If a doctor leaves your Medicare Advantage plan’s provider network, you generally must choose another in-network provider for covered care. Your plan should make a good-faith effort to notify you, help you find a replacement, and protect access to medically necessary care already in progress. A major network change may support a Special Enrollment Period (SEP), but Medicare reviews that situation case by case; one doctor leaving does not automatically give you the right to switch plans.
What happens when a Medicare Advantage doctor leaves the network?
Medicare Advantage plans contract with doctors, hospitals and other providers. Those contracts can change during the year. The Centers for Medicare & Medicaid Services (CMS) says providers can join or leave a plan’s network at any time. When your doctor leaves, the ordinary coverage rule is that you select a provider who remains in your plan’s network.

The effect depends on the plan type and the care you need. A Health Maintenance Organization (HMO) generally requires you to use network providers, except for emergencies, urgent care and out-of-area dialysis. A Preferred Provider Organization (PPO) may cover out-of-network care, but its cost-sharing and authorization rules can be different and usually less favorable. Read your plan’s Evidence of Coverage rather than assuming that every Medicare Advantage plan treats an out-of-network visit the same way.

Do not schedule care on the assumption that a previous directory listing or an old appointment guarantees coverage.

What notice and transition help should you receive?
Notice of the network change
Medicare’s provider-network guidance says a plan should make a good-faith effort to give at least 30 days’ notice when a provider leaves its network. The notice may identify the effective date and explain how to locate another provider. “Good-faith effort” is an expectation for communication, not a promise that every member will receive a notice exactly 30 days in advance.
Help finding a replacement
The 2026 Medicare & You handbook says that if a provider leaves a network, you usually cannot change plans solely for that reason, but you can choose a new provider. It also says the plan should help you choose a new provider and continue needed care already in progress. This is a network-access and transition protection. It is not a blanket guarantee that you may keep treating with the departing doctor at in-network cost.

Ongoing or medically necessary care
Tell the plan if changing doctors could interrupt treatment, delay a scheduled procedure or put your health at risk. Ask what transition arrangement applies to your specific course of care, including any referral or prior-authorization requirements. If the plan cannot explain how medically necessary care will continue, contact Medicare at 1-800-MEDICARE (1-800-633-4227).
Verify whether the doctor is really leaving your exact plan
Network information can change, and a practice may accept one product from an insurer while rejecting another. Confirm the exact Medicare Advantage plan name or product, not merely the insurer’s name.

- Call the number on your plan card. Ask whether the doctor is currently in network, the effective date of any termination, and whether visits already scheduled or treatment already under way will be covered.
- Call the doctor’s office. Ask whether the practice still accepts your exact plan and product. Confirm again when you schedule an appointment because participation can change between calls.
- Check the care team. Verify relevant specialists, the hospital where you receive care and any practice locations. A replacement doctor may be in network at one location but not another.
- Request written details. Keep the plan notice, directory result, call date, representative’s name and any reference number. These records help if a claim or authorization is later disputed.
Medicare recommends checking with doctors when scheduling and asking whether they participate in a plan. An online directory is useful for a first check, but it is not conclusive by itself.
Can you switch Medicare Advantage plans?
A single doctor’s departure
Normally, no. Medicare states that a provider can leave during the year and that you usually cannot change plans just because this happens. Your ordinary option is to choose another provider in the existing network.

A significant network change
Medicare’s Special Enrollment Period guidance says a significant provider-network change may qualify as an exceptional circumstance. Medicare decides eligibility individually. If approved, the guidance provides two months to join or switch plans. The possibility should not be treated as an automatic enrollment right: ask Medicare whether your facts meet the standard before relying on an SEP.
The plan contract ends or is not renewed
A provider leaving the network is different from the plan itself ending, being terminated or not being renewed. Those events have separate enrollment windows and dates described in the official notice. Read the notice carefully and contact Medicare for the rule that applies to your plan and location.

