Yes. Receiving Social Security does not automatically disqualify you from Medicaid. The result depends on whether your benefit is SSI, Social Security retirement, or SSDI; which Medicaid eligibility group applies; your income and, in some groups, your resources; and the state where you live.
SSI has a special connection to Medicaid in most states, while retirement and SSDI benefits are tied to Medicare rather than automatic Medicaid enrollment. You can receive Medicare and Medicaid at the same time if you qualify for each program separately.

First, identify which Social Security benefit you receive
“Social Security” is often used as an umbrella term, but the programs have different rules. Before estimating Medicaid eligibility, identify the exact benefit shown on your award notice or benefit statement.
Supplemental Security Income (SSI)
SSI is a needs-based federal benefit for people with limited income and resources who are aged, blind, or disabled. The Social Security Administration (SSA) says SSI recipients may be automatically eligible for Medicaid in most states. However, some states require an SSI recipient to complete a separate Medicaid application through a state or local agency. The process also can depend on the state’s Medicaid structure and the person’s circumstances. See SSA’s SSI and eligibility guidance.

Social Security retirement benefits
Retirement benefits are based primarily on a worker’s earnings record and claiming status. Receiving them does not by itself create Medicaid eligibility. A retiree must qualify under a state Medicaid category, such as an aged or low-income pathway, or may qualify for a Medicare Savings Program that helps with Medicare expenses.
Social Security Disability Insurance (SSDI)
SSDI is an insurance benefit based on work history and disability. SSDI recipients commonly become entitled to Medicare after the applicable waiting period, but SSDI alone does not guarantee Medicaid. A person may qualify for Medicaid through disability-related rules, income-based rules, a Medicaid Buy-In option, or another state pathway.
People receiving SSI and Social Security together
Some people receive both SSI and a Social Security benefit. SSA treats Social Security benefits as unearned income when calculating SSI, although deductions and exclusions can affect the amount of SSI payable. That SSI calculation should not be treated as a universal Medicaid income formula: Medicaid uses different methods depending on the eligibility group. SSA explains the SSI treatment of Social Security at SSI Income.

How a state decides Medicaid eligibility
Medicaid is a joint federal-state program. Federal law sets mandatory eligibility groups and permits states to cover additional groups. Each state Medicaid agency applies the rules for its programs, sets many income thresholds, and determines which application route and verification documents are required. The federal framework is described in CMS’s Eligibility Policy.

MAGI rules for most children, pregnant people, parents, and adults
Most children, pregnant people, parents, and adults who qualify through an expansion or family-based category are assessed using Modified Adjusted Gross Income (MAGI). MAGI generally starts with taxable income and applies tax-filing relationships and household rules. For MAGI categories, the method generally does not include an asset or resource test. A bank balance or vehicle therefore is not normally evaluated under the MAGI financial test, although other nonfinancial requirements still apply.
MAGI eligibility is not one national number. Income limits vary by state, household size, category, and year. The state may ask about wages, interest, pensions, Social Security, and other income, then apply the category’s counting rules. Residency in the state and U.S. citizenship or qualifying noncitizen status are also generally required, subject to program-specific exceptions.
SSI-related methods for people who are aged, blind, or disabled
When Medicaid eligibility is based on being at least 65, blind, or disabled, states generally use financial methods associated with SSI rather than MAGI. These rules can consider both income and countable resources. A Social Security payment that produces one result in a MAGI category can produce a different result in an aged, blind, or disabled category because the methodology and allowable deductions differ.

Some states are known as “209(b)” states. They may use eligibility criteria that are more restrictive than SSI, within federal limits. SSA guidance explains that a 209(b) state must allow an aged, blind, or disabled applicant to spend down countable income with incurred medical expenses. The spend-down amount and procedure are state-specific; ask the agency how it calculates the liability, which expenses qualify, and what documentation is needed. The relevant policy is at SSA’s Medicaid and SSI Program guidance.

Special disability-related pathways
States may offer a Medicaid Buy-In for certain people with disabilities who work. These programs can allow coverage at income levels above a standard disability category, often with premiums or other conditions. Availability, disability definitions, income limits, and premiums differ by state.
Section 1619(b) is another specialized route. Under SSA’s rules, a person whose earnings end the SSI cash payment may continue Medicaid if the person has a qualifying disability or blindness, previously qualified for SSI and Medicaid, needs Medicaid in order to work, and has earnings insufficient to replace the value of SSI, Medicaid, and certain publicly funded attendant or institutional care. Details and thresholds are explained in SSA’s 1619(b) Spotlight. This is not automatic for every former SSI recipient; the state and SSA must confirm the conditions.
Can you have Medicare and Medicaid at the same time?
Yes. Medicare enrollment does not bar Medicaid. Many people receiving retirement or disability benefits have Medicare, and some also qualify for Medicaid because of limited income, resources, age, disability, or another state category.

