OtoSet, developed by SafKan Health, is a clinical ear-cleaning system that combines irrigation and suction to remove earwax from the external ear canal. It received FDA 510(k) clearance in 2020, but the filing identifies it for prescription use in adults—not as a broadly approved, unsupervised home gadget.
The device became a business and social-media story in January 2023, when SafKan announced an $8 million Series A and coverage highlighted a TikTok demonstration that had reportedly exceeded 15 million views. Those milestones show commercial and public interest; they do not prove that OtoSet is superior to conventional earwax removal or currently available to ordinary consumers.

What is OtoSet?
OtoSet is shaped somewhat like oversized headphones, but it is not an audio product that happens to clean ears. Its head-mounted design positions two ear units over the patient’s ears while the system performs a clinical ear-cleaning procedure.

How the procedure works
- A clinician positions the head-mounted device over the patient’s ears and fits the ear units.
- Liquid is directed through disposable tips toward the ear-canal walls.
- Irrigation helps loosen or break down cerumen, the medical term for earwax.
- Continuous micro-suction draws liquid and loosened wax into disposable waste containers.
SafKan described OtoSet as an automated irrigation-and-micro-suction system. Treatment time will vary with the amount and hardness of wax, the patient’s anatomy, the fit of the ear tips, and the clinician’s workflow. A short automated cycle does not guarantee that every impaction will be removed without additional manual treatment.
What the FDA clearance actually means
OtoSet was not “FDA-approved” in the way that wording is commonly understood for a drug or a high-risk device. The FDA cleared it through the 510(k) pathway.
Under a traditional 510(k), the FDA determines whether a new device is substantially equivalent to a legally marketed predicate device. OtoSet’s record identifies the Welch Allyn Ear Wash System as its primary predicate and records a November 5, 2020 decision of substantial equivalence.
The FDA record lists the product under “syringe, irrigating (non-dental)”, regulation number 21 CFR 880.6960, and product code KYZ. Its cleared indication is narrower than “general ear cleaning”: removal of earwax from the external ear canal using irrigation and suction in adults.
Clearance therefore does not establish that OtoSet is better than microsuction, manual removal, or ordinary clinical irrigation. It also does not automatically authorize use in children, people with every type of ear condition, or unsupervised consumers.

Why SafKan saw a business opportunity
Impacted earwax can interfere with hearing, block an ear examination, and affect hearing-aid use. Primary-care offices, audiology practices, hearing-care providers, and other clinics may spend staff time removing wax through irrigation, manual extraction, or suction.
GeekWire reported in 2023 that the American Academy of Otolaryngology estimated excessive or impacted earwax affects roughly 10% of children and 5% of adults, and that about 8 million earwax-removal procedures take place annually in U.S. clinics. Those are attributed historical estimates, not newly verified 2026 statistics.

SafKan’s commercial proposition was mainly a workflow proposition: automate parts of a familiar clinical procedure, combine irrigation with suction, capture waste, and potentially make treatment more consistent for practices. That is different from selling a convenience appliance for every household.
The $8 million financing and the TikTok effect
On January 17, 2023, GeekWire reported that SafKan had raised an $8 million Series A, bringing the company’s reported total funding to $13 million. The round was reported as led by Unorthodox Ventures, with participation from Dreamit Ventures, PCOM Primary Care Innovation Fund, Riptide Ventures, Healthtech Capital, Alliance of Angels, and individual investors.
The reported financing was intended to support expanded manufacturing, marketing, hiring in training and sales, manufacturing optimization, a second-generation device, and broader clinic adoption. Earlier financing reportedly included a $2.5 million seed round and a $2.5 million convertible note.

SafKan was founded in 2016 by brothers Sahil and Aadil Diwan. According to the 2023 profile, Aadil’s recurring earwax problem helped inspire the concept; early prototypes used 3D printing and were tested with a Seattle-area clinical partner.
The TikTok demonstration gave the product a much wider audience. Viewers could see a person press a button while fluid moved through the headphone-like device, creating the visually satisfying proof of concept that often performs well on social platforms. The video reportedly exceeded 15 million views by January 2023.
That view count is a historical measure of attention, not clinical evidence. Virality does not establish safety, effectiveness across different types of impaction, sustainable demand, or a viable consumer price.

OtoSet’s FDA filing identifies the Welch Allyn Ear Wash System as its predicate, underscoring that the product automates and combines established approaches rather than inventing irrigation or suction.
Is OtoSet available for consumers?
The available records confirm the original clinical clearance, but do not establish current consumer availability in the same form described in the 2023 coverage.

The FDA materials identify prescription use, and a historical SafKan announcement said OtoSet was intended to be used by or with a healthcare professional rather than sold over the counter. GeekWire reported a price of $1,759 for a six-month bundle in January 2023, but that figure should not be treated as the current price. The bundle may have included recurring disposable supplies, and product pricing, distribution, and clinic partnerships can change.
SafKan’s 2023 announcement also described a Beltone provider partnership. That historical relationship should not be assumed to represent the current clinic network. A practice evaluating the system should confirm current availability, geographic coverage, consumable pricing, service terms, training, maintenance, and reimbursement before purchasing.
What evidence is available?
FDA clearance establishes authorization for the stated use through a substantial-equivalence determination. It does not establish superiority over manual irrigation or microsuction.
The 2023 reporting said clinicians had treated thousands of people with OtoSet, but that the company had not yet publicly released its clinical data and planned to collect and publish more. Without a publicly available comparative study specifying sample size, endpoints, adverse events, and publication status, phrases such as “clinically proven,” “safer,” or “more effective” should not be treated as established facts.
Safety and patient selection
A clinician should follow the device’s current instructions for use, manufacturer labeling, and professional ear-care protocols. The FDA clearance itself is not a substitute for screening the patient.

People should seek medical guidance rather than improvise an irrigation or suction procedure if they have a history of eardrum perforation or ear surgery, ear tubes, current pain, drainage, bleeding, severe dizziness, unexplained hearing loss, a suspected foreign body, or a possible ear injury. Children are outside the adult indication cited in the FDA filing.
Do not attempt to reproduce the clinic procedure with household syringes, water, camera-tipped tools, or unapproved suction gadgets. Discomfort should not simply be treated as a normal part of the process; the procedure should stop and be assessed if a patient experiences significant pain or other concerning symptoms.
What clinics should evaluate
- Patient population: Whether the practice primarily treats adults who fit the cleared indication.
- Workflow: Whether automation improves throughput compared with existing equipment and staff techniques.
- Consumables: The cost and availability of disposable tips, solution components, and waste containers.
- Training and infection control: Positioning, patient screening, cleaning, single-use components, and waste handling.
- Clinical evidence: Current data, not just a viral demonstration or promotional claim.
- Total cost of ownership: Equipment, supplies, maintenance, service, storage, and staff time.
- Reimbursement: Whether local payers cover the procedure and under what rules.
- Hearing-aid workflow: Whether wax removal improves examinations or hearing-aid service efficiency.
Bottom line
OtoSet is a legitimate FDA-cleared clinical device and a notable health-tech financing story. Its innovation is the automated combination of irrigation and suction, not a new form of earwax biology. The FDA clearance is a 510(k) substantial-equivalence determination for adult prescription use, not blanket approval for unsupervised home treatment.
