How Medicare Savings Programs Affect Part B Premiums and Social Security Payments

Yes—an eligible Medicare Savings Program (MSP) can pay your Medicare Part B premium, and your Social Security deposit may increase because the premium is no longer withheld. The program does not raise your gross Social Security benefit or create a cost-of-living increase. It changes who pays the premium: after the state’s Medicare buy-in is active, the federal government can stop collecting Part B premiums from your benefit payment.

The practical effect depends on which MSP you receive, your state’s rules and effective date, and whether the change has reached Medicare and Social Security systems. A deduction can continue temporarily after approval, and a deduction can reappear after eligibility ends.

What changes when an MSP pays Part B

Most people with Medicare Part B have the premium deducted automatically from Social Security or Railroad Retirement Board (RRB) benefits. People who do not receive either payment generally receive a Medicare premium bill instead. Medicare describes both collection methods on its premium-payment page.

For QMB, SLMB and QI participants, a state can use the Medicare buy-in process to pay the Part B premium. The Social Security Administration (SSA) explains that buy-in allows the federal government to bill the state and stop collecting the premium from the person’s Social Security, RRB or Civil Service Retirement deduction, or from CMS direct billing (SSA POMS HI 00815.001).

Suppose your monthly Social Security benefit is $1,500 before deductions and $202.90 is withheld for the standard 2026 Part B premium. Once state buy-in is effective and reflected in your account, the deposit could be about $202.90 higher. Your gross benefit remains $1,500; only the withholding changes. The standard premium is $202.90 per month in 2026, although people with higher income may owe more (Medicare 2026 costs).

That distinction matters when reading a benefit statement. “Net payment” or “amount deposited” can rise while the Social Security award amount stays unchanged. The change is not a Social Security increase and does not alter your Medicare coverage by itself.

Which Medicare Savings Program pays which costs?

The state decides which MSP applies. The programs do not provide identical protection. Medicare’s program descriptions and 2026 limits are at Medicare Savings Programs.

Program Premiums covered Other help or condition
QMB (Qualified Medicare Beneficiary) Part B and, when owed, Part A premiums Also pays Medicare-covered deductibles, coinsurance and copayments. Providers generally may not bill a QMB beneficiary for Medicare-covered cost sharing, subject to program rules.
SLMB (Specified Low-Income Medicare Beneficiary) Part B premium Requires Part A and Part B. Medicare says SLMB enrollment also confers eligibility for the federal Extra Help program for prescription-drug costs.
QI (Qualifying Individual) Part B premium Requires Part A and Part B; limited to people who do not qualify for other Medicaid coverage or benefits. You must apply each year, and states give priority to people who received QI the prior year.
QDWI (Qualified Disabled and Working Individual) Part A premium only For qualifying disabled people who work and lost premium-free Part A after returning to work. QDWI does not pay the Part B premium.

Therefore, an MSP approval does not automatically mean every Medicare bill disappears. QMB has the broadest Medicare cost-sharing protection in the table. SLMB and QI generally address the Part B premium, while QDWI addresses only Part A. Ask your state which program and effective dates appear on your notice.

2026 income and resource figures—and why they are not universal cutoffs

Medicare lists the following federal screening figures for 2026. They are monthly income limits, paired below with the listed resource limits for QMB, SLMB and QI. Alaska and Hawaii have somewhat higher figures, and states may set higher limits or disregard certain income or resources. The state application decision—not the table alone—determines eligibility.

Program (2026) Monthly income: individual Monthly income: married couple Resources: individual / couple
QMB $1,350 $1,824 $9,950 / $14,910
SLMB $1,616 $2,184 $9,950 / $14,910
QI $1,816 $2,455 $9,950 / $14,910

These figures come from Medicare’s 2026 MSP information (Medicare.gov). Federal limits increase over time, and state counting rules can change the result. Someone slightly above a displayed figure should still contact the state Medicaid agency rather than assuming an application is futile.

QI has additional renewal and funding rules

QI is not an open-ended entitlement in the same way as ordinary Medicaid coverage. Medicare says QI applicants must apply every year; states process applications on a first-come, first-served basis and give priority to people who received QI the previous year. QI is limited to people who do not qualify for other Medicaid coverage or benefits. A missed renewal or a state funding limit can end the Part B buy-in even when income has not changed.

How Part B withholding appears on your payments

If you receive Social Security or RRB benefits

Your benefit statement normally shows a gross benefit, deductions and a net payment. The Part B line is a collection entry. When buy-in is active, that line can be removed or reduced, increasing the amount deposited without changing the gross award. Medicare explains the ordinary deduction process at How to Pay Part A & Part B premiums; SSA explains the state buy-in mechanism at HI 00815.001.

If you do not receive a monthly benefit

Without a Social Security or RRB payment from which to withhold premiums, Medicare generally sends a premium bill. State buy-in can stop that direct billing once the account is updated, but an approval notice and a billing statement may cross in the mail. Keep both documents until the effective date and payment status agree.