CMS lists 2026 MSP standards on its Seniors & Medicare and Medicaid Enrollees page. The figures below are the amounts displayed for most states and the District of Columbia; Alaska and Hawaii have different figures. They apply to the named MSPs, not to every Medicaid category.

There is no single answer for every Medicaid applicant. The state first identifies the eligibility category, then applies that category’s counting rules.

That can reduce or eliminate the SSI payment after applicable exclusions.
- For MAGI Medicaid: The state generally uses MAGI-based taxable-income rules and household relationships. Whether a particular Social Security amount is included, excluded, or treated in a particular way depends on the applicable tax and Medicaid rules.
- For aged, blind, or disabled Medicaid: The state generally uses SSI-related methods, which can include income and resource tests and state-specific deductions or disregards.
- For an MSP: The program has its own income and asset standards and may use rules that differ from full Medicaid.
Because the same monthly benefit can be evaluated differently in different categories, do not compare your gross Social Security payment with a number found online unless the number is for your state, category, household size, and coverage year.

How to apply or request an eligibility review
- Write down the exact benefit. Note whether you receive SSI, retirement, SSDI, a combination, or survivor benefits. Include the gross monthly amount and any deductions shown on your notice.
- Confirm your state and county of residence. Medicaid is administered through the state in which you live. A move can change the application channel, limits, and available categories.
- Decide what help you are seeking. State that you want full Medicaid, help paying Medicare costs through an MSP, a disability-related Medicaid pathway, or more than one of these.
- Contact the state Medicaid agency. Use the agency’s online application, telephone line, local office, or other official channel. CMS states: “To find out for sure if you’re eligible for Medicaid, you must contact your state Medicaid agency.” Start at CMS’s Medicaid eligibility page to locate the correct agency.
- Ask which methodology will be used. Specifically ask whether the review is MAGI or aged/blind/disabled SSI-related, whether an asset test applies, and whether your state is a 209(b) state with a medical spend-down process.
- Submit verification and keep proof. Provide the documents requested, save the confirmation number, and note deadlines for responding to notices or requesting a fair hearing.
Documents and questions that make the review faster
Requirements vary, but gathering the following can reduce delays:
- Social Security or SSI award letters and recent benefit statements
- Medicare card and premium information, if enrolled
- Proof of identity, date of birth, and state address
- Citizenship or qualifying immigration documents, if requested
- Bank and other resource information when applying under a category with an asset test
- Medical bills and receipts if the state uses a spend-down calculation
- Pay stubs, employer information, and disability-related work details for a Medicaid Buy-In or 1619(b) inquiry
Ask the caseworker: “Am I being screened for full Medicaid, an MSP, or both?” “Does my state require a separate Medicaid application even though I receive SSI?” “Which income and resources count for this category?” “If I am over the limit, is there a spend-down, Buy-In, or continued-Medicaid option?”
Common situations
SSI recipient in a state with automatic Medicaid linkage
The state may enroll the person through the SSI process, but the person should still read notices and report changes. A missing card or an unexpected denial should be taken to the state Medicaid agency rather than assumed to be an SSA error.
SSI recipient in a state requiring a separate application
SSI approval does not replace the Medicaid application. The person should follow the state or local agency referral and submit the requested information promptly.
Retiree with Medicare and limited income
The person may not qualify for full Medicaid, but an MSP could pay some Medicare costs. The MSP application is worth asking about even when the person assumes Medicare makes Medicaid impossible.
SSDI recipient who is working
Ordinary SSDI and Medicare rules may not provide Medicaid. A state Medicaid Buy-In or, for someone formerly receiving SSI, section 1619(b), may offer a separate route if all conditions are met.
Mistakes to avoid
- Assuming every Social Security recipient is excluded: Social Security income does not create a blanket Medicaid disqualification.
- Calling SSI “Social Security retirement”: SSI is a separate, needs-based program with its own rules.
- Promising automatic Medicaid for every SSI recipient: Most states link SSI and Medicaid, but some require a separate application.
- Using one national income limit: Limits depend on state, category, household, year, and sometimes resources.
- Confusing an MSP with full Medicaid: MSPs target Medicare premiums and cost sharing; they are not automatically comprehensive Medicaid coverage.
- Ignoring nonfinancial rules: Residency and citizenship or qualifying noncitizen status can affect eligibility even when income is low.
- Failing to report changes: Moving, marriage, work, a change in benefits, or a change in resources can alter eligibility.
Bottom line
You can receive Medicaid while receiving Social Security benefits. SSI usually provides the clearest Medicaid connection, but state procedures differ. Retirement and SSDI benefits do not guarantee Medicaid, yet they also do not prevent it. The decisive questions are your exact benefit, Medicaid category, state rules, income, resources where applicable, and whether you need full coverage or only help with Medicare costs. Your state Medicaid agency is the only source that can make the individual determination.
