Employer health insurance comes through a job, while personal health insurance is bought by an individual or household directly or through a state or federal Marketplace. Neither route is automatically better or cheaper. The plan’s premium, deductible, provider network, prescription formulary, out-of-pocket limit, dependent pricing and your eligibility for Marketplace savings determine the practical value. An affordable, minimum-value employer offer can also make you ineligible for Marketplace premium tax credits, even if you decline the job plan.
This U.S.-specific guide explains what each route generally covers, how to compare actual plans, what happens when employment ends and which exceptions can change the answer.

What employer and personal health insurance actually mean
Employer health insurance
Employer health insurance is a group plan offered through your workplace. The employer may pay part of the premium, leaving you responsible for an employee contribution. You must check the amount for employee-only coverage separately from the amount for a spouse, children or other dependents. An employer plan can use a narrow or broad provider network, require referrals, impose different deductibles for individuals and families, and apply its own prescription rules.

Two employers can offer plans with very different premiums, cost sharing, hospitals, doctors and drug formularies. The employer’s Summary of Benefits and Coverage (SBC), enrollment materials and provider and drug directories are the controlling documents.

Personal or individual-market health insurance
Personal health insurance is purchased by an individual or household, either from an insurer or through a state or federal Health Insurance Marketplace. A Marketplace application uses household composition and projected income to determine whether the applicant may qualify for a premium tax credit or other savings. In some states, the federal application directs the person to a state-run Marketplace.
Marketplace-certified qualified health plans cover essential health benefits, follow Affordable Care Act requirements and have established cost-sharing limits. Those standards do not make every plan identical: premiums, deductibles, copayments, coinsurance, networks, formularies and covered details still vary by plan and location.
What each type of plan covers
Essential health benefits on Marketplace plans
HealthCare.gov identifies ten essential-health-benefit categories for Marketplace qualified health plans. They include:
Recommended Free Tools
- Ambulatory patient services, such as visits that do not require hospital admission
- Emergency services
- Hospitalization, including surgery and overnight stays
- Maternity and newborn care
- Mental-health and substance-use-disorder services
- Prescription drugs
- Rehabilitative and habilitative services and devices
- Laboratory services
- Preventive and wellness services and chronic-disease management
- Pediatric services, including oral and vision care for children
A plan may cover additional services. The category names do not tell you whether a particular therapy, facility, medication or device is covered, whether prior authorization is required, or what you will pay. Read the plan’s SBC, evidence of coverage and formulary for those details.
Benefits and protections in job-based plans
Employer plans can include many of the same medical categories, but the employer’s selected design controls the details. Review whether your preferred doctors and hospitals are in-network, how the plan treats out-of-network care, which drugs are on the formulary, and whether services such as fertility treatment, adult dental care, hearing aids or specific therapies are included.
HealthCare.gov describes protections that can apply under federal rules, including coverage for pre-existing conditions, preventive care protections, no lifetime or yearly dollar limits on essential health benefits, and appeal rights for certain coverage decisions. Applicability depends on the plan and the governing rules; some protections do not apply in the same way to grandfathered plans. Treat these protections as a starting point, not a substitute for reading your plan documents.
Minimum value: what an employer offer does and does not prove
“Minimum value” is a defined test used in the Marketplace subsidy rules. HealthCare.gov says a plan meets the standard when it is designed to pay at least 60% of the total cost of medical services for a standard population and includes substantial physician and inpatient hospital coverage. That 60% is not a promise that the plan will pay 60% of your own bills. Your actual share depends on the deductible, copayments, coinsurance, services used and provider network.

