If you are enrolled in premium-bearing Medicare coverage but are not receiving Social Security or Railroad Retirement Board (RRB) payments, Medicare sends you a bill instead of deducting the premium from a benefit. Medicare states: “If you don’t get benefits from Social Security (or the Railroad Retirement Board), you’ll get a premium bill from Medicare.”
The key distinctions are the date your Social Security retirement benefit starts, your Medicare enrollment obligations, and the way Medicare collects premiums. Delaying the benefit changes the collection route when there is no payment from which to withhold; it does not by itself change whether you must enroll in Medicare or what your premium amount is.

What changes when you delay Social Security
Social Security withholding is only possible when a Social Security or RRB benefit is being paid. Without that payment, Medicare uses direct billing. You receive a CMS-500 premium bill for the Medicare charges that apply to you, then submit payment yourself or authorize an automatic debit.
This is an administrative change, not a premium waiver. A person can delay Social Security and still owe Part B premiums, a premium for Part A when applicable, and income-related monthly adjustment amounts (IRMAA) for Part B or Part D. Starting Social Security later does not, on its own, raise or lower those charges.

Medicare enrollment is a separate decision
Direct billing answers how a premium is collected. It does not answer whether you should enroll in Medicare, when enrollment is required, or whether delaying a particular coverage is appropriate. Those questions depend on your circumstances and Medicare’s enrollment rules. Treat a premium bill as evidence that Medicare has coverage and a payment obligation on record, not as advice to enroll or delay.
What the CMS-500 Medicare bill covers
Medicare uses the CMS-500 for people who pay Medicare directly for Part A premiums, Part B premiums, and/or Part D IRMAA. A first bill or a bill issued after a premium change can include more than one month, so the amount may be larger than the recurring charge you expected.
Billing frequency depends on the charge
| Charge on the bill | Typical billing interval | What to watch for |
|---|---|---|
| Part B premium, including Part B IRMAA when owed | Generally every three months | The bill can include prior months or a changed amount. |
| Premium for Part A | Monthly | Only applies when a Part A premium is owed. |
| Part D IRMAA | Monthly | This is an income-related charge separate from the drug plan’s own premium. |
The due date printed on your CMS-500 controls. Medicare’s general guidance says the due date is the 25th of the month. Medicare recommends sending a payment at least five business days before that date so it has time to arrive and be credited.

Four ways to pay when no benefit is available for withholding
1. Use your secure Medicare account
Sign in to your secure Medicare account and choose “Pay my premium.” The payment flow takes you to the Treasury Department’s secure Pay.gov service. Medicare describes this as the fastest option. Use the amount and due date on your current bill rather than relying on an old statement, especially after an annual premium change or an IRMAA reassessment.
2. Enroll in Medicare Easy Pay
Medicare Easy Pay is a free recurring debit from a checking or savings account. The debit amount updates automatically when your Medicare premium changes, which avoids having to revise a standing payment instruction each time Medicare changes the bill.
Debits generally occur on the 20th of the month or the next business day. Enrollment can take six to eight weeks. Continue paying by another method until you see that Easy Pay has started; otherwise, an account that is still waiting for setup can become past due.

Your bank may offer a one-time or recurring online bill-payment service. Enter the Medicare payment information exactly as shown on the CMS-500 and schedule it early enough for the bank and Medicare to process it. Some banks charge a fee. A manually scheduled amount may not update automatically when Medicare changes your premium, so review every new bill and change the payment instruction when necessary.

4. Mail a payment
Mail the payment using the coupon attached to the CMS-500 and the payment forms Medicare currently accepts. Follow the instructions printed on your bill, because accepted methods and mailing details are tied to the statement. If the coupon is missing, processing can be slower; request or print the current instructions rather than sending an old coupon. Mail early enough to account for delivery and processing time.
Comparing the payment routes
| Route | Recurring or manual | How premium changes are handled | Timing considerations |
|---|---|---|---|
| Secure Medicare account and Pay.gov | Manual payment initiated from the account | Check the current CMS-500 and enter the amount due. | Medicare calls it the fastest option; submit before the printed due date. |
| Medicare Easy Pay | Recurring automatic debit | Medicare updates the debit when the premium changes. | Usually debits on the 20th or next business day; setup takes six to eight weeks. |
| Bank bill pay | One-time or recurring, depending on the bank | Manual payment amounts may not update automatically. | Allow the bank’s processing time; some banks may charge a fee. |
| Manual payment with the bill’s coupon | Use the amount on each current bill. | Mail at least five business days before the due date, and allow for delivery; a missing coupon can slow processing. |
How the premium amount is determined
The 2026 standard Part B premium
The standard Part B premium is $202.90 per month in 2026, according to the Centers for Medicare & Medicaid Services. That figure is a baseline, not a promise that every enrollee’s CMS-500 will show $202.90. Individual bills can be higher because of IRMAA, a late-enrollment penalty, or another premium amount that applies to the person’s coverage. Premiums can also change in future years.
Delaying Social Security does not create a special Part B rate. Someone who starts Social Security at 62, someone who claims later, and someone who has not claimed yet can face the same Medicare premium rules for the same coverage and income circumstances.

