Can You Reduce Medicare Premiums Withheld From Social Security?

Yes, in some circumstances—but changing how you pay does not lower what Medicare charges. If your Social Security deduction includes an income-related monthly adjustment amount (IRMAA) and your household income fell after a qualifying life-changing event, you can ask Social Security to recalculate it using more recent information. If you have limited income and resources, your state may be able to pay the Part B premium through a Medicare Savings Program.

First identify exactly what is being withheld. The remedy depends on whether the amount is the standard Part B premium, an IRMAA surcharge, a Part D IRMAA amount, or another premium.

What the deduction may include

Most people have the Part B premium deducted from Social Security or Railroad Retirement Board benefits. Medicare also uses income and tax-filing information to set IRMAA, an additional amount charged to higher-income beneficiaries for Part B and, separately, Part D. The income information generally comes from an earlier tax year, usually two years before the premium year. A fall in your current income therefore does not automatically change the deduction.

For 2026, the standard Part B premium is $202.90 per month, according to the Centers for Medicare & Medicaid Services. That is a dated annual figure, not a permanent amount; premiums and IRMAA thresholds can change each year. Your notice or premium statement should show whether the amount above the standard premium is an income-related adjustment.

About the standard Part B amount The regular Part B premium is being withheld. Changing the payment method will not reduce it. Check whether a state Medicare Savings Program could help.
More than the standard Part B amount Part B IRMAA may be included. Ask Social Security for a new determination after a qualifying income-changing event, or appeal an error.
A separate amount from your drug plan Part D IRMAA may be collected in addition to your plan premium. Use the same Social Security life-changing-event or appeal routes for the IRMAA portion.
A Medicare bill rather than a benefit deduction You may not be receiving Social Security or Railroad Retirement Board benefits. Medicare bills you directly; automatic bank debit changes payment logistics, not the assessed premium.

Route 1: ask Social Security to lower IRMAA after income falls

Social Security says, “If you’ve had a life-changing event that reduced your household income, you can ask to lower the additional amount you’ll pay for Medicare Part B and Part D.” The request concerns the IRMAA surcharge, not the underlying standard premium.

Events that can qualify

Social Security lists marriage, divorce, the death of a spouse, loss of income, and an employer settlement payment among qualifying life-changing events. Retirement can be associated with a loss of income, but approval is based on the event, your evidence, the more recent income information, and your tax-filing status—not simply on saying that you retired.

How to make the request

  1. Locate the determination notice. Confirm the premium year, filing status, income used, and whether the amount is for Part B, Part D, or both.
  2. Complete the current form. Social Security’s Medicare Income-Related Monthly Adjustment Amount—Life-Changing Event form is SSA-44. The current version is dated 12-2025. Use the version and instructions that apply to your premium year.
  3. Describe the event and its date. Explain what changed and provide the supporting evidence requested in the form instructions, such as documentation of the event or loss of income.
  4. Report recent income information. SSA-44 allows actual or estimated information for a more recent tax year in the circumstances described by its instructions. Clearly identify estimates and keep records supporting them.
  5. Submit through Social Security’s available channel. Social Security directs applicants to submit SSA-44, contact the agency, or request an appointment. Ask what submission method and documents apply to your case.
  6. Review the new determination. Social Security evaluates the evidence, income and filing status. A request is not guaranteed to be granted, and the resulting amount may still include the standard Part B premium.

If the income information or decision is wrong

A life-changing-event request and an appeal are different procedures. If the IRS income data or another part of Social Security’s determination is incorrect, ask SSA about a new initial determination or an appeal. Your determination notice explains the available appeal rights and how to use them. Do not rely on an SSA-44 request to correct an unrelated data error; tell SSA specifically what information is wrong.

Route 2: apply for a Medicare Savings Program through your state

Medicare Savings Programs (MSPs) are administered by states and are designed for people who meet income and resource rules. Depending on the program and your state’s rules, an MSP can pay the Part B premium, Part A premiums for people who owe them, and some Medicare cost sharing.

