Health insurance tax deductions and common exclusions explained

Health-insurance premiums can be tax-deductible, but the answer depends on your filing route, who paid the premiums, whether Marketplace assistance was involved, and which tax year you are filing. Most taxpayers use Schedule A only for unreimbursed medical costs exceeding 7.5% of adjusted gross income (AGI). A self-employed taxpayer with qualifying net profit may instead claim an above-the-line deduction, subject to earned-income and employer-coverage limits. Premiums paid or reimbursed with tax-free benefits generally cannot be deducted again.

Start with the two deduction routes

There are two fundamentally different ways health-insurance premiums may affect a federal income-tax return:

As an Amazon Associate I earn from qualifying purchases.

  • Itemized medical deduction: Report eligible, unreimbursed medical and dental expenses on Schedule A. Only the portion above 7.5% of AGI is deductible.
  • Self-employed health-insurance deduction: A qualifying self-employed person may claim an adjustment to income, generally through Schedule 1 and Form 7206, without itemizing. The deduction is limited by earned income from the relevant business and by months in which subsidized employer coverage was available.

You cannot claim the same premium through both routes. You also must remove amounts paid or reimbursed by an employer, an HSA, FSA, HRA, or a Premium Tax Credit before calculating any deduction.

Which route applies to your situation?

Situation Potential treatment Main limitation
You itemize deductions and paid premiums yourself Include qualifying premiums with other unreimbursed medical expenses on Schedule A Only expenses above 7.5% of AGI produce a deduction
You have net profit from self-employment Claim qualifying medical, dental, vision, and qualified long-term-care premiums as an adjustment to income Limited by earned income and unavailable for months when subsidized employer coverage was available
You received an advance Marketplace Premium Tax Credit Reconcile the credit on Form 8962 before finalizing a medical deduction Only the premium amount that remains your responsibility can be considered
Premiums were paid from an HSA, FSA, or HRA Do not deduct the reimbursed amount again HSA payment of ordinary insurance premiums is generally not a qualified distribution

The tax year matters. Use the Schedule A, Form 7206, Form 8962, Form 8889, and other instructions that apply to the return year you are filing; form lines and eligibility rules can change.

How the Schedule A medical-expense deduction works

What counts as a medical expense

IRS Publication 502 describes medical expenses as costs for the diagnosis, cure, mitigation, treatment, or prevention of disease, together with costs affecting a body function. Qualifying health, dental, and vision insurance premiums can be part of that category when you paid them yourself and were not reimbursed.

Apply the 7.5% AGI floor

Schedule A allows only the amount of total unreimbursed medical and dental expenses that exceeds 7.5% of AGI. The calculation is:

Deductible medical expenses = qualifying unreimbursed expenses − (0.075 × AGI)

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

For example, in a purely illustrative calculation, a taxpayer with $80,000 of AGI has a 7.5% floor of $6,000. If qualifying unreimbursed medical expenses total $10,000, the amount above the floor is $4,000. The taxpayer must still itemize and satisfy all other Schedule A requirements for that amount to matter.

Subtract every reimbursement or subsidy first

Reduce medical expenses by reimbursements from insurance, an employer, an HSA, FSA, or HRA. Premiums paid with tax-free employer benefits are not your deductible expense. A Premium Tax Credit also reduces the amount of premium treated as paid by you. Do not deduct a reimbursement merely because it arrived after you paid the bill; account for the reimbursement when determining the amount that is ultimately yours.

Complete Marketplace reconciliation before Schedule A

If you received advance Premium Tax Credit payments, or believe you qualify for the credit, complete Form 8962 before settling the Schedule A calculation. Form 1095-A supplies the Marketplace coverage and advance-credit information used in that reconciliation. Schedule A instructions also require premiums to be reduced by any self-employed health-insurance deduction, preventing the same dollars from producing two benefits.

Self-employed health-insurance deduction

Who may claim it

A self-employed taxpayer with net profit from a trade or business may generally claim an above-the-line deduction for qualifying medical, dental, and vision premiums, plus qualifying long-term-care premiums. Eligible coverage can be for:

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • the taxpayer;
  • the taxpayer’s spouse;
  • dependents; and
  • a child under age 27, even if that child is not the taxpayer’s dependent.

The plan must be established under the trade or business, and the deduction cannot exceed earned income from that business. The deduction is generally handled as an adjustment to income on Schedule 1, with Form 7206 used for the calculation for applicable tax years.

Check employer-plan eligibility month by month

The deduction is unavailable for months in which the taxpayer could participate in a subsidized health plan maintained by an employer of the taxpayer, spouse, dependent, or child under age 27. The issue is eligibility for subsidized employer coverage, not simply whether the taxpayer chose to enroll. Review coverage availability for each month rather than applying a single annual assumption.

Coordinate with Schedule A

If the self-employed deduction is limited by earned income, a remaining premium amount may potentially be considered with other medical expenses on Schedule A, subject to the 7.5% AGI floor and the other Schedule A rules. A nondependent child under age 27 can qualify for the self-employed coverage rule, but premiums attributable to that child may not be claimable on Schedule A when the self-employed deduction is limited. Keep the allocation clear so the same premium is not used twice.

Marketplace coverage and the Premium Tax Credit

What the credit changes

The Premium Tax Credit is a refundable credit intended to help eligible people and families with low or moderate income afford Marketplace insurance. When the Marketplace paid an advance credit to your insurer, you did not bear that portion of the premium. Only the amount that remains your responsibility can potentially enter a deduction calculation.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Reconcile advance payments on Form 8962

Use Form 1095-A to reconcile advance credit payments on Form 8962. The final credit can be larger or smaller than the advance amount, changing the net premium attributable to you. Complete that reconciliation before determining either a Schedule A medical deduction or the self-employed deduction.

