Medicare generally does not pay for routine care outside the United States, and the U.S. government does not pay citizens’ medical bills abroad. A practical shortlist for retirees is Cigna Global, Allianz Care, AXA Global Healthcare, IMG Global Medical or Global Prima, and GeoBlue Xplorer—but no insurer is best for every destination, age, medical history, or travel pattern.
Your decision should follow your residence country’s public-health and visa rules, the amount of time you will spend in the United States, whether you need routine outpatient care, how pre-existing conditions are handled, and what deductible, evacuation, renewal, and annual-limit terms you can accept.

What kind of insurance do you need after retiring abroad?
Choose a residence-based international medical plan when you will live overseas for months at a time or indefinitely. IPMI can combine inpatient treatment, outpatient visits, diagnostics, surgery, prescriptions, preventive services, cancer care, and medical evacuation, depending on the plan and options selected. It is designed to continue from year to year, subject to the policy’s renewal and premium provisions.

How Medicare fits in
Medicare.gov states: “Medicare usually doesn’t cover health care while you’re traveling outside the U.S. but there are some exceptions.” Those exceptions are narrow and fact-specific; Medicare is not an overseas primary plan. Medicare Part D generally does not pay for prescriptions purchased outside the United States. Medigap policies may cover limited emergency care abroad, so read your own policy rather than assuming it provides comprehensive international coverage.
The U.S. Department of State says: “The U.S. government does not pay medical costs for U.S. citizens traveling abroad.” A consulate may help you locate providers or communicate during a crisis, but it does not replace insurance or pay the bill.

| Cigna Global | Retiree-oriented plans; worldwide or worldwide-excluding-U.S. choices; three plan levels and four optional add-ons on its expatriate offering; support for selected ongoing conditions and comprehensive cancer care; senior page says there is no upper age limit and describes guaranteed renewability. | Whether the desired plan is offered in your residence country; which chronic conditions qualify; U.S. visit limits; exclusions, waiting periods, annual maximums, and how future premiums can change. |
| Allianz Care | International plans for people living, working, or traveling abroad for extended periods; typical benefits include hospitalization, outpatient treatment, vaccinations, surgery, diagnostics, and emergency evacuation. | Exact benefits and sublimits, pre-existing-condition underwriting, direct-billing locations, renewal terms, and the effect of age, residence, destination countries, and medical history on price. |
| AXA Global Healthcare | Worldwide access to medical care and a second-medical-opinion service. | Whether the selected area includes the U.S., how long you may return there, specialist and cancer benefits, evacuation language, authorization rules, and claims procedures. |
| IMG Global Medical or Global Prima | Plan levels, geographic coverage areas, deductible choices, and different payment modes. IMG also publishes a GlobeHopper Senior travel product with single-trip terms from five days to 12 months for Medicare-qualified U.S. citizens and permanent residents age 65 or older who have Parts A and B plus Medigap or Medicare Advantage. | Do not confuse the single-trip senior product with residence-based IPMI. Confirm eligibility, maximum trip length, pre-existing-condition rules, renewability, and whether the medical plan covers routine care where you live. |
| GeoBlue Xplorer | A brochure describes comprehensive worldwide benefits and access to care inside and outside the United States. | U.S.-return limits, exclusions, direct billing, preauthorization, annual limits, cost sharing, and renewal provisions in the current certificate and schedule. |
These descriptions do not establish a universal ranking or an apples-to-apples price comparison. Availability, underwriting, and wording can differ by U.S. state, destination, age, and plan edition.
Decide whether you need U.S. coverage
Worldwide coverage that includes the United States is usually more expensive than worldwide coverage excluding the United States, because American hospital and specialist charges can be substantially higher. The right choice depends on where you will actually obtain care, not on a vague desire for flexibility.

