatory work before relying on the alternative.
- Set monitoring: track supplier performance, quality signals, shipment status, inventory time and upstream changes.
- Exercise response: test decision rights, communications, allocation, testing and release under realistic disruption scenarios.
- Review continuously: update the map and risk ranking after supplier changes, inspections, demand shifts, process changes and regulatory decisions.
Done this way, reducing China-linked exposure becomes a controlled quality and continuity program rather than a disruptive geopolitical reaction. The objective is not to remove one country from every supply chain at any cost; it is to ensure that a single upstream failure cannot unexpectedly stop safe, approved production.
