Short answer: workplace critical illness cover is usually easier to join and may be subsidised by your employer, but it is tied to the scheme and can end when your employment does. An individual policy is owned by you, can be designed around a long-term need, and remains separate from your job, but it requires personal underwriting and you pay the premiums. Neither is automatically better. Compare the exact illness definitions, exclusions, benefit amount, underwriting, family options, premium terms and what happens when you leave before deciding whether one policy is enough.
This comparison is for UK consumers. Critical illness insurance pays a lump sum only when a claim meets the policy’s definition of a listed condition. It is not a general replacement for income protection, and it normally does not pay a death benefit. MoneyHelper notes that common causes of time off work, including back problems and stress, are generally outside the selected-condition model used by critical illness policies: MoneyHelper guidance.

What critical illness cover actually pays for
A critical illness policy pays a one-off lump sum if you survive and meet the contract’s definition of one of the specified illnesses or operations. The definition may require a particular severity, diagnostic test, permanent impairment or survival period. A diagnosis that sounds serious in ordinary language is not automatically a valid claim.

The money can be used for any purpose, such as adapting a home, paying a mortgage, covering childcare, replacing lost earnings while recovering or funding treatment. The policy does not normally pay simply because you cannot work. Income protection is designed around inability to work and can cover a wider range of illnesses; life insurance is designed to pay on death. A critical illness policy should therefore be assessed alongside, not instead of, those protections where your household needs them.

The Association of British Insurers (ABI) 2023 guide sets minimum model standards for the critical-illness part of long-term policies, including group cover, for participating ABI members. Insurers adopted the provisions for new policies by dates no later than 16 September 2023. The guide does not force an insurer to use every model exclusion, so the policy wording remains decisive. Read the guide here: ABI guide to minimum standards.
Workplace and individual policies: the differences that matter
| Decision point | Workplace group cover | Individual policy | What to verify |
|---|---|---|---|
| Who owns and arranges it | Your employer arranges a scheme and the scheme rules determine eligibility and benefits. | You buy and own the contract, either as standalone critical illness cover or alongside life insurance. | Who can change the policy, who receives claim communications and whether the cover is linked to another contract. |
| Illnesses and definitions | The employer’s insurer sets the list, severity tests, exclusions and any partial-payment rules. Group and individual versions from the same insurer can differ. | The insurer sets its own definitions and options after assessing your application. | Exact definitions, tests, survival or waiting rules, exclusions, partial benefits and second-event provisions—not just the number of conditions. |
| Medical underwriting | There may be a free-cover limit with no individual medical questions, with evidence required above that limit. Some schemes use pre-existing and associated-condition exclusions. | Personal underwriting normally determines acceptance, premium and exclusions. | What medical evidence is requested, which prior or related conditions are excluded and whether an increase in benefit triggers new underwriting. |
| Benefit amount | The scheme may use a salary multiple, a fixed amount or an employee-selected amount. Spouse, partner or child benefits may be optional. | You choose the insured amount and family options, subject to product limits and underwriting. | Whether the amount meets your mortgage, debts, essential spending and recovery-cost needs, and whether it changes with salary or age. |
| Duration and leaving employment | Cover can be conditional on employment and the scheme’s eligibility rules. | The contract continues independently of your job while you pay premiums and satisfy its term conditions. | The exact end date on resignation, dismissal, retirement, long-term absence or a change of eligibility; any continuation or conversion route and its deadline. |
| Cost and tax | The employer may pay all, part or none of the premium; voluntary employee-paid arrangements also exist. | You pay the premium directly. Price reflects age, health, amount, term and policy features. | Total cost over the likely term, premium type, affordability and the tax treatment of this precise arrangement. |
These are structural differences, not guarantees. A particular employer scheme can be unusually generous or restrictive, and an individual contract can have features not available through work. Obtain both sets of current policy documents before comparing.
Condition lists, definitions and exclusions
Why the headline condition count can mislead
Two policies can advertise a similar number of illnesses yet provide materially different protection. One may pay only for a severe form of a condition; another may offer partial payments for an earlier stage. Check the definition for the condition most relevant to your family history and occupation, not only the marketing summary.
The ABI guide provides model definitions for participating members but expressly leaves insurers’ policy choices and exclusions in place. Its standards apply to both group and individual critical illness cover, so the existence of the guide does not make the two contracts identical. Product documents should use clear language; the ABI guide says “Plain English” should always be used in product information to aid consumer understanding.
Pre-existing and associated conditions
Joining through payroll without a medical questionnaire does not mean every prior condition is insured. For example, one insurer’s group information identifies pre-existing and related-condition exclusions and directs members to its exclusions and technical guides for the full terms: group critical illness information. That is an example of one provider’s wording, not a universal rule.

