How Much Do GLP-1 Weight-Loss Drugs Cost Without Insurance in 2026?

For U.S. patients paying cash, there is no single GLP-1 price. Current manufacturer-published offers start at $149 per month for the Wegovy pill, $199 per month for new Wegovy pen patients for a limited time, $399 per month for the Wegovy HD pen, and $299 per month for Zepbound 2.5 mg in a KwikPen or vial. These are starting offers tied to specific products, doses, devices and fulfillment routes—not universal prices for every prescription. A valid prescription is required, and taxes, dispensing fees, dose changes and offer terms can change your bill.

Current cash prices at a glance

The figures below are the latest 2026 prices published by the manufacturers or the Centers for Medicare & Medicaid Services. A “starting” amount does not guarantee the same price at every dose or for every device.

As an Amazon Associate I earn from qualifying purchases.

Medicine and presentation Published price Who and what it applies to
Wegovy pill $149 per month starting self-pay price Uninsured or self-paying patients using the qualifying NovoCare offer or pharmacy route; prescription required
Wegovy pen $199 per month starting self-pay price New pen patients; described as a limited-time offer
Wegovy HD pen $399 per month starting self-pay price Self-pay starting offer for the HD pen presentation
Wegovy injection, 2.4 mg $1,349.02 per package list price NovoCare’s listed price; not the same as a cash offer or every patient’s out-of-pocket cost
Zepbound 2.5 mg KwikPen or vial $299 per month starting self-pay price Lilly self-pay offer; excludes the single-dose pen
Medicare GLP-1 Bridge covered formulations $50 monthly copayment Only eligible Medicare beneficiaries meeting program, prescription and coverage conditions

These amounts should not be compared as if they were all the same kind of price. A list price, a limited-time manufacturer cash offer, a commercial-insurance copay and a Medicare program copayment each has different eligibility rules.

Why the same GLP-1 prescription can cost different amounts

List price versus a self-pay offer

A list price is a reference price for a package. NovoCare lists $1,349.02 for a package of Wegovy injection 2.4 mg. That figure does not establish what an uninsured patient will pay after using a manufacturer self-pay offer, nor does it tell you the price of other doses or presentations.

Self-pay prices are conditional discounts. The published Wegovy amounts require the specified savings offer or NovoCare Pharmacy route, while the Zepbound amount is tied to Lilly’s self-pay terms. The pharmacy filling the prescription, the device selected and the offer’s current rules all matter.

Dose and device

Manufacturers publish prices by presentation, not simply by brand name. Wegovy’s pill, standard pen and HD pen have different starting figures. Lilly’s $299 Zepbound amount is identified with the 2.5 mg starting dose and applies to a KwikPen or vial, not the single-dose pen. The 2.5 mg Zepbound amount is a starting dose, not an approved maintenance dose, so you should not assume that the same monthly amount applies after titration.

Coverage category

A $25 Wegovy amount shown by NovoCare is for eligible people with commercial insurance. It is not an uninsured cash price. Medicare’s $50 GLP-1 Bridge copayment is conditional program coverage, not an offer available to every person without insurance. Keeping those categories separate prevents an advertised copay from being mistaken for a cash price.

Wegovy: what uninsured patients can expect to pay

Wegovy pill: starting at $149 per month

NovoCare’s 2026 self-pay information lists the Wegovy pill from $149 per month. This is a starting amount under the specified self-pay arrangement, not a promise that every strength, refill or pharmacy will cost $149. Confirm which dose and dispensing option the offer covers before your prescription is processed.

Wegovy pen: $199 per month for new patients for a limited time

New patients using the Wegovy pen are listed at a starting self-pay price of $199 per month for a limited time. “New patient” and “limited time” are material conditions: an existing patient, a renewal after the offer ends or a different device may not qualify. Ask the dispensing pharmacy to confirm both your eligibility and the offer’s expiration or renewal terms.

