How Medicare Part D’s Extra Help Program Lowers Prescription Costs in 2026

Medicare Part D’s Extra Help program can reduce or pay part of your prescription-drug plan premium, deductible, coinsurance and other costs if you have limited income and resources. Some people qualify automatically through Medicaid, a Medicare Savings Program or Supplemental Security Income (SSI); everyone else can apply through Social Security.

For 2026, Medicare lists annual income limits of $23,940 for one person and $32,460 for a married couple. The corresponding resource limits are $18,090 and $36,100. Those figures are for the 50 states and the District of Columbia; Alaska and Hawaii have higher limits. Your final costs still depend on the Part D plan, covered drugs and pharmacy you use.

What Extra Help pays

Medicare’s official description says: “Extra Help” is a Medicare program to help people with limited income and resources pay Medicare drug coverage (Part D) premiums, deductibles, coinsurance, and other costs. The assistance is attached to your Part D coverage, so it lowers what you owe under the plan rather than replacing the plan’s drug list or pharmacy rules.

Depending on your eligibility category and plan, Extra Help may:

  • Pay some or all of your monthly Part D premium, subject to plan and area rules.
  • Reduce or eliminate the plan deductible.
  • Lower your copayments or coinsurance for covered prescriptions.
  • Protect you from the Part D late-enrollment penalty while you receive the benefit.

There is no specific medication, commercial service or physical product you must buy to receive Extra Help. You need Medicare Part D coverage or an appropriate plan assignment, and the drug must be covered by that plan.

Do I qualify for Medicare Extra Help?

2026 income and resource limits

Medicare’s 2026 overview lists these annual limits:

Household situation 2026 annual income limit 2026 resource limit
Individual $23,940 $18,090
Married couple $32,460 $36,100

The displayed amounts exclude the higher limits used for Alaska and Hawaii. Limits can change each year, and the figures alone do not determine every application. Social Security and Medicare consider the information on your application and the rules in effect for the benefit year.

What counts as resources

Medicare’s April 2026 publication describes countable resources as money in checking, savings or retirement accounts, plus stocks and bonds. It lists common exclusions, including:

  • Your home.
  • Personal belongings and household furniture.
  • One car.
  • Burial plots.
  • Qualifying set-aside burial expenses up to $1,500.
  • Other household and personal items identified in the program rules.

Do not make a final eligibility decision from a quick calculation. Gather your records and use the official application or speak with Social Security or a State Health Insurance Assistance Program (SHIP) counselor for an individual determination.

People who may qualify automatically

You generally do not need a separate Extra Help application if you receive one of these benefits:

  • Compact for travelling
  • Full-benefit Medicaid.
  • State assistance with your Medicare Part B premium through a Medicare Savings Program.
  • Supplemental Security Income (SSI) payments from Social Security.

Medicare says people who qualify automatically should receive a notice. Keep that notice: it explains your status and may include information about your assigned or available Part D plan.

How the savings appear on your Part D bill

Premiums and deductibles

Medicare’s April 2026 publication says qualifying enrollees are enrolled in a drug plan with a $0 premium and $0 deductible under the applicable Extra Help rules. The amount of assistance can depend on the plan and service area, so check the notice and the plan’s current information rather than assuming every plan has identical terms.

If you do not have a drug plan when you qualify, Medicare says you may be enrolled automatically in one so you can receive the savings. You can select a different plan, but choosing one that does not meet the relevant $0-premium and $0-deductible terms could leave you paying more.

Copayments and covered drugs

Extra Help does not make every prescription free and does not require a plan to cover every drug. Your plan’s formulary determines whether a drug is covered, and the pharmacy you choose can affect the price. Review the plan’s covered-drug list, preferred-pharmacy network and current cost information before changing plans or pharmacies.

Medicare’s overview page currently displays the following figures under “What you’ll pay under Extra Help in 2027”: $0 plan premium, $0 deductible, up to $5.80 for a generic drug and up to $14.40 for a brand-name drug, with $0 for covered drugs after the stated $2,400 total-drug-cost point. These are explicitly 2027 figures, not 2026 amounts. Do not use them to predict a 2026 copay.

Late-enrollment penalty protection

Extra Help recipients do not pay the Part D late-enrollment penalty while they receive the assistance. That protection is important if you delayed Part D enrollment, but it does not remove the need to enroll, review your coverage or follow your plan’s rules. A lapse in eligibility or coverage can change what you owe.

Year-specific Part D numbers to keep separate

Several Medicare figures are easy to mix up because they apply to different years or describe different parts of the benefit:

Figure Year and meaning
$23,940 individual income; $32,460 married-couple income 2026 Extra Help annual income limits listed by Medicare
$18,090 individual resources; $36,100 married-couple resources 2026 Extra Help resource limits listed by Medicare
$615 General 2026 Part D deductible ceiling; this is a Part D context figure, not a personal Extra Help deductible quote
$2,100 General 2026 Part D covered-drug out-of-pocket threshold; certain payments made on your behalf, including Extra Help, count toward the threshold under Medicare rules
$0 premium and deductible; up to $5.80 generic and $14.40 brand-name copays Figures shown by Medicare for Extra Help in 2027, not 2026

Your plan notice is the controlling source for the copay on a particular prescription. The general Part D deductible and out-of-pocket figures do not replace the plan-specific Extra Help terms.

