Five Common Errors in Requirements Analysis (and How to Avoid Them)

Requirements analysis goes wrong when a team mistakes an informal request for a shared, testable understanding of a need. The result may be ambiguous, incomplete, contradictory, infeasible, or impossible to verify—and those defects can carry into design, development, testing, and operations. NASA’s requirements-analysis guidance highlights qualities such as clarity, completeness, consistency, feasibility, verifiability, and traceability. There is no universally ranked list of five errors; these are recurring failure patterns across requirements elicitation, specification, validation, and management.

What makes a requirement good?

A good requirement is clear, necessary, complete enough for its level, consistent with related requirements, feasible, individually verifiable, uniquely identifiable, and traceable to a need, goal, parent requirement, or constraint. It should also be written at the right level: specific enough to guide and verify the outcome, but not burdened with an unjustified design decision. NASA’s requirement-writing guidance recommends concise statements, one thought per requirement where practical, consistent terminology, and bidirectional traceability.

Keep four kinds of statements distinct:

  • Stakeholder need: Customers need confidence that a submitted claim was received.
  • Requirement: The system shall display a submission confirmation within five seconds after a valid claim is submitted.
  • Design decision: The confirmation will appear in a green modal dialog.
  • Acceptance test: Submit a valid claim and measure the time until confirmation appears; it must be five seconds or less.

The word “shall” can help signal a binding requirement, but it does not make an unclear or incomplete statement good. The goal is a shared understanding and a credible way to show whether the requirement has been met.

1. Writing vague or ambiguous requirements

“The reports shall load quickly,” “the service shall be highly available,” and “users shall receive timely alerts” sound purposeful, but reasonable people can interpret them differently. What counts as quick? Which reports, under what load, and measured from which event? What qualifies as timely? If developers and testers answer those questions differently, the requirement is not yet precise enough.

Common warning words include fast, easy, secure, appropriate, large, promptly, etc., and and/or. NASA specifically cautions against indefinite expressions such as “as appropriate,” “etc.,” and “and/or.” These words are not automatically forbidden, but they need a defined meaning or replacement with observable conditions.

A useful starting structure is: The [system or component] shall [observable action] under [specified condition] within or by [measurable threshold, when applicable]. For example:

  • Weak: The system shall load reports quickly.
    Stronger: The system shall display the first page of a standard report within three seconds for 95% of requests under the defined normal-load profile.
  • Weak: The system shall support large files.
    Stronger: The system shall accept files up to 2 GB and return a validation result within 30 seconds on the supported network profile.
  • Weak: The system shall be secure.
    Better approach: Decompose this into requirements for authentication, authorization, encryption, logging, retention, and recovery as relevant to the product.

The numbers above are examples, not universal targets. Performance and availability thresholds should come from the business need, service commitments, operating environment, and cost trade-offs. Define the measurement conditions too: “within three seconds” is still open to interpretation if the dataset, device, network, load, and start and end points are unknown.

Review each statement by asking: Could two competent people implement it differently? Could two testers disagree about whether it passed? Are the actor, action, object, trigger, condition, and threshold clear? Does the sentence combine multiple obligations? If it does, split it where that creates clearer ownership and verification. Atomicity does not mean fragmenting every sentence into unreadable pieces; it means giving each requirement one clear obligation and a meaningful proof path.

Discovery work may leave legitimate uncertainty. Record an unresolved item as an assumption, TBD (“to be determined”), or TBR (“to be resolved”), with an owner and due date. Do not conceal missing decisions inside language that looks final. NASA recommends maintaining a complete list of unresolved TBDs and TBRs.

2. Missing stakeholders, scenarios, constraints, or quality requirements

A feature can be described while the real operating need remains incomplete. A team may capture what a sponsor requested but overlook administrators, support staff, data owners, compliance reviewers, people with accessibility needs, external systems, or those affected when the service fails. It may also specify the happy path while omitting invalid input, recovery, retention, or operational support.

NASA’s software requirements guidance emphasizes customer and stakeholder requirements and operational concepts. A stakeholder inventory is a practical way to turn that principle into action. Consider primary and secondary users, administrators, operations and support teams, security and privacy owners, legal or compliance stakeholders, data owners, suppliers and external systems, business sponsors, and groups with specialized access needs. The relevant set depends on the product; inviting everyone to every decision is neither necessary nor efficient.

Walk through normal operation and exceptions, including missing or invalid data, permission failures, high volume, concurrent use, network or external-service interruption, recovery and restart, data correction, migration and rollback, audit and reporting, and eventual decommissioning. Ask what happens, who acts, what information is retained, and who owns the decision at each point.

A compact completeness check can cover these areas:

  • Function: What must the product do?
  • People: Who performs or receives each action?
  • Data: What enters, leaves, changes, or must be retained?
  • Quality: What performance, reliability, security, accessibility, privacy, or capacity is needed?
  • Constraints: Which laws, contracts, platforms, interfaces, or standards apply?
  • Failure: What happens when the normal path fails?
  • Operations: Who monitors, maintains, supports, and restores the system?
  • Verification: How will each obligation be demonstrated?

Do not treat every preference as a requirement. Investigate why it is wanted, whether it is necessary, and whether it conflicts with other needs. “Nonfunctional” requirements are not optional simply because they describe quality rather than a user-facing feature: security, accessibility, reliability, and maintainability can determine whether a product is acceptable at all.