Choose worldwide including the U.S. when
- You expect planned treatment, specialist visits, or extended stays with family in the United States.
- You divide the year between the United States and another country and need the same policy in both places.
- Your destination has limited specialist or cancer services and you want the option to return to a U.S. provider.
Consider worldwide excluding the U.S. when
- You will be resident abroad and use local or regional providers for nearly all care.
- You can meet U.S. medical costs through Medicare, a separate domestic policy, or personal funds during visits.
- The premium difference is significant and the plan’s U.S.-return provision would not otherwise be used.
Read the definition of “area of cover” and the permitted U.S. stay in the certificate. A plan can say “worldwide” while limiting planned treatment in the U.S., requiring preauthorization, or imposing a maximum number of days per visit.
Compare the contract, not the headline
Use the following checklist for every quote. Record the answer in writing; an insurer’s sales summary is not a substitute for the policy contract.
Eligibility and medical underwriting
- What is the maximum entry age, and is there an upper age limit for renewal?
- Is a full medical questionnaire required, or does the insurer use a moratorium or automatic exclusion approach?
- Which diagnoses, medications, surgeries, and symptoms must be disclosed?
- Are pre-existing conditions covered, excluded permanently, covered after a waiting period, or accepted with a premium loading?
Answer health questions completely. Omitting a diagnosis or symptom can lead to a claim being declined or a policy being voided, even if the condition seems unrelated to the later treatment.
Benefits and limits
Separate broad annual limits from sublimits. Check inpatient room and board, intensive care, surgery, outpatient consultations, diagnostic imaging, laboratory work, cancer treatment, mental-health care, rehabilitation, prescription drugs, preventive examinations, dental and vision benefits, and medical evacuation. A plan can advertise a high overall maximum while imposing much lower limits on outpatient drugs, therapy, or evacuation.
Cost sharing
Compare the deductible, coinsurance percentage, copays, and annual maximum together. A low premium with a high deductible may be sensible if you mainly want protection from a catastrophic hospitalization; it may be poor value if you expect frequent outpatient visits. Ask whether the deductible applies per person, per condition, per policy year, or per claim, and whether it applies to evacuation or only treatment.

Administration during a crisis
- Is there a 24/7 assistance line, and in which languages?
- Which hospitals can bill the insurer directly?
- What treatment requires preauthorization, and how quickly can approval be obtained?
- Can claims be submitted electronically, and what documentation is required?
- Who arranges evacuation, and does the policy pay for a medically appropriate destination or only transport to the nearest facility?
Continuity after relocation or diagnosis
Ask whether the policy is guaranteed renewable, portable when you change residence, and renewable after you develop an expensive condition. A guaranteed-renewability statement does not necessarily freeze premiums. Cigna’s senior material, for example, says future premiums are not based on an individual’s claims history, but you still need to read how age bands, medical inflation, residence, and overall portfolio pricing affect renewal rates.
Local legal requirements
Some countries require enrollment in a public system, a local supplementary policy, or visa-compliant insurance with specified minimum limits. An international plan may pay claims yet fail a visa application if it lacks required wording, territorial coverage, or an insurer licensed for that jurisdiction. Confirm requirements with the destination’s official government or embassy source before paying a deposit.
How to buy coverage step by step
- Confirm your status in the destination. Determine whether you will be a resident, long-stay visitor, or dual resident, and identify public-health enrollment, visa, and local-insurance rules.
- Map your calendar. Write down the countries where you will sleep, the number of days in the United States, and any planned treatment trips. This determines whether you need worldwide coverage including the U.S., worldwide excluding the U.S., or a regional area.
- Set a benefits floor. Decide the minimum annual maximum, evacuation benefit, outpatient coverage, cancer coverage, prescription benefit, and mental-health coverage you can accept before comparing prices.
- Request at least three quotes. Apply with the same residence, travel pattern, deductible, and benefit options so the results are comparable. Disclose every material medical fact accurately.
- Read the full documents. Compare the certificate, schedule, exclusions, waiting periods, definitions, preauthorization rules, claims deadlines, renewal clause, and U.S.-return limits—not only the summary page.
- Test the provider network. Ask whether hospitals and specialists near your intended home accept direct billing and whether the insurer can support your preferred language.
- Plan for Medicare separately. Use Medicare.gov for enrollment and penalty questions. Do not cancel or delay a U.S. enrollment decision solely because an overseas insurer’s brochure sounds comprehensive.
- Keep an emergency file. Store the policy number, assistance telephone number, exclusions, medication list, medical history, and copies of passports and prescriptions where a companion can access them.
How the five options may fit different retirees
Cigna Global: a retiree-focused first quote
Cigna’s retiree page describes tailored plans, a global network, support for eligible chronic conditions, and comprehensive cancer care. Its expatriate offering lists Worldwide and Worldwide excluding U.S. choices, three plan levels, four optional add-ons, and flexible payment options. The senior page states, “No upper age limit – we welcome applications up to any age.” Treat these as product descriptions: confirm availability, underwriting, and exact wording for your country and age.
Cigna reports that more than 1.5 million hospitals, clinics, and doctors are in its network. A separate corporate page cites more than 2.5 million hospitals and health-care professionals and support in over 200 markets and territories. Those figures are corporate network claims and should not be assumed to mean every provider is available under every individual retiree policy. Verify the facilities you would actually use.
Cigna also reports that 13% of U.S. retirees and workers dream of living abroad. The page does not identify the underlying survey, so regard the figure as Cigna-reported marketing context rather than an independently verified estimate.