The four federal program categories

Program What it may pay Who it generally serves
Qualified Medicare Beneficiary (QMB) Part A and Part B premiums and covered Medicare cost sharing. Medicare beneficiaries meeting the program’s income and resource requirements.
Specified Low-Income Medicare Beneficiary (SLMB) Part B premiums. Beneficiaries whose income is above the QMB range but within the state’s SLMB limits.
Qualifying Individual (QI) Part B premiums. Applicants within QI limits; funding and enrollment administration are handled by states.
Qualified Disabled and Working Individual (QDWI) Part A premiums. Eligible people with disabilities who work and meet the program rules.

2026 limits are a starting point, not a guarantee

Medicare’s 2026 federal figures include a QMB limit of $1,350 in monthly income for an individual and $9,950 in resources. Those are federal figures for 2026 and should not be treated as a universal eligibility test. Medicare says some states use higher limits or exclude certain income or resources, so your state, household and circumstances matter.

How to apply

  1. Contact your state Medicaid or Medical Assistance office. That office accepts and decides MSP applications; Social Security does not make the state eligibility decision.
  2. Ask which MSP categories are available. Request the state’s current income, resource, household and documentation rules, including any exclusions.
  3. Submit the application and requested proof. Keep copies of the application, income records, resource statements and correspondence.
  4. Apply even if you are unsure. Medicare specifically advises people who may qualify to apply rather than relying on a national table alone.
  5. Use free counseling if needed. State Health Insurance Assistance Programs (SHIPs) offer personalized Medicare counseling and can help people understand costs and apply for Extra Help. For the MSP application itself, follow the state office’s instructions.

An MSP can address the Part B premium itself and, for some categories, cost sharing. That is broader than an IRMAA review, which addresses the income-related surcharge. You may explore both routes when your income has fallen and you also have limited resources.

Withholding, billing and bank payments: what will and will not change

Switching from Social Security withholding to a Medicare bill, or setting up Medicare Easy Pay to debit a bank account, changes the payment method only. It does not reduce the premium or remove IRMAA. People who are not receiving Social Security or Railroad Retirement Board benefits are generally billed by Medicare, while others commonly have premiums withheld from benefits.

Should you drop Part B to stop the deduction?

Terminating Part B is a coverage decision, not a routine way to trim a check. Medicare says you usually must request termination in writing, and coverage ends after the request is processed. If you later reenroll without qualifying for a Special Enrollment Period, you may face a late-enrollment penalty as well as a gap in coverage.

Before requesting termination, ask Social Security how the date will work, whether you have other qualifying coverage, and what penalty rules would apply if you return to Part B. Do not cancel solely because the withholding increased until you have compared the premium savings with the loss of outpatient medical coverage and possible future costs.

A practical decision path

  1. Read the notice. Separate the standard Part B premium, Part B IRMAA, Part D IRMAA and any other charge.
  2. Check the cause. If the amount is IRMAA and a listed life-changing event reduced household income, prepare SSA-44 and supporting evidence.
  3. Check for an error. If the tax information, filing status or calculation is wrong, ask SSA about a new initial determination or appeal.
  4. Check state assistance. If income and resources are limited, apply to your state for an MSP even if a national table does not clearly show eligibility.
  5. Keep payment mechanics separate. A bill or bank debit may be more convenient, but neither lowers the assessed amount.
  6. Get help before cancelling coverage. Contact Social Security or a SHIP counselor before any Part B termination request.

Common mistakes to avoid

  • Assuming every increase is IRMAA. Verify the line item; a standard premium, Part D amount or another charge may be involved.
  • Using an old income year without explanation. SSA-44 is for a qualifying event and more recent information under its instructions; use the current 12-2025 form and follow its premium-year guidance.
  • Expecting automatic relief after retirement. Provide the event, date, evidence and income details so SSA can evaluate the request.
  • Using federal MSP limits as a final answer. State rules may be more generous or count income and resources differently.
  • Confusing an appeal with an SSA-44 request. Tell SSA whether you are reporting a qualifying change, disputing the underlying data, or both.
  • Stopping Part B without checking consequences. Coverage loss and a later late-enrollment penalty can outweigh the immediate withholding reduction.