Ask for the scheme’s exclusion schedule and determine whether a prior diagnosis, symptom, investigation or treatment can exclude the same condition or an associated condition. If you move from group to individual cover, the new insurer may apply a different exclusion after underwriting.
Partial payments and multiple claims
Some contracts pay a smaller amount for specified less-severe conditions, or allow a second claim after an initial payment. Others pay once and end. These features can be more valuable than a longer list of conditions if they match your circumstances. Confirm the amount, qualifying definition, waiting or survival period and whether the original benefit reduces or the policy terminates after payment.
How underwriting works in each route
Workplace underwriting and free-cover limits
Group schemes commonly set a free-cover limit: employees receive cover up to a stated amount without full individual medical underwriting, while a higher amount may require medical questions or a report. The limit, evidence and exclusions are scheme-specific. A scheme can also accept everyone on a pre-existing-condition exclusion basis.
One provider’s FAQ describes an arrangement in which employees do not answer health questions for acceptance, while pre-existing and associated conditions remain excluded: group critical illness FAQ. Do not infer from that example that every employer plan has the same process.
Individual medical underwriting
An individual application normally asks about diagnoses, symptoms, medication, investigations, family history and lifestyle. The insurer can accept standard terms, charge more, exclude a condition, offer a lower amount or decline the application. Answer accurately; incomplete medical information can create a claim dispute later. Ask how long disclosed information remains relevant and what happens if your health changes before the policy starts.

Questions to ask about either policy
- Is acceptance automatic, subject to a free-cover limit, or individually underwritten?
- Which prior conditions and associated conditions are excluded?
- Does increasing the benefit, changing occupation or adding a family member trigger new evidence?
- Are there waiting or survival periods, and when does cover begin?
- What evidence will the insurer require to assess a claim?
Benefit amount and family protection
Start with the financial job the lump sum must perform. List mortgage and other debts, essential household spending during recovery, likely care or accessibility costs, childcare, and any income shortfall not covered elsewhere. Decide whether the target is a one-off debt repayment, a reserve for several years of expenses, or a combination. Then compare that target with the scheme’s actual benefit, rather than assuming a salary multiple is sufficient.

A salary-linked benefit can rise after a pay increase but can also fall if your salary or eligibility changes. Individual cover lets you choose an amount that is not tied to one employer, but the premium may be higher and affordability must be maintained for the full term.
Check family eligibility separately. A scheme may allow spouse or partner and child cover with different limits, definitions or ages of eligibility. An individual policy may offer family options, but they can have separate underwriting, maximums and claim rules. Confirm whether a child’s payment is a percentage of the main benefit, a fixed amount or capped at a stated limit.
As an illustration of why product figures must be treated cautiously, one current provider page describes a basic tier with 13 conditions, an extra tier with 26 additional conditions and a maximum benefit of £500,000: provider group-cover details. Those are that provider’s specifications, not UK market averages or a promise that your employer’s scheme has the same tiers or limit.
What happens when you leave your employer?
Employment-linked cover can stop when you resign, are dismissed, retire, cease to meet the scheme’s eligibility rules or the employer changes provider. The decisive date is in the scheme rules. For one insurer’s arrangements, cover ends on the employee’s leaving date or at the end of that month, depending on the employer’s scheme: FAQ on group-cover end dates.

Do not assume that group critical illness cover is portable or that you have a guaranteed right to convert it to an individual policy. The available evidence does not establish a universal conversion right. Before your employment ends, ask HR and the insurer, in writing:
- The exact date and time cover stops, including treatment of notice periods, garden leave, redundancy, retirement and long-term absence.
- Whether continuation or conversion is available, and the application deadline.
- Whether new medical underwriting, exclusions or a new term apply.
- The premium, payment frequency, benefit amount and any change in family cover.
- Whether the option is with the same insurer or requires a new application.
MoneyHelper says people who had insurance through employment may be able to continue with the same providers and recommends asking what continuation costs. It also warns that an individual policy is likely to cost more than employer-associated cover. Treat that as a prompt to investigate your scheme, not as a guarantee that continuation exists: MoneyHelper’s employment-insurance guidance.
If you need uninterrupted protection, arrange any replacement policy before cancelling or allowing the workplace cover to lapse. A new application can take time, and a change in health during the gap may affect acceptance or terms.

Comparing premiums fairly
There is no reliable market-wide premium comparison in the available sources. Workplace cost may be fully employer-funded, shared, or employee-paid through a voluntary arrangement. Individual price depends on age, health, smoking status, benefit amount, term, occupation and selected options. Compare the recurring premium, not only the apparent payroll cost, and test whether you could maintain it during a career break or job change.
Recommended Free Tools
Ask whether premiums are guaranteed, reviewable or linked to age; when they are collected; what happens after a missed payment; and whether the employer can change the scheme at renewal. A low current deduction can conceal a benefit that ends with employment or a salary-linked amount that is below your target.