| Situation | What usually happens | What to ask |
|---|---|---|
| One doctor leaves the network | Choose another in-network provider; a plan change is usually not available solely for this reason. | When does the termination take effect, and how will ongoing care be handled? |
| Several providers or a major part of the network changes | An exceptional-circumstances SEP may be possible after individual Medicare review. | Does Medicare find the change significant enough for an SEP, and when would the two-month window run? |
| The plan contract ends, is terminated or is not renewed | Separate enrollment rules and dates apply. | What does the official plan notice say about your enrollment window? |
| The doctor remains available out of network | A PPO may cover the visit under out-of-network rules; an HMO generally requires network care except for specified exceptions. | What cost-sharing, authorization and referral rules apply to this exact service? |
What to do if treatment is already scheduled or underway
- Identify the service and timing. Note the appointment date, procedure, prescription, therapy course or other treatment that could be affected.
- Ask the plan for a transition decision. Use the member-services number and ask whether the service is covered, whether the departing doctor can complete a defined course of care, and whether an authorization must be transferred or renewed.
- Ask for an in-network replacement with the right expertise. Explain the condition, specialty, location and timing. Request more than one option if the first provider cannot accept new patients.
- Confirm referrals and prior authorization. HMOs and some other plans may require a primary-care referral or prior authorization. A new doctor may need records and a new authorization even when the treatment itself is covered.
- Protect continuity of records. Ask the departing practice to send your records, medication list, test results and treatment plan to the replacement provider. Keep copies of your own documents.
- Escalate an unsafe interruption. If the plan’s response could interrupt medically necessary care or put your health at risk, call 1-800-MEDICARE and describe the network change and the care at stake.
How to compare another Medicare Advantage plan
If Medicare confirms that an SEP applies, or if you are comparing plans during an otherwise available enrollment period, evaluate the whole plan rather than one doctor’s name. A plan that includes your doctor may exclude the hospital, specialist or pharmacy you also need.
| Check | Why it matters | Verification question |
|---|---|---|
| Doctor and practice location | Participation can differ by product and office location. | Is this exact doctor at this exact location in network on the effective date? |
| Hospitals and specialists | Your preferred doctor may refer to providers outside the plan. | Are the relevant hospital and specialists in network? |
| HMO or PPO rules | Out-of-network coverage, referrals and cost-sharing vary. | What happens if I use an out-of-network provider? |
| Prescription coverage | A plan change can affect drug coverage and pharmacy participation. | Are my prescriptions covered, and which pharmacies are preferred or in network? |
| Premiums and out-of-pocket costs | Total spending is more than the monthly premium. | What costs apply to my expected visits, procedures and medicines? |
| Effective date | A new plan does not necessarily start immediately. | When does coverage begin, and which plan pays for care before then? |
Medicare advises checking doctors and pharmacies with the plan before joining. Confirm participation with both the plan and the practice close to enrollment and again before care. Keep in mind that plan rules and costs differ by plan and service area.
Common mistakes to avoid
- Assuming the insurer’s name is enough. One insurer can offer multiple Medicare Advantage products with different networks.
- Treating a directory as a guarantee. Directories can lag behind contract changes; call the plan and provider.
- Assuming 30 days’ notice guarantees continued visits. The 30-day figure describes the plan’s good-faith notice effort, not an automatic right to keep the doctor at in-network rates.
- Switching plans before checking the effective date. A new plan may not cover a service until its start date, and an SEP may not apply.
- Confusing a network departure with plan termination. The enrollment rights and deadlines are different.
- Ignoring referral or authorization rules. A replacement provider may require steps that your former doctor handled under the old arrangement.
Questions to ask on your calls
Questions for the plan
- Is my doctor in network today under my exact plan and product?
- What is the effective date if the doctor is leaving?
- Will my already scheduled visit or ongoing treatment be covered?
- Can the doctor complete a current course of medically necessary care?
- Which in-network providers can take over this condition, and do I need a referral or prior authorization?
- What appeal or escalation process applies if access to care is interrupted?
Questions for the doctor’s office
- Do you still accept my exact Medicare Advantage product?
- If not, what date will you stop participating?
- Will you continue seeing me under any transition arrangement communicated by the plan?
- Can you transfer my records and treatment plan to an in-network replacement?
The practical answer
Plan on choosing a new in-network doctor unless your plan confirms a specific transition arrangement or your PPO’s out-of-network rules make continued care workable. Verify the change with both the plan and the practice, ask for help with medically necessary care already in progress, and contact Medicare before assuming that a network departure creates an enrollment opportunity. Only Medicare can determine whether a significant network change qualifies you for an exceptional-circumstances SEP; a plan contract ending or nonrenewal follows separate rules.