Higher-income Part B premiums

The $202.90 figure is the standard 2026 Part B premium. Medicare charges an income-related amount to some beneficiaries. An MSP’s coverage is tied to the Part B premium obligation recorded for your account; do not assume the standard figure is the amount that would otherwise be withheld in every case.

When buy-in starts—and why the first payment may not change

SSA says Part B buy-in generally begins when a person is both entitled to Part B and in the applicable MSP coverage group. The exact start-date rule depends on the group and circumstances (SSA POMS HI 00815.018).

State approval, Medicare enrollment and federal payment processing do not always occur on the same day. A state notice may show an effective date before a Social Security payment has been recalculated. Do not treat one unchanged deposit as proof that the state denied coverage. Compare the effective date on the state notice with the premium line on your Medicare or benefit statement.

What happens when MSP eligibility ends?

Ending buy-in reverses the payment arrangement: you become responsible for the Part B premium again. SSA cautions that a benefit payment can take about two months to reflect the end of state buy-in. After it ends, SSA may collect as many as three months of Part B premiums in one monthly payment—the current month plus two retroactive months (SSA POMS HI 00815.042).

If no monthly benefit is payable, Medicare may send a Medicare Premium Bill instead. The timing and collection method can make the first bill or deduction look larger than expected; it does not necessarily represent a permanent increase in the monthly premium.

Relief for retroactive amounts

SSA describes two options for people who cannot afford retroactive premiums after buy-in ends: request a waiver or ask about installment payments. Contact the office identified on your notice promptly; do not ignore a bill while waiting for a correction.

How to resolve a deduction that does not match your approval

  1. Read the state notice. Record the MSP name, approval or termination date, and any renewal deadline. The state Medicaid agency—not Medicare—makes the MSP eligibility decision.
  2. Check Medicare billing information. Look for the Part B premium amount and whether a premium bill was issued. Save statements showing the month of each charge.
  3. Check the Social Security payment record. Compare the gross benefit, Part B deduction and net deposit for the first payment after the state effective date. A two-month processing lag is specifically described for an end of buy-in; do not assume every start follows the same schedule.
  4. Contact the right agency. Ask the state Medicaid agency to verify that its buy-in transaction was transmitted with the correct Medicare number and effective date. Ask Social Security or Medicare to explain a federal deduction or bill that remains after the state confirms transmission.
  5. Protect coverage while correcting the record. If a bill is due, ask whether a hold, waiver review or installment arrangement is available. Keep proof of every call, notice and payment.

Use the state’s written effective date as the reference point, but recognize that a payment system can update later. Neither Medicare nor Social Security promises that an application will change the very next deposit.

Examples of how the numbers work

Approval before a payment cycle closes

Maria receives $1,800 in Social Security before deductions. Her 2026 statement withholds the standard $202.90 Part B premium, leaving $1,597.10. Her state approves QMB with an effective date that reaches the next payment cycle. Once buy-in posts, the Part B line can disappear and her deposit can move to about $1,800. Her gross Social Security amount has not increased; the $202.90 withholding has stopped. QMB may also address Medicare-covered cost sharing under its rules.

Approval after a deduction has already been taken

James receives an SLMB approval dated June 1, but his June payment was processed before the transaction reached federal systems. The deduction can still appear in June. He should retain the statement, verify the state’s effective date and ask whether a later payment or account adjustment will reflect the covered month. The approval does not guarantee an immediate refund or next-day change.

Loss of QI eligibility

Rita’s QI coverage ends December 31. Her January statement still shows no Part B deduction because the termination has not yet posted. When the update arrives, SSA may collect January plus as many as two earlier months in one payment. She should review the notice and ask about a waiver or installments if the retroactive amount is unaffordable.

Application and renewal decisions

You apply for an MSP through your state. Medicare’s program page states, “You’ll apply for Medicare Savings Programs through your state.” Start with the state Medicaid agency’s application channel and provide the requested income, resource, Medicare and household information.

  • Apply even if your income appears slightly above Medicare’s federal table; a state may use a higher limit or disregard certain resources.
  • Ask how the state counts wages, pensions, Social Security, savings and other assets, because counting rules vary.
  • Confirm whether your state renews the program automatically or requires a new form. QI specifically requires an application every year.
  • Report changes in income, household status, Medicare entitlement or work status promptly. A change can move you between QMB, SLMB, QI, QDWI or no MSP.

Keep the approval, renewal and termination notices. They establish the dates needed to reconcile a Social Security deduction or Medicare bill.

Bottom line for your Social Security check

An MSP can make your monthly deposit larger by removing the Part B premium deduction, but it does not increase your Social Security benefit itself. QMB, SLMB and QI can trigger state buy-in for Part B; QDWI cannot. The amount and timing depend on your program, state rules, effective date and federal processing. Use the state notice, Medicare billing record and Social Security deduction together, and act quickly if the records disagree.