How HMRC describes critical-illness payments
HMRC’s Insurance Policyholder Taxation Manual says a critical illness policy pays a lump sum for specified diseases and that the payment is not taxable as income in the recipient’s hands as an annual payment: HMRC IPTM6010.
Employer contributions require a separate analysis. HMRC explains that, where another person contributes to premiums, the exemption can apply to a just and reasonable portion, while a non-qualifying portion may be taxed as employment income. HMRC also says providing an employer scheme is generally not a taxable benefit unless optional-remuneration arrangements apply: HMRC IPTM6120. The payroll treatment depends on the exact policy and contribution arrangement, so ask your employer how the benefit is reported. Do not apply rules for income-protection or sick-pay benefits to a critical-illness lump sum without checking the relevant contract and tax guidance.
A practical comparison process
- Collect the documents. Obtain the current group policy schedule, certificate, definitions, exclusions, claim guide, free-cover limit and scheme rules. For an individual policy, collect the quotation, application answers, underwriting decision, policy schedule and full terms.
- Write down the insured event. For every condition that matters to you, record the exact diagnostic or severity test, survival period, exclusions and any partial-payment amount.
- Check the amount. Compare the benefit with debts, essential expenditure, care, accessibility and family needs. Note whether it is fixed, salary-linked, reviewable or reduced after a claim.
- Check eligibility and underwriting. Record who is covered, free-cover limits, medical evidence, pre-existing-condition exclusions and rules for adding or increasing cover.
- Check family benefits. Confirm eligible relationships, child age limits, separate definitions, maximums and whether family cover ends when your employment ends.
- Map the policy timeline. Write the start date, renewal or review dates, expiry date, retirement rule and exact leaving-work end date. Add any continuation deadline to your calendar.
- Calculate the real cost. Include employee payroll deductions, any tax reported by the employer, individual premiums, expected increases and the cost of replacement cover if you change jobs.
- Decide whether a gap remains. If workplace cover is temporary, too small or excludes a relevant condition, consider an individual policy or other protection after taking regulated advice where appropriate.
Keep a copy of the comparison and revisit it after a pay change, mortgage change, marriage or separation, new child, diagnosis, job move or employer scheme change. A regulated adviser can help where underwriting, trusts, linked life cover or tax treatment is complex.

Workplace cover may fit when
- The benefit meets your immediate financial target and the definitions suit your main concerns.
- The employer pays the premium or the payroll cost is affordable.
- You understand the free-cover limit and exclusions and accept the employment-linked end date.
- You have separate arrangements for death and long-term inability to work.
An individual policy may be important when
- You expect to change jobs, become self-employed or retire before the financial need ends.
- The workplace benefit is below your target or varies with salary.
- You need a definition, amount, family option or continuation feature not offered by the scheme.
- You want ownership that is independent of an employer’s future insurer or eligibility rules.
Holding both can be rational
Group and individual policies do not automatically duplicate each other. A workplace benefit can provide inexpensive short-term protection, while an individual policy can cover a personal debt or family need beyond your current employment. Compare definitions and exclusions before assuming the amounts can simply be added; two policies may respond differently to the same diagnosis, and each may have its own claim limits.
Quick Recap
Common mistakes and how to avoid them
- Choosing by condition count. Read the definitions and severity tests; a longer list is not necessarily broader protection.
- Assuming payroll enrolment covers medical history. Check pre-existing and associated-condition exclusions and any free-cover limit.
- Treating the benefit as income protection. Critical illness pays for listed conditions, not every illness that prevents work.
- Assuming a death payment. Critical illness cover does not usually pay on death; assess life insurance separately if dependants need it.
- Assuming portability. Get the leaving-work date and any continuation terms in writing before you resign.
- Comparing a current deduction with a full-term premium. Include reviewable premiums, age-related increases and the cost of replacement cover.
- Ignoring family rules. Partner and child benefits can have different definitions, limits and end dates.
- Relying on a general tax statement. HMRC treatment depends on the policy and who pays the premium; confirm the payroll position.
Questions to put to HR, the insurer or an adviser
- What are the exact insured conditions, definitions, exclusions and survival periods?
- What is my benefit today, and can it change with salary, age, renewal or a scheme amendment?
- What is the free-cover limit, and what medical evidence is needed above it?
- Are pre-existing, associated or family-history conditions excluded?
- Are partial payments, second-event claims or reinstatement available?
- Who can be covered as a partner or child, and what limits apply?
- When exactly does cover end on resignation, redundancy, retirement, sickness absence or a transfer?
- Is there a continuation or conversion option, what is the deadline, and will new underwriting apply?
- What will I pay now and over the expected term, and how are premiums reviewed?
- How will employer contributions and any claim payment be treated for tax in my circumstances?