Passing minimum value also does not mean an employer plan is generous, has your preferred doctors, or covers every treatment you need. It only addresses a threshold relevant to premium-tax-credit eligibility. Ask the employer to complete its coverage or employer-offer information and keep the written response.
Premiums and out-of-pocket costs: compare the whole year
A low payroll deduction can hide a high deductible or expensive dependent coverage. Compare the household’s likely annual spending rather than premiums alone.
| Cost or feature | Employer plan | Personal/Marketplace plan |
|---|---|---|
| Premium paid by household | Employee contribution for each coverage tier; employer may pay the rest. Dependent premiums can be substantially different from employee-only premiums. | Full unsubsidized premium unless the household qualifies for a premium tax credit or other savings. |
| Deductible | Plan-specific; may have separate individual and family amounts. | Plan-specific; metal category does not eliminate the need to check the actual deductible. |
| Copayments and coinsurance | Plan-specific and often different for primary care, specialists, hospital care and prescriptions. | Plan-specific; the SBC lists the charge for each service. |
| Out-of-pocket maximum | Limits covered in-network spending under the plan’s rules; verify which services and providers count. | Marketplace plans have established annual cost-sharing limits, but the exact limit is plan- and year-specific. |
| Provider access | Employer selects the network; local hospitals and doctors may differ from other employer options. | Choose among plans available in your state and rating area; networks can be narrow or broad. |
| Prescription coverage | Employer plan’s formulary, tiers and prior-authorization rules. | Selected plan’s formulary, tiers, pharmacy network and utilization rules. |
| Financial assistance | Employer contribution lowers the payroll deduction but is not a Marketplace tax credit. | Marketplace application determines potential premium tax credits and other savings based on household facts and any employer offer. |
Illustrative metal-level cost shares
HealthCare.gov’s SHOP guidance gives illustrative average cost shares: Bronze plans are shown as 60% paid by the plan and 40% by the employee, Silver as 70% and 30%, Gold as 80% and 20%, and Platinum as 90% and 10%. The page cautions that employees’ actual costs vary. These are category illustrations, not a prediction for your employer plan or a personalized estimate.
Estimate two spending scenarios
For each plan, calculate at least two scenarios:
- Low-use year: annual premiums plus routine visits and prescriptions.
- High-use year: annual premiums plus the deductible and expected coinsurance or copayments, capped by the applicable out-of-pocket maximum for covered in-network care.
Keep separate totals for employee-only and family coverage. A plan with the lowest premium may cost more in a year involving surgery, pregnancy, hospitalization or costly specialty drugs.

You can generally apply for Marketplace coverage, but an employer offer can affect whether you qualify for premium tax credits. HealthCare.gov says an affordable job-based offer that meets minimum value generally makes the employee ineligible for Marketplace premium tax credits. This can remain true even when the employee declines the job plan.
Do not assume that every job offer blocks all Marketplace help. The result depends on the offer’s affordability and minimum-value status, household income, household composition and the rules in effect for the relevant plan year. Dependents can have a different eligibility result from the employee, so evaluate each household member.
- Ask the employer for the formal offer details, including the employee contribution for the relevant coverage tier and whether the plan meets minimum value.
- Complete the Marketplace application with the entire tax household and expected annual income.
- Report the employer offer accurately, even if you intend to decline it.
- Review the eligibility result before relying on a displayed premium or tax credit.
Never cancel job-based coverage or miss an enrollment deadline based only on an estimate. Keep the written eligibility notice and compare the net premium with the actual plan benefits.

How to compare an employer plan with personal coverage
1. Collect the right documents
- Employer SBC for every plan option and coverage tier.
- Employer contribution and payroll-deduction schedule.
- Summary plan description or evidence of coverage.
- Current provider directory and prescription formulary.
- Marketplace plan documents, premiums and eligibility results for your address.
2. List the care your household is likely to use
Write down ongoing medications, specialists, preferred hospitals, planned procedures, pregnancy or adoption plans, behavioral-health care, durable medical equipment and recurring laboratory services. Include each dependent’s needs; a family plan that works for one person may be unsuitable for another.
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
3. Verify network access
Search the insurer’s directory for each doctor, hospital and facility by name and location. Then call the provider’s office to confirm the exact network and plan name. Directories can lag behind contract changes, and an office accepting an insurer does not necessarily mean it accepts every product from that insurer.
4. Check drugs and utilization rules
Search the formulary for every prescription, including dosage and formulation. Note the tier, copayment or coinsurance, specialty-pharmacy requirement, quantity limit, step-therapy rule and prior-authorization requirement. Ask the prescriber whether an equivalent covered option exists before enrolling.
5. Compare financial exposure
Record the annual household premium, deductible, copayments, coinsurance and out-of-pocket maximum. Confirm whether the maximum excludes premiums, out-of-network bills, non-covered services or balance billing. Use the low-use and high-use scenarios above, and identify which costs apply before the deductible.
6. Test subsidy eligibility
Enter the employer offer and household information in the Marketplace application. A displayed tax credit is conditional on the information supplied and the applicable rules. Reconcile advance credits as required when filing federal taxes, and update the Marketplace if income or household coverage changes.
7. Check timing and effective dates
Record the employer enrollment deadline, waiting period, Marketplace enrollment window and intended effective date. A plan that starts later can leave a coverage gap even if it is cheaper. Confirm whether claims incurred before the effective date are excluded.
8. Make a household decision
The employee, spouse and children do not always need the same route. Compare an employer plan for the employee with Marketplace options for dependents when permitted and financially sensible. Verify how choosing separate coverage affects eligibility, tax credits and the employer’s contribution.