3. Confusing an outcome with a premature solution

“The system shall use PostgreSQL,” “the team shall implement this as microservices,” or “the company shall build a mobile app” may be valid decisions in context, but they do not by themselves explain the need. Treating a proposed solution as the requirement too early can constrain alternatives before the team understands the problem.

Ask what outcome is needed, who needs it, what evidence supports the need, whether the implementation is mandatory or merely preferred, what alternatives exist, and what constraints rule those alternatives out. For example:

  • Premature solution: The system shall provide a mobile application for field inspections.
  • Need and outcome: Field inspectors must be able to record, save, and synchronize inspection results while disconnected from the corporate network.

A native app may be the right answer, but so might an offline-capable web application or another approved client. The outcome statement leaves solution evaluation open while still defining what matters.

Design detail can belong in requirements when justified—for example, a mandated interface, a regulatory or contractual platform, compatibility with an existing system, a safety-approved component, or an approved architecture constraint. Label and document it as a constraint or derived requirement, and record why it applies. NASA’s requirements-management guidance also suggests investigating requirements with no parent or acceptable justification as possible “gold-plating”: functionality added without a supported need.

4. Allowing conflicts, duplication, and uncontrolled change

One requirement may say that sessions expire after 15 minutes while another says 30. Two groups may use “customer” to mean different people. A change may be made in a document but not in the test plan, backlog, or operating procedure. These are not merely editing problems: they make it unclear which outcome is authorized and whether downstream work still matches it.

Use a controlled source of truth, assign each requirement a unique ID, and record its source, rationale, owner, type, status, and version. Link it to related parent needs, assumptions, dependencies, design or delivery items, tests, and risks where useful. When a change is proposed:

  1. Assess its effect on related requirements, architecture, tests, schedule, cost, risks, and operations.
  2. Record the decision, rationale, priority, and approver.
  3. Update affected artifacts and relationships rather than changing only the original statement.
  4. Repeat relevant checks for clarity, conflicts, completeness, feasibility, and verification.
  5. Baseline the approved set and communicate what changed.

A change is not automatically scope creep. New regulation, a safety issue, validated research, a changed business strategy, or a corrected misunderstanding can justify one. The problem is inserting change without assessment, approval, prioritization, or propagation to affected work. NASA recommends evaluating changes against an approved requirements baseline and maintaining traceability between parent and child requirements.

For a small project, a structured table may be enough. Complex or regulated work may need a dedicated requirements-management system, formal approvals, baselines, and audit evidence. The principle is proportionality: maintain enough control to prevent silent drift without creating process that costs more than the risk it addresses.

5. Skipping validation, verification, or traceability

A spelling review or stakeholder signature does not establish that requirements are complete, feasible, or testable. Nor does a test written late in development guarantee that the team understood the original need. NASA’s systems-engineering process guidance describes validation in terms that include well-formed requirements, completeness against stakeholder needs, consistency, individual verifiability, and traceability to higher-level goals or requirements.

Verification asks whether the requirement has been specified and the product built correctly against it. Validation asks whether the requirement and resulting product address the stakeholder’s actual need in the intended environment. A team might verify that a report loads within three seconds and still have built the wrong solution if users needed real-time alerts rather than reports.

For each requirement, establish a verification method—such as test, inspection, analysis, demonstration, or review—and define the condition, starting state, expected result, measurement, owner, and evidence. For example:

Requirement: The system shall lock an account after five unsuccessful authentication attempts within 15 minutes.
Verification: Perform five invalid attempts for the same account within the defined window; confirm that the next attempt is rejected and that the event is logged.

Trace requirements in both directions. Upward links show which stakeholder need, business goal, regulation, or parent requirement justifies an item. Downward links show how it is allocated to design, implementation, tests, deployment controls, or operational procedures. This makes unsupported additions, uncovered needs, and the impact of change easier to see. A link is useful when it explains why a requirement exists and how fulfillment will be demonstrated—not merely because a traceability field is populated.

Review requirements with the people who understand the need, the people who will implement and operate the solution, and those responsible for verification. Scenario walkthroughs, models, prototypes, analysis, and test-design reviews can expose different misunderstandings. NASA’s software engineering handbook notes the value of formal inspections and multiple viewpoints for shared understanding. Relevant stakeholders should resolve conflicts; unanimous agreement is not always realistic or necessary.

A practical requirements health check

For a fast first pass, choose a representative set of high-risk or frequently changed requirements and ask:

  1. Clarity: Is there one clear obligation, with defined terms, actors, conditions, and thresholds?
  2. Completeness: Are important user groups, exception paths, constraints, quality attributes, and operational needs represented?
  3. Right level: Does the statement describe an outcome, or is a design choice being imposed without justification?
  4. Consistency: Does it conflict with another item, use terms differently, or duplicate content without purpose?
  5. Feasibility: Can it be delivered within technical, financial, schedule, legal, and operational constraints?
  6. Verification: Can it be tested, inspected, analyzed, demonstrated, or reviewed, with known evidence?
  7. Traceability and change: Is its source known, and can its downstream impacts be found?

Record unresolved questions with owners and due dates. Then review the highest-risk items with representative stakeholders, implementers, operators, and testers. A tool can preserve links, history, workflows, and evidence, but it cannot decide whether the team has understood the right need. That remains analysis work.

For medical, aviation, automotive, defense, financial, and other regulated or safety-critical contexts, apply the relevant domain standards and lifecycle controls. The same five failure patterns matter, but required evidence, independence of review, change control, and baselining may be considerably stricter than in an informal product backlog.