Wegovy HD pen: starting at $399 per month

The Wegovy HD pen has a published starting self-pay amount of $399 per month. Because this is a separate presentation, do not use the standard pen’s $199 figure as a proxy for the HD pen. The offer route and any pharmacy or fulfillment requirements still apply.

Why the $1,349.02 figure can appear beside lower prices

NovoCare’s $1,349.02 figure is a package list price for 2.4 mg injection. A patient may see that number on a list-price page while qualifying for a lower self-pay offer through the manufacturer’s designated route. Conversely, a patient who does not meet the offer conditions, uses another route or receives a different presentation may face a different amount. Treat the list price as a reference, not as a universal uninsured bill.

Zepbound: the $299 offer and its limits

What the $299 amount covers

Lilly lists a $299-per-month starting self-pay offer for Zepbound 2.5 mg in a KwikPen or vial. The page expressly says the offer does not apply to the single-dose pen. The 2.5 mg strength is the starting dose, so your later prescription may not fit the same price terms.

Where the self-pay card can be used

Lilly says the Zepbound KwikPen self-pay card is for patients with a valid on-label prescription. In March 2026, Lilly said the card became available at major pharmacies in addition to LillyDirect. Availability does not eliminate the need to verify the specific pharmacy, device and current card terms. Vials and the single-dose pen have different eligibility under the cited offer.

Medication-use restrictions that affect your decision

Lilly states: “Zepbound contains tirzepatide and should not be used with other tirzepatide-containing products or any GLP-1 receptor agonist medicines.” Zepbound is used with a reduced-calorie diet and increased physical activity. Discuss candidacy, dose escalation and interactions with a licensed clinician rather than selecting a product solely by its introductory price.

Medicare’s GLP-1 Bridge is not general uninsured pricing

Who may qualify

The Medicare GLP-1 Bridge began July 1, 2026. Eligible beneficiaries may have a $50 monthly copayment for specified medicines and formulations. The program requires Medicare Part D enrollment, a prescription and clinical eligibility; prior authorization may also be required.

Covered products and excluded presentations

The official fact sheet lists Wegovy injection or tablet and the Zepbound KwikPen. It expressly excludes Zepbound single-dose pens and vials. Therefore, a beneficiary cannot assume that every Zepbound device qualifies simply because the brand appears in the program materials.

Provider and lifestyle-program requirements

Medicare says a participating provider must prescribe a covered drug and complete prior authorization when requested. The provider must certify use alongside a lifestyle program focused on diet and exercise. The $50 copayment does not count toward the beneficiary’s yearly Part D deductible or out-of-pocket limit, which makes it different from ordinary Part D cost sharing.

How to estimate your own monthly cash cost

  1. Identify the exact medicine. Write down whether the prescription is for Wegovy or Zepbound and verify that it is the product your clinician prescribed.
  2. Record the presentation. Note pill, standard pen, HD pen, KwikPen, vial or single-dose pen. Device differences can change eligibility even within one brand.
  3. Confirm the dose. Ask whether the quoted amount is for an introductory strength or a maintenance dose. For Zepbound, the $299 figure is tied to 2.5 mg, the starting dose.
  4. Check your coverage category. Tell the pharmacy whether you are paying cash, using commercial insurance, or seeking Medicare Bridge coverage. Do not substitute the $25 commercial-insurance Wegovy copay for an uninsured estimate.
  5. Verify the required route. Ask whether the offer requires NovoCare Pharmacy, LillyDirect or a participating major pharmacy, and whether a savings card must be activated before billing.
  6. Ask for the total at checkout. Request the medication price plus dispensing fees, delivery charges, taxes where applicable and any other amount the pharmacy will collect.
  7. Check renewal terms. A limited-time or new-patient offer may change at refill. Confirm how long the price remains available and what happens if your dose or device changes.

