In an Indian health-insurance policy, a deductible is a threshold you absorb before the policy begins paying, a co-payment is your specified share of an admissible claim, and a sub-limit is a ceiling on what the insurer will pay for a defined treatment, service, condition or situation. In shorthand: deductible = threshold; co-payment = share; sub-limit = category cap.
These are India-focused explanations based on Insurance Regulatory and Development Authority of India (IRDAI) guidance. Terminology and contract design differ by country. For a binding answer, use your policy’s Customer Information Sheet (CIS), schedule and full wording; the exact admissibility rules and claim clauses determine what is payable.

The three clauses answer different questions
When a hospital bill arrives, these provisions are often confused because all can leave part of the cost with the insured. They operate on different parts of the contract:
- Deductible: How much must be absorbed before benefits become payable?
- Co-payment: What specified amount or percentage of the admissible claim remains with the insured?
- Sub-limit: What is the maximum the insurer will pay for a particular category of expense?
A deductible and a co-payment are not interchangeable, and neither is the same as a sub-limit. A policy can contain one, two or all three. If several clauses apply to one claim, do not assume a standard calculation sequence; the policy’s definitions, admissibility provisions and settlement clauses control.

What is a deductible?
IRDAI describes a deductible as “a specified amount stated in the policy up to which an insurer will not pay any claim, and which will be deducted from total claim amount (if claim amount is more than the specified amount).” Practically, it is the amount you must bear before the insurer’s benefit becomes payable under the applicable cover.
How the threshold works
The contract should identify the rupee amount and the events to which it applies. A deductible may be expressed per claim, for a policy year, in aggregate, or for a particular cover. Those are materially different designs. A per-claim deductible can apply again when a separate claim is made; an annual aggregate deductible may be exhausted by earlier claims. Do not infer the frequency from the number alone.
Check whether the deductible applies to every insured person, only a named section of the policy, or a particular treatment or hospital-cash benefit. The schedule may also state exceptions or separate deductibles for optional covers. The wording for a hospital-cash policy can use a time-based deductible, meaning a specified waiting period before the daily cash benefit starts, rather than a rupee amount taken from a medical bill.
What a deductible does to the sum insured
IRDAI’s health-insurance FAQ says a deductible applies before benefits are payable and does not reduce the sum insured. That means the policy’s stated coverage limit is not automatically rewritten merely because a deductible exists. Whether a particular claim can be admitted, and how much of the remaining limit is available afterward, still depends on the policy terms.
Questions to ask about a deductible
- Is it a fixed rupee amount or time-based condition?
- Does it apply per claim, per admission, per person, per policy year or in aggregate?
- Which coverage section, treatment or insured person triggers it?
- Does it apply before or after any other contract adjustment specified in the wording?
- Are there stated exceptions, and are they shown consistently in the CIS, schedule and full wording?
What is co-payment?
IRDAI defines co-payment as “a specified amount / percentage of the admissible claim amount to be paid by policyholder / insured.” It is therefore a cost-sharing clause applied to the amount the insurer has first determined to be admissible under the policy.

Fixed amount or percentage
A policy can require a fixed co-payment or a percentage. A percentage is not automatically calculated on the hospital’s gross invoice: the definition refers to the admissible claim amount. Non-covered items, disallowed charges and amounts removed under other policy rules may not form part of that base. The wording should state the calculation base and the situations in which the co-payment applies.
Look for geographic, treatment, hospital, age-band or coverage-specific triggers. A co-payment might apply only to a particular option or to claims in a defined circumstance. A mandatory co-payment and an optional co-payment selected in exchange for a different premium have different implications for affordability and should not be compared as though they were identical.

