How Medicare premiums and other deductions affect your Social Security payment

Your Social Security benefit has a gross amount and a net payment. The net deposit can be lower because Social Security withholds Medicare premiums, recovers an overpayment, adjusts for excess earnings, honors voluntary tax withholding, pays an appointed representative, or responds to a legal garnishment or levy. Medicare is only one possible deduction. The itemized lines on your latest Social Security notice or payment record determine what happened in your case.

Gross benefit, deductions, and the amount that reaches your bank

Social Security calculates a gross monthly benefit before withholding. “Deductions” are amounts taken from that monthly payment, so the net amount deposited to your bank can be smaller even when your underlying benefit has not changed. A lower deposit is not, by itself, proof that your benefit was reduced permanently.

Start with the most recent benefit notice, payment record, or online account entry. Compare the gross benefit with the net payment, then read each itemized deduction line. The label usually points to the agency or process you must contact. A Medicare line calls for a Medicare or Social Security billing check; an overpayment line follows Social Security’s appeal or waiver process; a levy or garnishment generally requires contacting the court, IRS, or Treasury office named in the notice.

Medicare premiums that can appear as Social Security deductions

Social Security consumer guidance says the amount displayed as deducted from a benefit may include the monthly Medicare Part B premium, a Medicare Advantage Part C premium, and a prescription drug Part D premium. Higher-income beneficiaries may also owe income-related monthly adjustment amounts (IRMAA) for Part B and/or Part D. Part A usually has no monthly premium.

The display covers amounts Social Security actually withholds from the benefit. It does not include a Medicare premium paid by another person, sent by check, or taken automatically from a bank account. Therefore, a Medicare premium can exist without appearing as a Social Security deduction, and a deduction line does not necessarily show every Medicare charge associated with your coverage.

2026 Medicare figures

The following figures are for 2026 and come from the Social Security Administration’s Office of the Chief Actuary 2026 Trustees summary. Premiums and thresholds can change in a later year, so attach the year whenever you use one of these numbers.

2026 item Amount or threshold How to read it
Standard Part B premium $202.90 per month The standard monthly premium before any income-related addition.
Part B IRMAA income thresholds $109,000 for an individual filer; $218,000 for a joint filer Modified adjusted gross income thresholds stated in the 2026 Trustees summary.
Additional Part B IRMAA $81.20 to $487.00 per month The added monthly amount varies by income and tax-filing status.
Part D base beneficiary premium $38.99 per month A national base figure; an individual prescription drug plan’s actual premium can differ.

These are reference figures, not a way to calculate an individual deduction from a bank deposit. Your plan’s Part D premium, an IRMAA tier, past-due Medicare premiums, or a different billing arrangement can produce a different line on your record.

Why IRMAA can make the deduction larger

IRMAA is based on modified adjusted gross income and filing-status information supplied by the Internal Revenue Service. Social Security uses that information to determine whether the beneficiary owes more than the standard Part B premium and whether an additional Part D amount applies.

When a later income drop may support a new review

A tax return from an earlier year can overstate your current household income after a major event. Social Security lists marriage, divorce, the death of a spouse, loss of income, and an employer settlement payment as examples of qualifying life-changing events. A qualifying event does not automatically lower IRMAA; it gives you a basis to ask Social Security to make a new determination.

Use Form SSA-44 and provide the supporting evidence requested by Social Security, or contact the agency for help. Evidence can include documentation of the event and proof of the resulting income change. The review concerns the additional Part B or Part D amount, not whether you have Medicare coverage.

New initial determination versus reconsideration

Choose the route that matches the problem. A qualifying life-changing event, an amended tax return, or corrected IRS information can support a new initial determination. If you believe the IRMAA decision itself is wrong, you or your representative can request reconsideration within 60 days after receiving the IRMAA determination notice. The notice controls the deadline and instructions, so keep the envelope or electronic delivery record.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Neither route guarantees a lower premium. Social Security evaluates the event, income evidence, filing status, and applicable rules before changing the adjustment.

Other deductions Social Security may withhold

Social Security’s public definition includes several deductions that are unrelated to Medicare. The source of the deduction determines both the explanation and the remedy.

Deduction or withholding What it means First response
Medicare premium payment Part B, Part C, Part D, an IRMAA amount, or recovery of past-due Medicare premiums may be collected through the benefit. Match the line to your Medicare billing arrangement and check whether any premium is being paid by check, bank draft, or another person.
Social Security or SSI overpayment recovery Social Security says an overpayment is a benefit payment greater than the amount due. Read the overpayment notice, then decide whether to appeal the existence or amount or request a waiver based on repayment hardship and fault or fairness.
Excess earnings Work and earnings rules can require withholding when earnings affect benefits before full retirement age. Review the earnings information and the notice explaining the adjustment; contact Social Security if the reported work or amount is wrong.
Voluntary federal income-tax withholding You elected to have federal income tax withheld from the benefit. Check the withholding election and change it through the process Social Security provides if your tax situation has changed.
Payment of an appointed representative An authorized representative’s approved fee can be withheld from benefits under the applicable approval. Review the representative-fee approval and payment explanation before disputing the amount.