Allianz Care: broad benefits with location-sensitive pricing
Allianz describes plans for people living, working, or traveling abroad for extended periods. Hospitalization, outpatient care, vaccinations, surgery, emergency evacuation, and diagnostics are among the typical services it lists. It says price varies with age, country of residence, destination countries, number of members, coverage level, pre-existing conditions, and medical history. Compare the quote only after those variables are held constant across providers.
AXA Global Healthcare: useful if second opinions matter
AXA highlights worldwide access to expert care and a second medical opinion. That feature can be valuable for a serious diagnosis, but ask whether it is included or optional, whether it changes the benefit limit, and whether the opinion is advisory or connected to authorization for treatment.
IMG Global Medical and Global Prima: configurable designs
IMG emphasizes plan levels, geographic areas, deductibles, and payment modes. That configurability can help you balance premium and risk, but it also makes superficial comparisons unreliable. Place the selected area, deductible, annual limit, and outpatient option side by side with competing quotes.
IMG’s GlobeHopper Senior description is for single trips of five days to 12 months for Medicare-qualified U.S. citizens and permanent residents age 65 or older who have Parts A and B plus Medigap or Medicare Advantage. It is trip insurance, not evidence that a retiree living abroad has continuous residence-based coverage.

Pre-existing conditions, age, and chronic-care planning
Retirees should assume that medical history will affect eligibility, exclusions, or price until the insurer confirms otherwise in writing. Diabetes, heart disease, cancer history, respiratory disease, joint replacements, and ongoing prescriptions are not reasons to hide information; they are reasons to obtain a formal underwriting decision.
Ask specifically whether a condition is covered for acute flare-ups, routine monitoring, maintenance medication, complications, and related conditions. A policy might cover a new unrelated illness while excluding treatment connected to a known diagnosis. Also check whether a waiting period applies to cancer, maternity, mental-health care, dental work, or elective procedures.
For chronic conditions, continuity matters more than a headline annual maximum. Confirm that your doctors can continue treatment, prescriptions can be filled legally in the country, laboratory monitoring is covered, and the insurer will authorize referrals without forcing unnecessary travel.

No reliable universal premium can be quoted for a 60- or 70-year-old from the information available. International premiums are personalized and can change materially with age, residence country, destination countries, U.S. coverage, benefit level, deductible, members insured, pre-existing conditions, and medical history. Insurers may also apply country-specific taxes, currency rules, and medical inflation adjustments.
Instead of comparing a single monthly figure, request each quote with the same assumptions and calculate three numbers: annual premium, maximum routine spending from deductibles and copays, and the largest plausible exposure after annual limits or exclusions. A cheaper quote that removes outpatient drugs or evacuation may not be cheaper for your actual care pattern.
Common buying mistakes and fixes
- Assuming Medicare follows you abroad: verify the limited exceptions and keep U.S. enrollment decisions separate.
- Buying a trip policy for a permanent move: select annually renewable IPMI and use travel insurance only for genuinely temporary trips or gaps.
- Choosing worldwide coverage without reading U.S. limits: record the number of permitted days and whether planned care is allowed.
- Focusing on the annual maximum: inspect sublimits for drugs, outpatient care, evacuation, therapy, and mental health.
- Failing to disclose medical history: provide complete records and ask for written confirmation of accepted, excluded, or loaded conditions.
- Ignoring direct billing: confirm that nearby hospitals can obtain guarantees of payment and learn the preauthorization process before an emergency.
- Overlooking local law: verify visa and public-system requirements with official authorities, not an insurer’s sales page.
- Assuming a network number guarantees access: check the actual facilities and specialists in your destination.
A practical decision rule
For most Americans retiring overseas, obtain three or more IPMI quotes, then eliminate any plan that fails your destination’s legal requirements, excludes a necessary condition, lacks acceptable evacuation, or cannot support your normal U.S. travel pattern. Among the remaining plans, choose the one whose renewal language, direct-billing network, cost sharing, and realistic access to care best match how you will live—not the one with the most impressive headline network or the lowest initial premium.