What a co-payment does to the sum insured
IRDAI’s FAQ states that a co-payment does not reduce the sum insured. It does, however, reduce the amount the insurer pays on an admissible claim because the insured must fund the specified share. The policy can still contain separate limits, exclusions or restoration rules that affect later claims.
Questions to ask about co-payment
- Is the share a percentage, a fixed rupee amount or a combination?
- Is it calculated on the admissible amount, a defined sub-category or another contractual base?
- Does it apply to every claim or only to listed treatments, hospitals, locations or ages?
- Is it mandatory, optional or triggered by a policyholder-selected variant?
- Does the wording explain how it interacts with deductibles, sub-limits and room-category provisions?
What is a sub-limit?
IRDAI says: “Normally sub-limit is a pre-defined limit for specific conditions, treatments, services, or situations. The insurer will not pay any amount in excess of this limit.” A sub-limit is therefore a category cap inside the broader policy. The overall sum insured may be much higher, but the specified category cannot draw on it beyond the sub-limit.
Categories and units matter
The schedule may set a cap for a treatment, a service, a condition, a room category or another defined situation. The unit can be per day, per event, per admission, per policy year or another period. A cap stated as a daily amount is not equivalent to an annual cap, so the unit and frequency must be read together.

Sub-limits can also interact with room eligibility or other claim clauses. If a room-category restriction changes the proportion of associated expenses that are admissible, the resulting calculation may be different from simply subtracting one line item. Only the full wording can establish that interaction.

What a sub-limit does to the sum insured
A sub-limit does not necessarily reduce the headline sum insured for every purpose. It restricts the insurer’s payment for the named category. Money left in the broader sum insured cannot be assumed to cover that category above its cap unless the policy expressly allows it.
Questions to ask about a sub-limit
- Exactly which condition, treatment, service or situation is capped?
- Is the cap per day, per event, per admission, per year or aggregate?
- Is it a fixed rupee amount, a percentage of the sum insured or another formula?
- Does a room limit or another clause change the admissible amount?
- What happens when the expense exceeds the cap, and are related expenses treated separately?
Side-by-side comparison
| Term | Primary function | Amount the insured bears | Effect described in IRDAI guidance | Contract details to verify |
|---|---|---|---|---|
| Deductible | Threshold before benefits become payable | The specified amount or time-based threshold, as applicable | IRDAI’s FAQ says it does not reduce the sum insured | Amount or waiting time; per claim or aggregate; policy year; cover; exceptions |
| Co-payment | Cost sharing on an admissible claim | The specified fixed amount or percentage of the defined claim base | IRDAI says it does not reduce the sum insured | Percentage or rupee amount; admissible-claim base; triggers; mandatory or optional status |
| Sub-limit | Maximum payment for a specified category | Any admissible expense above the category cap, plus other non-payable amounts | The insurer will not pay above the stated limit | Category; cap; unit and frequency; room-limit interaction; related clauses |
How each clause can affect a claim
Start with admissibility, not the hospital’s total
The hospital invoice is not automatically the claim amount. The insurer first applies the policy’s coverage conditions, exclusions, waiting periods and admissibility rules. Only then can a co-payment percentage be applied to the contractual base, and only the wording can show how a deductible or sub-limit fits into that process.
Illustrative co-payment arithmetic
Illustration only: if the policy determines that ₹100,000 is admissible and requires a 10% co-payment on that amount, the co-payment share would be ₹10,000. This is arithmetic, not an IRDAI statistic or a promise that a particular claim will settle that way. A different definition of admissible expenses, a deductible, a sub-limit or an exclusion could change the claim base.
Do not invent a universal order
It is tempting to calculate every claim as “bill minus deductible, then co-payment, then sub-limit,” or to put those steps in another fixed order. Neither sequence is universally valid. Policies can define the claim base, the point at which a cap applies and the interaction between clauses differently. Use the operative wording and ask the insurer or third-party administrator to identify the clause and calculation used for a disputed line item.