Self-employed Marketplace enrollees

Self-employed taxpayers with Marketplace coverage may need the specialized worksheets in IRS Publication 974. Those worksheets address the interaction between the self-employed deduction and the Premium Tax Credit and distinguish:

  • Specified premiums: premiums for qualified Marketplace plans.
  • Nonspecified premiums: amounts involving coverage outside a qualified plan, people outside the coverage family, dental coverage, qualified long-term-care coverage, and certain Medicare premiums.

Because the credit and the deduction affect each other, do not simply use the gross amount shown on a bill. Follow the applicable Publication 974 worksheet for the tax year and retain the Form 1095-A used in the calculation.

Can an HSA pay health-insurance premiums?

Ordinary premiums are generally not qualified HSA expenses

Form 8889 instructions state that insurance premiums cannot be treated as qualified medical expenses except for specific categories. Therefore, using HSA funds for ordinary health, dental, or vision premiums generally creates a nonqualified distribution. HSA distributions used exclusively for qualified medical expenses are excluded from gross income; a nonqualified distribution is generally taxable and may also incur an additional 20% tax.

The limited premium exceptions

HSA funds may be used for premiums in these circumstances:

  • qualified long-term-care insurance;
  • COBRA or similar continuation coverage;
  • coverage while receiving unemployment compensation; and
  • Medicare coverage when the HSA beneficiary is age 65 or older.

Medigap premiums are not included in the Medicare exception. The HSA rule is separate from whether a premium might qualify for a Schedule A or self-employed deduction; payment from an HSA still prevents a second tax benefit for the same amount.

FSA and HRA reimbursements work the same way for double-deduction purposes

If an FSA or HRA reimbursed a premium or other medical cost on a tax-free basis, exclude that amount from any deduction. A cost is not both tax-free reimbursed and deductible.

Common health-insurance costs that are excluded or limited

  • Life-insurance premiums: generally not medical expenses for these deductions.
  • Loss-of-earnings policies: premiums for policies replacing income are excluded.
  • Fixed weekly hospitalization benefits: premiums for policies paying a fixed weekly amount are not treated like ordinary medical insurance premiums.
  • Coverage for nondependents: premiums attributable to people who do not meet the applicable coverage rules cannot automatically be included.
  • Tax-free retirement-plan-funded premiums: premiums paid with tax-free retirement-plan distributions or similar tax-advantaged funds cannot be deducted again.
  • Premium Tax Credit amounts: the portion paid through the credit is not your deductible premium.
  • Employer-paid or reimbursed premiums: amounts your employer paid or reimbursed tax-free are not your unreimbursed expense.

When a policy combines medical coverage with excluded benefits, use the insurer’s premium allocation or other reliable records rather than assuming the entire payment qualifies.

2026 HSA-compatibility change for bronze and catastrophic plans

The IRS says bronze and catastrophic plans are treated as HSA-compatible beginning January 1, 2026. That change affects whether a plan can meet the insurance requirement for HSA eligibility; it does not make ordinary insurance premiums qualified HSA expenses. Other HSA eligibility requirements still apply, including the rules governing disqualifying coverage and contributions. Verify the plan’s status and the tax year’s contribution guidance before contributing.

Filing workflow and records to keep

  1. Identify the tax year. Gather the instructions and form versions for that year before entering figures.
  2. List everyone covered. Separate the taxpayer, spouse, dependents, and any child under 27 covered under the self-employed rule.
  3. Trace the payment source. Mark each premium as personally paid, employer paid, HSA/FSA/HRA reimbursed, or covered by a Marketplace credit.
  4. Determine the route. Use Schedule A if you itemize and have unreimbursed expenses above the AGI floor; evaluate the self-employed adjustment if you have qualifying net profit.
  5. Check employer eligibility by month. For the self-employed calculation, document months when subsidized employer coverage was available to the taxpayer or specified family members.
  6. Reconcile Marketplace assistance. Use Form 1095-A and Form 8962; consult Publication 974 worksheets when self-employed Marketplace coverage is involved.
  7. Calculate without overlap. Remove reimbursements, credits, and amounts used for the self-employed deduction before computing Schedule A expenses.
  8. Retain support. Keep premium statements, Form 1095-A, employer-coverage information, reimbursement records, proof of payment, and HSA distribution records.

Common filing mistakes and how to correct them

Deducting the full premium shown on a statement

A statement may show the gross premium before an advance credit, employer contribution, or reimbursement. Recalculate the amount you actually bore and use that net figure.

Using HSA money for ordinary premiums

Check the Form 8889 exception list before treating an HSA distribution as tax-free. If the premium does not fit an exception, the distribution is generally taxable and may face the additional 20% tax.

Ignoring a spouse’s or child’s employer coverage

For the self-employed deduction, subsidized employer-plan eligibility can come through an employer of the taxpayer, spouse, dependent, or child under 27. Review eligibility rather than enrollment for each month.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Claiming the same dollars twice

A premium used for the self-employed adjustment cannot also be included in Schedule A. The same prohibition applies to any amount reimbursed tax-free or paid by a Premium Tax Credit.

Skipping Form 8962

Marketplace advance-credit information must be reconciled before the final premium amount is known. Complete Form 8962 from Form 1095-A and then carry the corrected figures into the applicable deduction calculation.

When professional help is worthwhile

Consider tax-professional assistance when you combine self-employment income, Marketplace coverage, multiple household policies, a limited self-employed deduction, or HSA distributions that may be nonqualified. Ask the preparer to confirm support for the tax year you are filing and to review Form 7206, Schedule A, Form 8962, Publication 974 worksheets, and Form 8889 as applicable.