How to compare offers without choosing the wrong headline price

Comparison question Why it changes the answer
Is the figure a list price, cash offer or copayment? List prices are references; cash offers and copayments require separate eligibility.
Which dose is quoted? A starting dose may not be the maintenance dose used later.
Which form or device is included? Wegovy pill, pen and HD pen have separate prices; Zepbound KwikPen, vial and single-dose pen are not interchangeable for the offer.
Is the offer limited or restricted to new patients? The Wegovy pen amount is described as limited-time and for new pen patients.
Which pharmacy route is required? Manufacturer offers can depend on a designated pharmacy, savings card or participating location.
Are you eligible for the program? Commercial insurance, self-pay and Medicare Part D use different rules.
Are fees included? The published monthly amount may not state delivery, dispensing, tax or other pharmacy charges.

Using this checklist is more reliable than ranking medicines by the smallest number in an advertisement. The lowest introductory amount may apply only to one device, one dose and one fulfillment channel.

Prescription, eligibility and safety checks

A prescription is required

Both manufacturer self-pay routes require a valid prescription. A pharmacy or savings program may also require that the prescription be on-label for the product and that the patient satisfy the offer’s terms. A price displayed online does not authorize treatment or guarantee that a clinician will prescribe it.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Ask about clinical suitability before price

Discuss your medical history, other medicines, dose plan and expected monitoring with a licensed clinician. For Medicare Bridge coverage, the prescriber must document the qualifying use and lifestyle-program requirement. For Zepbound, do not combine it with another tirzepatide-containing product or another GLP-1 receptor agonist unless your clinician specifically addresses a transition under current prescribing information.

Plan for changes over time

Cash offers can change after the published date, and a manufacturer can alter qualifying devices, pharmacies or renewal rules. Keep the current offer terms with your prescription records and recheck the price before each major dose or device change.

Common billing problems and what to do

The pharmacy quotes the list price

Ask whether the claim was processed as a self-pay offer and whether the prescription was sent through the required manufacturer pharmacy route. If the offer is limited to a particular presentation, verify that the prescribed device matches it.

The savings card is rejected

Confirm that the prescription is valid and on-label, that the product and device are covered by the card, and that the pharmacy is participating. For Zepbound, a card intended for the KwikPen will not establish eligibility for a vial or single-dose pen.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Your refill costs more than the first month

Check whether the initial amount was a new-patient or limited-time price, whether the dose changed, and whether the offer has a monthly or annual limit. Ask for the exact reason for the change rather than assuming the pharmacy made an error.

Medicare says prior authorization is needed

Contact the prescribing provider to complete the requested documentation. Until approval is recorded, the Bridge copayment may not apply. Verify that the prescribed product and formulation appear on the program’s covered list; Zepbound vials and single-dose pens are excluded in the cited fact sheet.

Questions to ask before filling the prescription

  • What exact amount will I pay today for this dose and device?
  • Is that amount a list price, a self-pay offer or a copayment?
  • Does the price require NovoCare Pharmacy, LillyDirect, a savings card or a participating pharmacy?
  • How long does the offer last, and does it apply to refills?
  • What will the price be if my clinician changes the dose or presentation?
  • Are delivery, dispensing fees or taxes added?
  • If I use Medicare, is prior authorization required and is this formulation covered?
  • What monitoring, diet and activity plan should accompany treatment?

What to remember when budgeting

Budget from the exact prescription you expect to fill, not from a brand-level headline. In 2026, published starting cash offers span $149 to $399 for the listed Wegovy presentations and $299 for the specified Zepbound 2.5 mg KwikPen or vial. The $1,349.02 Wegovy figure is a 2.4 mg injection package list price, while $25 is a commercial-insurance Wegovy copay and $50 is a conditional Medicare Bridge copayment. Those figures answer different questions.

Before treatment starts, have the prescriber confirm the product and dose, then have the pharmacy confirm eligibility, route and total charges. Recheck terms whenever the dose, device, pharmacy or coverage changes.