Cashless treatment does not mean zero payment
For cashless treatment at a network hospital, the insurer pays the hospital through the insurer or third-party-administrator arrangement, subject to approval and the policy. IRDAI’s policyholder guidance makes clear that expenses above the limits or sub-limits allowed by the policy, and expenses that are not covered, must be settled directly by the policyholder with the hospital.
Your payment at discharge can therefore include a deductible, co-payment, charges above a sub-limit, excluded items and any amount the insurer has not admitted. A pre-authorisation amount is not a final promise that every invoice item will be paid. Keep the final settlement statement and ask for the reason and policy clause for each deduction.
Read the Customer Information Sheet and the full wording together
IRDAI says the Customer Information Sheet is supplied with the policy and should summarise minimum information, including coverage, exclusions, sub-limits, deductibles and waiting periods. It is a useful comparison document, but the full policy wording and schedule contain the operative conditions.
A practical document-checking sequence
- Locate the CIS. Confirm that the insurer supplied it with the policy and identify every listed deductible, co-payment, sub-limit and waiting period.
- Match the schedule. Check the insured person, sum insured, selected options, deductible amount, co-payment percentage and any category limits against what you intended to buy.
- Read definitions. Find the policy’s definitions of admissible expenses, hospitalisation, room, treatment, claim and any special terms used in the limits.
- Read exclusions and waiting periods. A treatment can appear in a benefits table yet remain unavailable during a waiting period or excluded in a stated circumstance.
- Trace the claim clause. Look for the section explaining documents, pre-authorisation, cashless approval, reimbursement and the basis on which deductions are made.
- Record discrepancies. If the CIS, schedule and wording appear inconsistent, request written clarification from the insurer before relying on the benefit.
How to compare two health-insurance plans
Comparing only the headline sum insured or premium can hide the provisions that determine your out-of-pocket exposure. Put the actual contract language side by side using these fields.

Then model your own realistic scenarios without treating the result as a guaranteed settlement: a routine admission, a treatment that has a category cap, a claim at a non-preferred hospital if the policy distinguishes hospitals, and a second claim later in the policy year. Ask the insurer to confirm the applicable clause and claim base in writing.

Step-by-step: before buying, before admission and at settlement
Before buying
- Write down the maximum amount you could pay from savings for one admission and for repeated claims.
- Use the CIS to list every deductible, co-payment and sub-limit, including the trigger and frequency.
- Verify each item in the full wording, especially definitions of admissibility and room eligibility.
- Ask for written answers to any mismatch or ambiguous interaction before paying the premium.
- Keep the final CIS, schedule, wording, proposal information and insurer responses together.
Before a planned admission
- Confirm that the hospital is in the network if you intend to use cashless treatment.
- Ask the hospital’s insurance desk and the insurer or TPA which documents and pre-authorisation steps are required.
- Request an estimate that separates treatment charges, room charges and likely non-covered items.
- Ask which deductible, co-payment or sub-limit is expected to apply and what amount is not payable under the policy.
At discharge or reimbursement
- Obtain the final itemised bill, discharge summary, insurer authorisation and settlement statement.
- Separate your payment into deductible, co-payment, amounts over a sub-limit, excluded items and unexplained deductions.
- For each disputed amount, request the exact policy clause and the calculation base used.
- Submit missing documents or a written grievance through the insurer’s stated process if the explanation does not match the contract.
Common mistakes and how to avoid them
“The sum insured is ₹X, so every expense up to ₹X is covered.”
A sum insured is not a promise to pay every rupee of every bill. Exclusions, waiting periods, deductibles, co-payments, sub-limits and admissibility rules can all restrict payment.
“The co-payment is taken from the total invoice.”
IRDAI’s definition refers to the admissible claim amount. Identify the policy’s definition and ask which items were admitted before applying the percentage.
“A deductible reduces my sum insured.”
IRDAI’s FAQ says a deductible does not reduce the sum insured. The deductible still affects what the insurer pays on the claim, so distinguish the available limit from the amount payable for a particular event.
“Cashless means I owe nothing.”
Cashless processing changes how the insurer pays the network hospital; it does not waive expenses above limits or expenses outside coverage. Budget for the patient share and ask for a written estimate.
“The CIS is all I need to read.”
The CIS is a required summary of important features, but the full policy wording and schedule govern the claim. Use the summary to find issues, then verify the operative clause.
“All insurers apply these clauses in the same order.”
There is no safe universal sequence. Where more than one provision appears relevant, obtain the insurer’s clause-by-clause calculation rather than relying on an online formula.
India-specific scope and the limits of these definitions
The explanations here use IRDAI’s India-focused consumer and regulatory guidance: the Health Department material, the “FAQs on Health Insurance Regulations” and the “Health Insurance – Policy Holder” guidance. They are not a universal glossary for every country. Other jurisdictions may use the same words with different legal meanings, tax treatment or claim procedures.
If your policy was issued outside India, use that regulator’s definitions and the policy’s own customer-information document. Even in India, a short definition cannot decide whether a specific diagnosis, provider, charge or document satisfies the contract. The insurer’s full wording, the facts of the claim and the admissibility decision remain controlling.