A deduction can also be imposed through a legal garnishment or levy. Those actions are separate from ordinary SSA deductions and follow the authority identified in the notice.

Garnishment, levy, and federal debt withholding

Social Security identifies several circumstances in which benefits may be subject to garnishment or withholding: child, spousal, or family support obligations; court-ordered victim restitution; unpaid federal taxes; withholding for a current-year federal tax liability; and delinquent federal non-tax debts owed to another federal agency.

The rules and amounts depend on the debt and the order. Do not apply a protection or percentage from one debt type to another. For an IRS levy for overdue federal tax debt, the IRS authority described in Social Security’s FAQ permits a levy of up to 15% of each Social Security payment. Treasury may withhold benefits for delinquent federal non-tax debt. Support and restitution deductions depend on the applicable court order and law.

For a court-ordered garnishment, contact the court that issued the order. For an IRS levy, contact the IRS. For a federal non-tax debt, contact Treasury or the agency identified in the notice. Social Security can explain the payment record, but it generally cannot cancel an order issued by another authority.

If the deduction is an overpayment

An overpayment means Social Security says you received more than you were due. The notice should state the claimed reason, amount, and instructions. Treat two different questions separately:

  • Appeal: Use this when you disagree that an overpayment occurred or disagree with the amount.
  • Waiver: Use this when you accept that an amount may be owed but cannot afford repayment and believe the error was not your fault or that recovery would be unfair under the applicable standard.

Social Security’s current consumer guidance says it waits at least 30 days after sending an overpayment notice before collection. If no payment, appeal, or waiver action is taken in the relevant period, the guidance describes automatic monthly withholding. The exact recovery rate, start date, and available options depend on the benefit type and the notice you received. Follow the notice rather than relying on a general timetable.

Submit the appeal or waiver request using the forms, address, online method, or telephone instructions in the notice. Keep copies, proof of delivery, and records of any requested documents. A general explanation cannot determine your fault, waiver eligibility, or recoverable balance.

A practical process for finding out why your payment changed

  1. Record both figures. Write down the gross benefit, each deduction, and the net payment from the same payment record. Do not compare a current net deposit with an old gross amount.
  2. Identify the exact label. Look for Part B, Part C, Part D, IRMAA, overpayment, excess earnings, tax withholding, representative payment, levy, or garnishment wording.
  3. Check Medicare billing routes. If a Medicare deduction appears, determine whether it is the standard premium, an IRMAA, a plan premium, or past-due premium recovery. Confirm whether another Medicare charge is being paid by check, automatic bank deduction, or someone else.
  4. Compare the year. If the line relates to Medicare, compare it with the applicable year’s premium notice. The 2026 standard Part B premium is $202.90, but a plan premium or IRMAA can change the total.
  5. Review income changes. If IRMAA appears to rely on income that has since fallen, check whether a qualifying life-changing event applies and assemble the evidence for Form SSA-44.
  6. Protect appeal deadlines. For an IRMAA reconsideration, Social Security’s appeals manual states a 60-day period after receipt of the determination notice. An overpayment notice has its own deadlines and instructions.
  7. Match the remedy to the problem. Appeal an incorrect overpayment, request a waiver when hardship and fault or fairness are the issue, and contact the issuing court or agency for a garnishment or levy.
  8. Keep a case file. Save notices, payment records, tax information, Medicare bills, forms, evidence, confirmation numbers, and notes from calls.

Common payment-change scenarios

The deposit fell by about the standard Part B amount

A 2026 net payment that is roughly $202.90 lower could be consistent with the 2026 standard Part B premium, but the amount alone does not prove that is the cause. Check the itemized line and whether an IRMAA, Part C or Part D premium, or past-due premium recovery is included.

Medicare says a premium is due, but no Medicare line appears

Social Security’s displayed deduction excludes premiums paid by another person, by check, or through an automatic bank deduction. Check the Medicare billing notice and bank records before concluding that the premium was missed or removed.

IRMAA is based on a year when income was much higher

Gather proof of the qualifying event and the lower income, then ask Social Security about a new initial determination and Form SSA-44. If you are disputing the determination rather than reporting a qualifying event, use the reconsideration process and the deadline on the notice. A request is not an automatic reduction.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The notice says overpayment, but the amount looks wrong

That is an appeal issue. Compare the notice’s months, benefit type, reported income or event, and calculation with your records. If repayment would be unaffordable and you believe you were not at fault or recovery would be unfair, consider a separate waiver request as instructed by Social Security.

The line names a levy or garnishment

Do not treat it like a Medicare premium or ordinary SSA adjustment. Use the notice to identify the court, IRS, Treasury, or other federal agency with authority, and contact that authority about the order, balance, or dispute.

What a general explanation cannot tell you

No national guide can identify the exact reason a particular payment changed. The controlling evidence is your current benefit record, Medicare billing arrangement, tax and income information, and the notice that explains the deduction. Premium amounts, timing, appeal rights, and recovery rules can differ by benefit type and year. When the itemized record and a general explanation conflict, follow the current official notice and ask the responsible agency for a case-specific explanation.