How do I apply for Extra Help?

You can apply before or after enrolling in Part D. Social Security accepts applications online, by phone or through an appointment. The application asks about income, resources and household circumstances.

Prepare the information you may need

Having records ready can make the application easier. Social Security identifies documents that may help, such as:

  • Recent bank statements.
  • Tax returns.
  • Current IRA or 401(k) balances.
  • Statements for pensions, veterans’ benefits, annuities or Railroad Retirement Board benefits.

These are preparation examples, not a rule that every applicant must submit every document. Follow the application’s instructions if Social Security requests verification.

Application steps

  1. Check whether you may already qualify automatically. If you receive full-benefit Medicaid, a qualifying Medicare Savings Program or SSI, watch for Medicare’s notice before submitting a duplicate application.
  2. Apply through Social Security if you are not automatic. Use the online application, call Social Security or make an appointment. You may apply even if you have not yet enrolled in Part D.
  3. Review the decision notice. If approved, the notice explains your Extra Help status and plan information. If information is missing or incorrect, follow the notice’s instructions to respond.
  4. Check your Part D plan. Confirm the plan’s premium after assistance, deductible, covered drugs, pharmacy network and copayments. Ask the plan how a prescription will be priced before filling it when the cost is unclear.
  5. Get free counseling if needed. Contact your local SHIP for help with the Extra Help application, plan selection or a coverage question.

Applying for Extra Help and a Medicare Savings Program

Medicare says you can make the Extra Help and Medicare Savings Program applications at the same time. Unless you opt out on the Extra Help application, Social Security sends information to your state to initiate an MSP application. That can help you pursue assistance with Medicare costs without completing an entirely separate first step.

What happens after approval?

If Medicare awards Extra Help, the assistance continues through December 31 of that calendar year, even if your income changes during the year. Continued help in the following year depends on meeting the limits and other rules for that year. Medicare and Social Security send notices when eligibility or plan assignment changes, so open and keep every notice.

When a plan is assigned or changed, compare it with your prescriptions and pharmacy. A $0 premium does not guarantee that every drug is covered or that every pharmacy charges the same amount. Confirm the formulary and network before relying on a new plan for an important medication.

Where Extra Help is available

Medicare says Extra Help is not available in Puerto Rico, the U.S. Virgin Islands, Guam, the Northern Mariana Islands or American Samoa. Residents of those locations should contact their local Medicaid office about other programs. The 50-state and District of Columbia limits should not be applied automatically to those territories.

How to compare Part D plans when you receive Extra Help

Extra Help lowers costs, but it does not make plan selection irrelevant. Compare these items using your current prescriptions and preferred pharmacies:

  • Premium after assistance: Verify whether the plan remains within the applicable $0-premium terms for your assistance category.
  • Deductible: Check whether the deductible is $0 under your plan’s Extra Help terms.
  • Formulary: Confirm every medication, its tier and any prior authorization, quantity limit or step-therapy rule.
  • Pharmacy network: Check whether your usual pharmacy is preferred and what you would pay there.
  • Expected annual use: Compare the cost of the drugs you actually take, not an advertised generic or a sample prescription.

Medicare states that costs vary by plan, covered medicines and pharmacy. No single plan can be identified as best for every Extra Help recipient.

Common mistakes and how to avoid them

Using the wrong year’s figures

Do not use the 2027 copays shown on Medicare’s overview page as 2026 prices. Label every income, resource, deductible and copay figure with its benefit year.

Assuming the income limits are the whole test

Household status, countable resources, exclusions and the information verified by Social Security all matter. A number near or above a displayed limit is a reason to apply or ask for help, not a substitute for an official determination.

Assuming Extra Help covers any pharmacy or drug

The plan’s formulary and pharmacy network still control coverage and pricing. Ask the plan about a drug’s tier and restrictions, and check whether another in-network pharmacy has a different cost.

Ignoring notices

Notices explain approval, plan assignment and changes for the next year. Missing one can leave you unaware that your plan or eligibility has changed.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Buying a product to obtain the benefit

Medication organizers, books and document-storage products are optional. They do not create eligibility or deliver the subsidy. Use Social Security, Medicare and SHIP for the application and counseling.

A practical checklist

  • Record your 2026 household income and countable resources.
  • Check whether Medicaid, a Medicare Savings Program or SSI may qualify you automatically.
  • Gather available financial statements without assuming every listed document is mandatory.
  • Apply through Social Security if you are not automatically enrolled.
  • Read the approval or plan-assignment notice.
  • Verify your prescriptions, formulary, pharmacy network, premium, deductible and copays.
  • Contact SHIP if you need free, unbiased help comparing coverage.
  • Recheck notices and eligibility for the next calendar year.

The bottom line for 2026

Extra Help can substantially reduce Medicare Part D costs for people with limited income and resources. In 2026, the listed income limits are $23,940 for an individual and $32,460 for a married couple, with resource limits of $18,090 and $36,100. Apply through Social Security unless you already qualify automatically, then use your plan notice and current formulary to determine what each prescription will actually cost. Keep 2026 and 2027 figures separate, because Medicare’s displayed copays and other plan terms change by benefit year.