IRMAA is tied to income, not to the claiming date
IRMAA is an income-based charge associated with Part B and/or Part D. The Social Security Administration generally uses your tax-return information from two years before the premium year, along with your tax filing status, to make the determination. SSA procedures allow other information in specified circumstances.

Because IRMAA is calculated from income and filing information, it can apply even when you have postponed Social Security retirement payments. The absence of a benefit payment changes where you pay the charge; it does not remove an income-based adjustment.
Requesting a lower IRMAA after a major income change
A qualifying life-changing event that reduces household income may support a request to lower IRMAA. SSA lists events including marriage, divorce, the death of a spouse, loss of income, and an employer settlement payment. Form SSA-44 is one route for making the request. Gather the event documentation and the updated income information SSA requests. Until SSA changes the determination and Medicare issues a corrected amount, follow the current bill’s payment instructions.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.A practical setup plan for delayed Social Security
- Confirm the coverage and line items. Read the CMS-500 to see whether it contains Part B, a Part A premium, Part B IRMAA, Part D IRMAA, prior months, or a changed amount.
- Mark the printed due date. Although the general Medicare guidance uses the 25th, the date on your own bill is the controlling deadline.
- Choose a payment route. Use Pay.gov for a quick manual payment, Easy Pay for an automated recurring debit, your bank for one-time or recurring bill pay, or mail with the coupon.
- Cover the first Easy Pay cycle another way. If you enroll in Easy Pay, allow six to eight weeks for setup and keep making payments until an automatic debit is visible.
- Reconcile each statement. Compare the new bill with the prior one, particularly after an annual premium announcement, an IRMAA notice, or a change in coverage.
- Keep proof. Save online confirmation numbers, bank records, mailed-payment evidence, and the statements that show the payment was credited.
Common situations
You delay Social Security and are already paying Part B
Medicare sends the CMS-500 directly because there is no Social Security payment from which to deduct the premium. The bill is commonly issued every three months for Part B-only charges, including Part B IRMAA if applicable. Pay each statement by its printed due date.
You owe a Part A premium
A premium-bearing Part A charge is generally billed monthly. If Part B is also on the account, the CMS-500 may contain different line items with different billing patterns. Check the statement rather than assuming every Medicare charge is quarterly.

You have Part D IRMAA
Part D IRMAA is billed monthly. It is separate from the drug plan’s own premium and is based on the SSA income determination. Delaying Social Security does not change that calculation or the monthly billing route.
You receive RRB payments instead of Social Security
The same distinction applies: if you receive RRB benefits, Medicare may have a benefit payment from which to withhold premiums. If you receive neither Social Security nor RRB payments, expect direct Medicare billing.
Your first bill is unexpectedly high
Look for prior months or a recently changed premium. A first statement can combine several months, and an IRMAA or penalty adjustment can raise the amount above the standard Part B premium. Compare the line items with your Medicare and SSA notices before choosing a payment amount.
Quick Recap
What to remember
- Delaying Social Security does not make Medicare premiums disappear; it generally means Medicare bills you directly when no Social Security or RRB payment is available for withholding.
- Part B-only bills are generally quarterly, while Part A premiums and Part D IRMAA are billed monthly.
- You can pay through a secure Medicare account, free Medicare Easy Pay, bank bill pay, or mail.
- Easy Pay updates automatically but may take six to eight weeks to begin, so keep paying another way during setup.
- IRMAA and the 2026 standard Part B amount of $202.90 are separate from the decision to claim Social Security later.