Is employer health insurance cheaper?
There is no universal answer. An employer contribution can make job-based coverage less expensive than the full Marketplace premium, but a household may qualify for Marketplace savings, and dependent premiums can change the result. A job plan with a low payroll deduction can also carry a high deductible, narrow network or costly drug tiers.
Compare the net annual premium and the likely cost of care under each plan. Do not compare an employer employee-only premium with a Marketplace family premium, or a subsidized Marketplace premium with an unsubsidized employer figure. Use the same people, coverage period and care assumptions in both calculations.

Marketplace special enrollment
Loss of job-based coverage can trigger a Marketplace special enrollment period. HealthCare.gov says a person can apply within 60 days of losing job-based coverage for the specified special enrollment period. The exact qualifying event, deadline and effective date depend on the circumstances, so read the employer’s termination notice and confirm current Marketplace instructions promptly.
If a new job has a waiting period, Marketplace coverage may bridge the gap. Apply before the old coverage ends when possible, and verify the start date in writing.
COBRA continuation
COBRA is a separate route that generally lets an eligible person temporarily continue the former employer’s plan, usually for 18 months. The person generally pays the full premium, including the amount the employer previously contributed, plus a small administrative fee. COBRA may preserve access to current doctors and deductibles, but its price can be much higher than the payroll deduction you had while employed.
Compare the COBRA election deadline, premium, remaining deductible and out-of-pocket progress with Marketplace alternatives. Do not assume that electing COBRA and later switching to a Marketplace plan creates another enrollment opportunity; confirm the timing rules for your event.
Individual coverage HRAs: a separate branch
An individual coverage Health Reimbursement Arrangement (ICHRA) is employer-funded reimbursement for eligible medical expenses, including premiums. To receive the reimbursement for a covered month, eligible employees and dependents generally must have individual insurance or specified Medicare coverage for that month.

ICHRA interaction with Marketplace tax credits is conditional. HealthCare.gov explains that eligibility depends on whether the employer’s HRA offer meets affordability standards and whether the person opts out. Do not treat an ICHRA like an ordinary employer group plan or assume the reimbursement can be combined with a premium tax credit. Read the employer’s ICHRA notice, including the amount, covered expenses, opt-out procedure and required proof of individual coverage, before enrolling.

Fix: Add deductible, cost sharing, drug costs and the out-of-pocket maximum, then model a high-use year.
Assuming “minimum value” means comprehensive coverage

A practical decision checklist
- Confirm the plan year and coverage effective date.
- Write down the employee and dependent premiums after any employer contribution.
- Record individual and family deductibles, copayments, coinsurance and out-of-pocket maximums.
- Verify every important doctor, hospital and facility in-network.
- Check every regular medication on the formulary and note restrictions.
- Ask whether referrals, prior authorization or step therapy apply to expected care.
- Submit the complete employer offer and household information to the Marketplace.
- Compare low-use and high-use annual totals, not premiums alone.
- For a job change, compare Marketplace coverage and COBRA before the old plan ends.
- For an ICHRA, read the reimbursement and opt-out rules before selecting individual coverage.
The bottom line
Employer and personal health insurance can both provide broad medical protection, but the coverage route does not determine the quality or cost of the plan. The decisive evidence is in the plan documents: premiums for every household member, cost sharing, provider and hospital access, prescription rules, subsidy eligibility and timing. An employer offer that is affordable and meets minimum value can remove Marketplace premium-tax-credit eligibility, while loss of job coverage opens distinct Marketplace and COBRA choices. Compare the actual plans for your state, household and plan year before enrolling or declining coverage.
